Thirty-nine patients with small cell carcinoma of the lung were treated sequentially with induction chemotherapy, radiotherapy and then maintenance chemotherapy. Induction chemotherapy consisted of two regimens, cyclophosphamide, vincristine, methotrexate (COM) and adriamycin, ACNU, vindecine (ANV) given by randomization. Radiotherapy was given for patients with limited disease (LD) as a rule. After radiotherapy the drugs used for maintenance chemotherapy were alternated and reduced in dose. Eighteen patients were treated with a COM-ANV sequential combination and sixteen patients were treated with an ANV-COM combination. Thirteen patients had limited disease (LD) and eleven patients had extensive disease (ED). Of 12 patients with LD treated with COM-ANV therapy, 9 patients (75%) responded with 3 (25%) complete responses. Of 11 patients with LD treated with ANV-COM therapy, 9 patients (81.8%) responded to the therapy. According to disease extent, response rate was 82.6% for LD and 54.5% for ED. The median survival times were 9 months for patients with COM-ANV therapy and 12 months for those with ANV-COM therapy. Also, the median survival time was 15 months for LD patients and 5 months for ED patients. Major toxicities in ANV therapy were anorexia, nausea, and myelosuppression, and were more frequent than with COM therapy. These results showed no clear evidence of superiority in either the COM-AMV or ANV-COM regimen.

A total of 39 patients with non-small cell carcinoma of the lung (NSCL) were treated with cisplatin, etoposide, and mitomycin. A major response rate (complete response + partial response) was seen in 15 patients (39%). Median survival for all patients was 340 days; median survival of the responding group was 514 days. Toxic effects included moderate hematologic toxicity, nausea, and vomiting. There were no treatment-related deaths. This regimen clearly is effective in treating NSCL.

Ethylenediamine platinum(II) malonate [JM-40 (NSC 146 068)] has been selected for clinical studies because of its favorable preclinical toxicity profile as a "second generation" platinum analogue. When compared to cisplatin, JM-40 was less emetic in the ferret and less nephrotoxic in the dog, while its antitumor activity approached that of cisplatin. Twenty-nine patients received 86 courses of JM-40 as a single dose every 3-4 wk. After 13 dose escalation steps the maximum tolerable dose was reached at 1200 mg/m2. The dose limiting toxicities were nausea, vomiting, and nephrotoxicity. The renal damage seemed reversible up to a dose level of 1000 mg/m2 and consisted of a glomerular and tubular dysfunction. JM-40 did not cause any other dose related side effect or myelo-suppression. Pharmacokinetic studies at a dose of 1000 mg/m2 revealed mean terminal half-lives of 5.0 and 1.9 days for platinum in plasma and plasma ultrafiltrate, respectively. The mean cumulative excretion of platinum in urine accounted for 57% of the dose up to day 5. Two partial responses were observed in a patient with a large cell carcinoma of the lung and in one with a carcinoma of the lacrimal gland. Limited evaluation of JM-40 in phase II studies is warranted. The recommended dose is 1000 mg/m2 every 4 wk and 800 mg/m2 for patients pretreated with platinum analogues.

Tiazofurin (2-beta-D-ribofuranosylthiazole-4-carboxamide, TCAR, Riboxamide, NSC 286193) is a novel C-nucleoside with antitumor activity against several murine tumor models, including Lewis lung carcinoma. The mechanism whereby this compound exerts its antineoplastic effects is most likely related to a state of guanine nucleotide depletion whereby the anabolite, thiazole-4-carboxamide adenine dinucleotide, potently inhibits inosine-5'-monophosphate dehydrogenase. This Phase I study was designed to determine the maximally tolerated dose of Tiazofurin administered on a 5-day, every-28-day schedule. Tiazofurin levels were measured using a high-pressure liquid chromatography assay, and pharmacokinetic studies were performed in patients treated at each dose level. Nineteen patients received a total of 24 courses of the drug in doses ranging from 550 to 2200 mg/sq m. The dose-limiting toxicities were pleuropericarditis and a general illness best described as a "viral-like" syndrome (manifested by severe malaise, headaches, myalgias, fever, nausea, vomiting, and diarrhea). Other toxicity included myelosuppression, hyperuricemia, elevated serum creatine phosphokinase and serum glutamic oxaloacetic transaminase, conjunctivitis, mucositis, and desquamation of the palms of the hands. Plasma clearance of Tiazofurin followed a biexponential pattern with a harmonic mean terminal half-life of 7.6 h. The mean volume of distribution at steady state was 30 liters/sq m, and the mean plasma clearance was 3 liters/h/sq m. The total cumulative urinary excretion ranged from 15 to 49%. The maximally tolerated dose of Tiazofurin on a 5-day schedule was 1650 mg/sq m. The recommended dose for Phase II evaluations is 1100 mg/sq m for 5 days. However, exploration of other schedules which might allow administration of more Tiazofurin combined with biochemical studies including thiazole-4-carboxamide adenine dinucleotide measurements would be desirable.

