From the case records of the Istituto Nazionale Tumori of Milan from 1956 to 1976, 49 cases of
metastases
in axillary lymph nodes from an occult
carcinoma of the breast
were selected. Forty-four patients underwent removal of the suspicious breast. In 11 patients no tumor was found at the pathologic examination. The survival of these patients was no different from the survival of those in whom the tumor was found. The 5-year survival of the total series was 87%, confirming that this type of presentation of breast cancer has a prognosis that is better than that generally reported for stage II

Only patients with

who have a good prognosis after curative surgery are the best candidates for reconstructive surgery. On the other hand, patients who have a poor prognosis should not be considered for reconstructive breast surgery. Patients with guarded and intermediate prognoses who are candidates for adjuvant chemotherapy programs may be considered for reconstruction after completion of their programs and not prior to or during the programs. They should be evaluated at the end of such program, which ranges from one to two years, to assure absence of occult

develop at a later date (after insertion of the prosthesis), tissue sloughing may occur at the site of the implant that may delay therapy or cause overwhelming infection.

The estrogen receptor (ER) level in
carcinoma of the breast
is a useful predictor of response to hormonal therapy.
Metastatic disease
may not have the same level of ER as the primary. In a series of 37 patients who had simultaneous ER determination in both primary neoplasm and regional nodal metastasis there was 81% agreement. The true ER character of the tumor would have been missed in seven patients if only the primary tumor had been sampled. The possible reasons for this discrepancy are discussed. We feel that the nodal metastatic deposit may reflect the true nature of the ER status because it represents a purer concentration of tumor cells as well as representing the aggressive element of the tumor.

The bone scan has become a major staging and screening test for patients with
carcinoma of the breast
. The routine use preoperatively of bone scans to detect occult osseous
metastases
has been advocated to prevent radical operations on patients with Stage IV disease. Periodic use post-operatively of bone scans has also been advocated to detect metastic disease before the onset of symptoms. In view of the prevalence of this disease, the implications of this approach in terms of cost alone warrant consideration. All bone scans obtained of patients with

during a five year period were reviewed and the results correlated with the pathologic stage of the disease, the presence or absence of symptoms and pertinent clinical data. I-131-labeled antibodies to carcinoembryonic antigen (CEA) were administered subcutaneously in the finger webs. Subsequent scintigraphic immages demonstrated localization of radioactivity in the ipsilateral axillary

of all patients and in the contralateral axillae of three. Fifteen patients with either gastrointestinal or genitourinary cancers were studied as controls; in 12 both the hands and feet were injected with antibodies to CEA and in the other three either the hands or feet. Radioactivity was observed in the inquinal nodes of four control patients with tumors below the diaphragm and in the axillary nodes of one patient with a tumor above the diaphragm. She had considerable bleeding from the lesions and had to be given repeated blood transfusions. Radiotherapy was without effect and she died of pulmonary emboli 3 months after the diagnosis of angiosarcoma was made. No sign of distant

, 0ut a small carcinoma was found in the right breast. On going through the literature, the author has found 206 cases of Stewart-Treves Syndrome (development of angiosarcoma in post-mastectomy lymphoedema), 7 of which were from Scandinavia.

Every year in the German Federal Republic there are 15000 new cases of
carcinoma of the breast
in woman. In those cases in which
metastases
in the lymph nodes have not appeared, the rate of recovery is so high that a rehabilitation is worth careful consideration in each case. Methods of operation have been worked out over years of experimentation, with results that are now so good that not only breast-like forms, but genuine female breasts are created. With the reconstruction possibilities, the quality of the patients' survival time is improved.
