. We report a case involving a 44-year-old woman who developed

5 months after hematopoietic stem cell transplantation for
myelodysplastic syndrome
. Despite aggressive medical management, the patient required intubation and was administered extracorporeal membrane oxygenation while awaiting recovery. However, her condition continued to deteriorate, and she subsequently underwent a double lung transplant with uneventful recovery. With the high mortality of medically managed
IPS
, lung transplant could prove to be lifesaving.
