In this study, the authors describe certified nursing assistants' (CNA) Explanatory Models (EMs) of depression and aspects of their EMs that may contribute to the underdetection of depression in nursing homes. Interviews with 18 CNAs working in two nursing homes are guided by Kleinman's Explanatory Models of Illness framework. Interview data are content analyzed and CNAs' descriptions of depression are compared to the

-IV-TR diagnostic criteria for depression. The result is that the CNAs are unsure about the duration and normalcy of depression in residents. Although they have no formal training, CNAs feel responsible for observing for signs of depression and describe verbal and nonverbal ways of interacting when providing emotional care to residents. CNAs hold potential to improve the detection of depression and contribute to the emotional care of residents. Attention to staff knowledge deficits and facility barriers may enhance this potential.

There is a movement towards a dimensional classification of personality disorders (PD). However, data linking dimensional systems and the categorical system for classifying PD are lacking. In the present study, N = 165 normal subjects and N = 222 nonpsychotic in-patients (including N = 81 patients with a PD diagnosis) completed the Dimensional Assessment of Personality Pathology-Basic Questionnaire (DAPP-BQ) measuring 18 PD traits.

-IV PD symptoms were assessed by SCID-II interviews. Group differences were analyzed by ANCOVA, and the relation between the dimensional and categorical approach was investigated by regression, ROC, and

analyses. Patients with PD exhibited elevated scores on all DAPP traits compared with controls. Patients without PD scored in between. Each

-IV PD could be described by a distinct profile of DAPP traits. Results support the assumption that the DAPP trait system can represent mean differences between clinically defined subgroups. The categorical system can be mapped onto the dimensional DAPP system with sufficient clinical specificity.

The diagnosis of Parkinson's disease with dementia (PDD) is currently based on clinical criteria (
DSM
-IV,
MDS
-Task Force). In daily practice and research studies, these criteria still depend on the subjective impression of the examiner. Brief screening tests (BST) are helpful in identifying patients with PD with dementia, which can be difficult in patients with advanced PD. We aimed to develop a BST for PD, the PDD-Short Screen (PDD-SS), to accurately and quickly screen for PDD. In this prospective study, 70 patients with nondemented (age 73.8 +/- 4.4) and 32 demented (age 73.8 +/- 4.4) PD regularly attending a Movement Disorders Clinic were included. Diagnosis of dementia was based on
DSM
-IV criteria, CDR score >or=1, and PD-CRS total score <or=64. The PDD-SS, Mattis Dementia Rating Scale (MDRS), and Mini-Mental State Examination (MMSE) were administered to all participants. Validity, reliability, and discriminative power of the PDD-SS were examined. The final version of the scale included the items immediate and delayed verbal memory, clock drawing, alternating verbal fluency, and a questionnaire covering cognitive and psychiatric (hallucinations, apathy) symptoms common in PDD. A cutoff score <or=11 on the PDD-SS yielded high sensitivity (89.8%) and specificity (88.5%) for diagnosing PDD. The MDRS displayed similar accuracy, but the PDD-SS administration time was significantly shorter (4.8-6.9 vs. 17.5-25.2 minutes). Diagnosis of dementia using the PDD-SS was not influenced by age, education, or motor function. The PDD-SS appears as the first BST for diagnosing PDD, displays an excellent diagnostic accuracy, and takes 5 to 7 minutes to be administered.
