Twenty-seven small cell carcinomas of the lung and three tumors of the large intestine with combined adenocarcinomatous and small cell and/or anaplastic carcinoid-type histologic features were studied by light and electron microscopy. showed small cell carcinoma. The combined tumors and the numerous other similar neoplasms described in the literature and reviewed here suggest an endodermal origin for digestive and respiratory tract APUD cells based on the hypothesis that cancer is a clonal proliferation, and mucous and squamous cell differentiation is an endodermal rather than neural crest characteristic. The ultrastructural features of tumors of cells of known neural crest origin, including a medullary carcinoma of the thyroid, three carotid body tumors, a pheochromocytoma, and two cutaneous melanomas were compared with those of other APUD cell tumors including small cell carcinomas of the lung, two bronchial carcinoids, a carcinoid of the appendix, and a carcinoid of the kidney. Cells of the latter group sometimes possessed cytoplasmic tonofibrils, round compact masses of cytoplasmic microfilaments, and ductal lumina. These features were lacking in the former group and may signify a different embryologic origin. The histologic, histopathologic, and embryologic evidence regarding the origin of digestive and respiratory tract APUD cells is reviewed, showing that the former are, and the latter probably are, of endodermal and not neuroectodermal origin.

The liver was normal microscopically, with no evidence of bile duct obstruction. alpha-Fetoprotein in the tumor tissue was detected by immunoprecipitation reaction in agar. Regan isoenzyme and variant alkaline phosphatase were also detected in the tumor tissue and total alkaline phosphatase activity of the tissue was very high. These findings suggested their tumor origin.

An experimental model of gastric sarcoma was elaborated experimentally on 228 Wistar rats. Tumors were induced by single DMBA injections into the glandular stomach wall in rats or by securing a cellophane plate onto its anterior surface. Tumors developed in 95 rats. Most tumors would show a mesenchymatous origin (89.8% of cases) and may be defined as leiomyosarcomas partly polymorphocellular ones, and more rarely as fibro- and reticulosarcomas. Tumors of the

The in vitro spontaneous lymphocyte rosette (T cell) assay was used to determine cellular immunologic competence in 112 patients with bronchogenic carcinoma. Among preoperative patients with localized tumors. T cell levels were significantly lower than in 237 normal controls. With advanced stages of disease, T cell levels declined progressively among patients with squamous cell carcinoma, oat cell carcinoma, and undifferentiated carcinoma, but not among patients with
adenocarcinoma
. Squamous carcinoma patients considered cured had persisting low T cell levels, but cured
adenocarcinoma
patients had normal levels. Serial determinations that showed a fall in T cell levels preceded the development of clinically evident
metastases
by an average of 2.5 months. Postoperative patients with rising T cell levels have remained clinically free of disease. The data indicate that T cell levels correlate with extent of tumor and clinical course of patients with bronchogenic carcinoma. The assay may, therefore, provide a rational basis for the selection of patients who are at high risk for the development of recurrence after surgical resection and who may benefit from the early institution of adjunctive therapy.
