Using the indirect immunoperoxidase method, alpha 1-fetoprotein (AFP) can be demonstrated in paraffin sections of tumours by light-microscopy. Among 160 tumour preparations it was demonstrated in five of six endodermal sinus tumors, 19 of 40 teratomas, the three hepatoblastomas and 9 of 16 primary liver-cell carcinomas. Among 15 rhabdomyosarcomas, 15 Wilms tumours, 15 neuroblastomas and 15 other malignant tumours only one rhabdomyosarcoma contained AFP. No AFP was demonstrated in 35 various benign tumours and various other human tissue samples. Distribution of AFP-producing cells in tumour tissue is generally focal samples. In malignant teratomas AFP was found only in areas which corresponded to an endodermal sinus tumour or an embryonic carcinoma. The practical significance of the results consists of (1) demonstration and localisation of AFP in tumour tissue; (2) aiding the differential diagnosis and classification of tumours and their metastases; and (3) recognition of pathogenetic connection between embryonic carcinoma and endodermal sinus tumour.

Sixty postmenopausal patients with microscopically confirmed, progressive mammary carcinoma and local recurrence alone or in combination with distant metastases or with distant metastases only, not previously treated with cytostatics, were given tamoxifen (Nolvadex), 10 mg 3 times daily. The response rate in 37 evaluable patients was 48 per cent, with a median duration of the response of 26+ weeks. The effectiveness of the drug increased significantly with an increasing interval between primary treatment and the appearance of the first recurrence. No serious side effects occurred.

Authors have demonstrated presence of lipide droplets in the tumour cells in 16 cases out of 22 of the carcinoma of prostata. In 8 cases the tumour was of the xanthomatous variant of Kinoshita. Metastases were examined in 4 cases. Cells of the metastatic tumours contained lipide alike the primaer ones. Authors believe, that lipide content of the metastatic carcinoma may help in recognition of the original tumour, i.e. prostatic carcinoma.

Disappointing results of radical mastectomy for treatment of inflammatory breast carcinoma led to its abandonment and the use of alternative therapeutic methods without improvement of survival rates. Results of radical mastectomy combined with other therapeutic modalities have not been fully evaluated so far. In a series of nine patients with proven inflammatory breast carcinoma and no distant metastases, two underwent radiotherapy, oophorectomy, and adrenalectomy (group A) and seven underwent preoperative irradiation, radical mastectomy, postoperative irradiation, and chemotherapy (group B). There were no local recurrences in either group. Group A patients survived five and eight months (mean, 6.6+/-2.1) and patients of group B survived 45.5+/-26.2 months (p less than 0.05). Results show no rational basis for withholding radical mastectomy, but suggest that improved survival may be obtained when radical mastectomy is an integral part of a rational sequential therapeutic schedule.

Retrospective review of 160 patients with carcinoma of the supraglottic larynx was accomplished for the period 1964 to 1974. The majority were treated by low dose preoperative radiation and conservation surgery and the rest by preoperative radiation and total laryngectomy or radiation alone. Relapse-free and actuarial 5 year survival for all patients was 71% and 45%, respectively. Of primary and neck failures, 85% occurred within 2 years. Primary, nodal, and distant failures occurred in 11%, 21%, and 14% of all patients, respectively. Of 40 T and/or N failures, 33 (83%) occurred in the neck and three T and six N failures (23%) were surgically salvaged. Voice was preserved in 71% of the patients. Major complications were noted in 16% and minor complications in 26% of surgically managed patients; operative mortality was 4%. The presence of microscopic lymph node metastases best predicted those who would later develop T and/or N failure and distant metastases. Second primary tumors occurred in 26% and the leading cause of death was tumor.

