This female patient was born in 1935 and died in 1976 with a malignant bone tumor involving the proximal humerus following multiple local recurrences, axillary lymph node
metastases
, and pulmonary
metastases
. Histologically, over the course of time, there was an increase in features of an atypical Ewing's sarcoma, at the expense of findings of a typical, extra-cranial so-called
adamantinoma
. In contrast to another report, our case showed neither epithelial nor endothelial features on fine structural examination, but rather primitive mesenchymal cells, as is described for Ewing's sarcomas.

Forty-nine cases of primary tumors of the mandible have been reviewed. The anatomic location, pathologic features, sites of
metastases
, survival rates, and treatment methods were evaluated. Lesions studied included
ameloblastoma
, osteogenic sarcoma, reticulum cell sarcoma, fibrosarcoma, chondrosarcoma, myxosarcoma, epidermoid carcinoma, adenocarcinoma, and giant cell sarcoma. An in-depth discussion of primary osteogenic sarcoma of the mandible is presented. Because of upper cervical lymph node
metastases
in two cases of osteogenic sarcoma of the mandible, an upper neck dissection should be considered in the primary treatment. Also presented in this study are the first reported cases or primary myxosarcoma of the mandible and giant cell sarcoma of the mandible. Recent methods of treatment of ablative resection of the mandible followed by immediate or delayed repair are discussed. A revised technic for mandibular replacement which has met with success in six of seven cases is presented.

A young male had an assumed dentigerous cyst marsupialized and later a recurrent
ameloblastoma
resected. Eleven years later he was admitted with renal stones and hypercalcemia and
metastases
of the
ameloblastoma
in the left lung were discovered. Death occurred as a result of spinal and hepatic spread of his tumor and thrombosis of the renal veins. Renal calcification was demonstrated. The possible causes of the hypercalcaemia which was not associated with a raised serum parathormone or affected by parathyroidectomy is discussed.

A patient with
ameloblastoma
of the mandible with histologically confirmed pulmonary
metastases
9 years after onset of tumor is described. The effectiveness of three chemotherapeutic agents (cyclophosphamide, methotrexate, Adriamycin), each given alone intravenously, were evaluated. Marked symptomatic improvement was noted with Adriamycin therapy. Ten previous cases of metastatic
ameloblastoma
are reviewed. The incidence of
metastases
cannot be predicted on the basis of histology. Three commonly discussed modes of metastasis are via hematogenous and lymphatic routes and the unusual mechanism of aspiration of tumor cells.

Partial mandibulectomy was performed for the treatment of benign or malignant oral tumors in 142 dogs. Forty-two dogs with a benign tumor (
ameloblastoma
) had a 22.5 month (range, 6 to 74 months) median disease-free interval, with a 97% 1-year survival rate; there was local recurrence in one dog. Twenty-four dogs with squamous cell carcinoma had a disease-free interval of 26 months (range, 6 to 84 months), with a 91% 1-year survival rate; recurrence and metastasis developed in two dogs and
metastatic disease
in one dog. Based on survival curves, 37 dogs with a melanoma had a median survival time of 9.9 months (range, 1 to 36 months), with a 21% 1-year survival rate; 20 dogs died or were euthanatized for recurrent or
metastatic disease
. Twenty dogs with osteosarcoma had a median survival time of 13.6 months (range, 3 to 28 months), with a 35% 1-year survival rate; nine dogs died or were euthanatized for recurrent or
metastatic disease
. Nineteen dogs with fibrosarcoma had median survival time of 10.6 months (range, 3 to 32 months), with a 50% 1-year survival rate; 12 dogs died or were euthanatized for recurrent or
metastatic disease
. Results of this and previous studies demonstrated that partial mandibulectomy was effective in prolonging survival and decreasing recurrence for squamous cell carcinoma and
ameloblastoma
. Progressive disease and corresponding low survival times were common in dogs with melanoma, osteosarcoma, and fibrosarcoma. There were no differences in survival times or the progression of disease among five partial hemimandibulectomy procedures. The high rates of recurrence and metastasis in dogs with these tumors suggest a need for evaluation of ancillary chemotherapy and local radiation therapy to decrease the prevalence of progressive disease.

Two cases of malignant
ameloblastoma
with
metastases
to the lungs are reported. One originated in the mandible and the other in the maxilla. The doubling time of the metastatic lesions to the lungs were calculated according to Collins' method, and ranged from 129 to 201 days.

Ameloblastoma
is an aggressive locally recurring neoplasm of odontogenic epithelium. We describe a case of a mandibular
ameloblastoma
with a 17-year history of local recurrences followed by two metachronous intracranial
metastases
. Central nervous system metastasis without pulmonary involvement is previously unreported in a living patient with
ameloblastoma
. The behavior of this tumor qualifies it as a malignant
ameloblastoma
.

Adamantinomas of long bones are rare primary malignant bone tumors. A case of a woman who died of pulmonary
metastases
of an
adamantinoma
of the tibia is presented. A unique feature of this case is the association with hypercalcemia. The association of hypercalcemia, hypophosphatemia, decreased parathyroid hormone levels and increased urinary cAMP excretion suggests a humorally mediated hypercalcemia. Histologic and ultrastructural analysis of the pulmonary
metastases
demonstrated that the tumor was composed of a heterogeneous cell population with mesenchymal and epithelial differentiation.

Odontogenic carcinomas of the jaws are classified as malignant
ameloblastoma
, ameloblastic carcinoma or primary intraosseous carcinoma. Because these lesions are extremely rare, microscopic diagnosis is difficult. An aggressive case of ameloblastic carcinoma of the mandible is presented. In spite of radical surgery and radiotherapy, the patient expired eight months following initial diagnosis. - A review of the literature seems to indicate that so called simple ameloblastomas rarely can dedifferentiate and
metastasize
following multiple inadequate surgical procedures. Although radical surgery is not necessary, local excision should be thorough. - Ameloblastic carcinoma and primary intraosseous carcinomas may be histogenetically similar. They are highly malignant tumours which should be treated aggressively.
Metastasis
is common and prognosis is poor.

Ameloblastoma
of the jaw is an aggressive benign tumor of epithelial origin that has generally been treated surgically even for
metastases
. This article is a review of the literature and a report of a previously unpublished case. Despite earlier reports, it is suggested that the available data clearly indicate that it is a radiosensitive tumor. In this report, specific recommendations for radiation therapy are outlined, and guidelines for treatment planning, radiation dosage, and fractionation expected outcome and follow-up are given.