The effects of chemotherapy by single administration of Cisplatin were studied in 26 patients with non-small cell carcinoma of the lung. There were 22 males and 4 females with a median age of 63 years. 21 cases were epidermoid carcinoma, 4 cases were adenocarcinoma and 1 case was atypical carcinoid. Cisplatin 20 mg/m2 with hydration and diuresis was given daily for 5 consecutive days. The course was repeated every 3 weeks. Partial response was observed in 4 patients (3 epidermoid carcinoma and one atypical carcinoid). The response rate was 19%. Side effects induced by Cisplatin were gastrointestinal toxicity including vomiting, nausea and appetite less, bone marrow toxicity and renal damage. These were not so severe, and reversible. Gastrointestinal toxicities were controlled successfully by corticosteroids. In 12 patients gastrointestinal toxicities were not observed. We conclude that Cisplatin is effective for non-small cell carcinoma, especially for epidermoid carcinoma. Hematologic toxicities of Cisplatin were not so severe. Therefore, combination chemotherapy including Cisplatin would improve the quality of life of patients suffering from non-small cell carcinoma of the lung.

Twenty-nine patients with small cell carcinoma of the lung were treated with a combination therapy consisting of vincristine 1 mg/m2 i.v. day 1, etoposide 200 mg/body p.o day 1-5 and cyclophosphamide 500 mg/m2 i.v. day 1 (VEC) in 3 week interval. After 2 courses of VEC, four out of 10 patients with limited disease (LD) showed a complete response (CR), while 7 out of 19 patients with extensive disease (ED) obtained a CR. Six patients with LD and 6 patients with ED received subsequent radiotherapy to primary tumors and to regional lymph nodes, and 5 with LD and 3 with ED attained a CR. Overall complete response was observed in 9 patients (90%) in LD and 10 patients (53%) in ED. The median duration of survival was 17 months in complete responders with LD and that was 12 months in those with ED. Dose limiting toxicity of the VEC regimen was leukopenia which occurred in most patients and thrombocytopenia was less frequent. Non-hematologic toxicities containing alopecia, numbness, nausea and others were acceptable and well tolerated.

The syndrome of tumor-induced osteomalacia has been previously thought to occur only in association with mesenchymal tumors, although one report has linked prostatic carcinoma with the syndrome. We report the case of a patient who presented first with the clinical and biochemical features of the syndrome of inappropriate antidiuretic hormone secretion, and then oncogenic osteomalacia. The first syndrome was characterized by headaches, nausea, and vomiting; serum sodium determinations ranged between 107 and 118 meq/L with simultaneous urine spot sodium concentrations of 100 to 116 meq/L. The circulating antidiuretic hormone level was markedly elevated to 261.5 microU/mL. The osteomalacia was discovered incidentally when depressed serum phosphorus levels of 1.2 to 1.7 mg/dL were noted in association with 24-hour urine phosphorus excretion exceeding 1000 mg/24 h. Undecalcified tetracycline-labeled bone biopsy samples confirmed oncogenic osteomalacia. Only afterward was a small-cell carcinoma of the lung identified as the likely source of both of these syndromes.

Twelve patients with unresectable and recurrent non-small cell carcinoma of the lung were treated with a combination chemotherapy consisting of cis-platinum and adriamycin. The overall response rate was 42% with two complete responses and three partial responses. The durations of response were 9+ and 8.5+ months in two complete responses, and 7.5+, 5.5+ and 2.5 months in three partial responses. The overall median survival was 8 months ranging 3.5+ to 16+ months. Three patients had leukopenia less than 2,000/mm3, two patients had thrombocytopenia less than 100 X 10(3)/mm3, and four patients had hemoglobin decrease of 3 g/dl or more. Severe nausea occurred in nine patients, whereas vomiting did in seven patients. Nearly total alopecia was observed in all patients. Renal toxicity in four patients was also observed, but was reversible. We believe that the combination chemotherapy with cis-platinum and adriamycin is promising in the management of patients with non-small cell carcinoma of the lung.

A phase II trial of 5'-deoxy-5-fluorouridine (5'-DFUR), a new fluorinated pyrimidine analog which has been demonstrated to have potential superiority over 5-FU and tegafur for chemotherapy of murine tumors, was performed in patients with advanced non-small cell carcinoma of the lung and metastatic pulmonary tumors. 5'-DFUR at a dose of 800 mg/m2 was given per os every day for more than four weeks. None of 15 evaluable patients with non-small cell carcinoma of the lung and 15 evaluable patients with metastatic pulmonary tumors showed a complete or partial response. Toxic effects of 5'-DFUR included anorexia (29%), diarrhea (26%), nausea (23%), vomiting (10%), leukocytopenia (10%), general fatigue (10%), liver disorder (6%) and thrombocytopenia (6%).

Thirty-eight patients with advanced measurable non-small cell carcinoma of the lung (20 adenocarcinoma, 13 epidermoid carcinoma, 5 large cell anaplastic carcinoma) were treated with alpha(leukocyte)-interferon. Patients received 3 X 10(6) units intramuscularly 5 days out of 7. Patients were treated for 12 weeks or as modified for disease progression or positive response to therapy. In 37 patients evaluable for response, one partial response was observed (adenocarcinoma). Toxicity included fever and malaise, leukopenia, thrombocytopenia, nausea-vomiting, and hepatic toxicity. One additional patient with previous cardiac and pulmonary disease had a cardiorespiratory arrest several hours after his first interferon injection. The relationship between those events is not clear. As administered, alpha-interferon showed no meaningful activity as therapy for non-small cell carcinoma of the lung.