A case of female urethral diverticulum is reported by the authors. It is a rare disease and about 31 cases have previously been described. An analysis is made in relation to the diverticulum of the urethra and to female urethral carcinoma. Emphasis is given on the semiology and the recently acquired diagnostic procedures, as these have led to an increased number of recognized cases. In the case presented, an extended resection and a definitive urinary derivation was performed, together with chemo- and radiotherapy. A follow-up of 2.5 years showed no signs of recurrence or metastases.

In six patients, unusual metastases developed after definitive therapy for carcinoma of the cervix; three cutaneous, two biliary and one duodenal. The prognosis when such metastases develop is grave, although one patient is alive and well without evidence of carcinoma two and one-half years after the detection of the cutaneous lesion. Although the surgical approach is usually not curative, it is worth while for relief of symptoms.

Carcinoma of the cervix or endometrium was evaluated in 1,021 patients at the Joint Center for Radiation Therapy, Boston, between July 1968 and December 1977. The patients were retrospectively evaluated for the presence of lung metastases, appearing initially or during their disease course. On chest radiography, 42 patients were found to have metastases. Lung metastases were seen in 5.1% of patients with carcinoma of the cervix and in 3.6% of patients with carcinoma of the endometrium. Median time from initial disease staging to detection of lung metastases was 12 months. Once pulmonary spread was discovered, 80% of patients expired within 1 year. Lung nodules varied greatly in size. In 11 patients they were solitary; five patients had pleural effusions; three had mediastinal or hilar adenopathy; and none had excavation.

The mucosa of the cheek-pouch of five golden Syrian Hamsters was irradiated with the beam of an argonionlaser (diameter 0.5 mm and 200 mW). The effect on the micro- and macro-vessels and the bloodstream, especially of the microcirculation, is studied intravitalmicroscopically. In the microcirculation and also in the small and large venous and arterial vessels a restriction of bloodstream, praestasis, pendulumstream, stream reversal and some white microthrombi, -- emboli and agglutination of erythrocytes are visible. The bloodless welding of the vessels takes place even before the evaporation and coagulation necrosis of the surrounding epithelium of the mucosa as a result of the relatively selective effect of the Argon Laser on the reddish vessel-system by a continuous narrowing of the vessels-walls to the complete loss of their lumen rather than by a thrombosis. Firstly the capillaries, the venoles and the arterioles are obliterated, then those of middle size and at last the large venae and arteries in which case large vessels function for a moment as smaller ones. A continuously growing imbibition with increasing duration of the laser irradiation can be observed. Three clinically important phenomenon, depending on the laserdose can be observed: 1. a partial, 2. a subtotal and total obliteration of the network of the vessels, and 3. a total destruction of the blood-vessels and also of the tissue. Therefore clinically a subtotal or total network obliteration of the blood-vessels of strongly vascularised tissues, haemangiomas, teleangiectasias in case of morbus Rendu-Weber-Osler and livid enlarged nasal conchae might be obtained. Further a total tissue-destruction with a break in the continuity of the tissue for example with strongly vascularised carcinoma or its metastases might be effected.

The in vitro cytotoxicity of lymphocytes from patients suffering from breast carcinoma against autochthonous, allogeneic and established breast carcinoma cells was evaluated. Lymphocytotoxicity to breast carcinoma cells was observed in all stages of the disease. Control lymphocytes from healthy donors or patients suffering from other carcinoma are not cytotoxic for the breast carcinoma cell lines. A follow-up study of the cell mediated immune reactions before and after surgical removal of the breast carcinoma showed that the cytotoxic lymphocyte population which is demonstrated in the presence of the tumor disappears quickly after excision of the carcinoma. The non-reactivity of lymphocytes is not due to a general immune defect. Serum of the tumor bearers did not block the lymphocytotoxicity in early stage breast carcinoma; in metastatic disease inhibition occurred in more than half of the cases. Preincubation of lymphocytes with antigen preparations of allogeneic breast carcinoma cells did not inhibit the cytotoxicity to breast carcinoma cells whereas autologous serum preincubated with the antigen preparation diminished the lymphocyte reactivity to the target cells in some cases.
