Sensitizing and neurotoxic effect of ornidazole, was tested in a double-blind randomized study in patients with carcinoma of the cervix and larynx. Ornidazole or placebo were given orally, two times weekly, for 3 weeks. Dose was 2.5 g/m2 for each administration. Total dose given was 15 g/m2. Radiation therapy was given 3 h after the drug administration. Ornidazole was well tolerated in the majority of the patients. No neurotoxic side effects, such as peripheral neuropathy or convulsion, were observed with a total dose of up to 30 g. Dizziness, somnolence and nausea were the prominent acute side effects, seen mostly (70%) in women. In the placebo group this rate was 17% (p less than 0.01). No important side effect was observed in men receiving ornidazole. Serum concentration of ornidazole reached the maximum level in 2-4 h after oral administration and ranged (23 patients) from 65.1 to 139.8 micrograms/ml. Mean half-life was 15.6 +/- 2.8 h. Peak concentration in tumour tissue was achieved 1-3 h after the administration, ranging from 13.0 to 78.0 micrograms/g. Tumour concentration of ornidazole ranged from 14 to 93% of the serum concentration at the time of irradiation.

A randomised prospective clinico-laboratory evaluation of the efficacy of ornidazole versus clindamycin in anaerobic infections was performed in 140 patients; 67 were given ornidazole and 73 received clindamycin. Patients were mainly suffering from peritonitis, pelvic cellulitis, endometritis, soft tissue infections and abdominal abscesses, which were distributed rather equally in both groups. Ornidazole was administered at a dose of 500 mg every 12 h i.v. or/and orally, and clindamycin 600 mg every 8 h i.v. for 7-60 days. In pus cultures, Escherichia coli and Bacteroides fragilis were the main isolates. The coexistence of aerobes necessitated the addition of an aminoglycoside in 111 patients, while six times chloramphenicol had proved ineffective against anaerobes. Between the two groups no statistically significant difference was found in the excellent response rate, although the overall cure rate was superior in the ornidazole group (80.6 vs. 68.5%), with a prompt response within less than 48 h in the case of ornidazole. Side effects necessitating discontinuation of chemotherapy included severe nausea in 1 patient treated with ornidazole and diarrhea in 8 patients given clindamycin.

Ornidazole (Tiberal La Roche) was used in 50 persons infected with Giardia intestinalis in the doses of 500 mg twice a day during 5 days. Examinations were performed after 7-10 days, having been repeated one month and 3 months later. A positive results were achieved after one month in 46 cases (92%) and after 3 months in 45 cases (90%). Tiberal is thought to be effective drug in the treatment of Giardiasis, causing no side effect apart from headache and nausea occurring sporadically.
