Titles and abstracts of the 700 papers presented in the scientific sessions of the American Congress of Rehabilitation Medicine and American Academy of Physical Medicine and Rehabilitation from 1972 to 1976 were reviewed. There were 81 papers in 1972, 123 in 1973, 131 in 1974, 158 in 1975 and 207 in 1976. Two hundred thirty-seven (34%) of the papers were presented by 11 Research and Training Centers and 463 papers (66%) were presented by other institutions. One hundred fifteen papers were related to spinal cord injury, 84 to electromyography, 57 to stroke, 51 to muscle histology, physiology and chemistry, 49 to orthotics and 43 to urologic problems. , only one to
myelodysplasia
, 4 to burns, 8 to cancer, 13 to human sexuality and 10 to prosthetics. It is believed that appropriate funding of research and an emphasis on the neglected subjects through seminars and courses may provoke the curiosity of the research and encourage him to do research in important areas other than the few which are currently most popular.

The triple osteotomy of the innominate bone is a total redirectional reconstruction and is obtained by circum-acetabular osteotomies through the ilium, ischium and pubis. The method achieves coverage of a dislocated or subluxated femoral head where other iliac osteotomies are ineffective or incomplete. Triple osteotomy was performed on 175 hips including 121 congenital dislocation-subluxations. The patients' ages ranged from 6 to 35, with 70 per cent falling between 9 and 12. Eighty-six per cent had a satisfactory result. Most of the unsatisfactory results were in
cerebral palsy
,
myelodysplasia
and peroneal muscular atrophy and associated with uncontrolled muscle contractures or progressive disease.

A computerized pressure plate system was used to analyze the sitting pressure distribution and balance of 100 subjects with both normal and abnormal spines. Normal subjects had a balanced sitting posture with the weight taken evenly on each ischial tuberosity. Patients with idiopathic scoliosis showed significant sitting imbalance especially in the presence of lumbar curves. Bracing improved the sitting balance and stability in most patients. Abnormal sitting patterns were seen with the spinal deformities of
myelodysplasia
or
cerebral palsy
. Some patients with severe scoliosis had very asymmetric patterns, which correlated with their loss of sitting stability. The kyphosis of
myelodysplasia
produced an abnormal tripod sitting pattern due to pelvic flexion and a hyperlordosis of
cerebral palsy
a pubic sitting pattern due to pelvic extension. The patients studied after spinal fusion also had poor sitting balance and occasionally persisting decubitus ulceration.

We measured body composition, resting metabolic rate (RMR), and total energy expenditure (TEE) in a group of adolescents with
cerebral palsy
(CP) and
myelodysplasia
(M) aged 13- to 20-y-old using indirect calorimetry and the doubly labeled water method. Fat-free mass (FFM), RMR, and TEE were significantly lower in both the CP and M groups than comparable measurements in a control group of normal adolescent males and females. The ratio of TEE to RMR did not differ between controls and ambulatory M and CP subjects. However, TEE/RMR was significantly lower in the nonambulatory M and CP subjects than in controls (p less than 0.01). Our data indicate that energy requirements are reduced in both populations because both FFM and activity are decreased. Although energy requirements were decreased in both groups, the relationships between FFM and body weight differed. FFM and body weight were significantly correlated with RMR only in the M group. These data suggest that the type of paralysis in a handicapped population may affect resting energy expenditure.

Four patients with severe
cerebral palsy
, mental retardation, and seizures who were treated with valproic acid showed a broad spectrum of hematologic toxicity, which included thrombocytopenia, macrocytic red cells with or without anemia, and the Pelger-Huet anomaly in the segmented neutrophils, along with elevated vitamin B12 levels, normal serum folic acid levels, and elevated fetal hemoglobin values (two cases). Bone marrow findings in all four patients were abnormal, suggestive of a
myelodysplastic syndrome
. These hematologic findings have not been previously reported and are important for monitoring a patient on valproic acid therapy. The Pelger-Huet anomaly may be mistaken for an elevated band count, the macrocytic anemia appears not to be secondary to a vitamin B12 or folate deficiency, and the thrombocytopenia may be sensitive to drug dosage. The bone marrow changes appear to be a drug-related myelodysplastic phenomenon.

Previous research has shown that children with physical conditions involving the brain are at increased risk for psychopathology. It is unclear whether brain dysfunction leads to disturbance directly or whether it does so by increasing the children's vulnerability to environmental stress. I examined the vulnerability hypothesis in a sample of 157 children with
cerebral palsy
,
myelodysplasia
, or multiple handicaps and in 339 randomly selected controls. Data on psychopathology came from direct interviews with the children; data on the family environment came from mothers' reports. Physical disabilities were associated with significant increases in depressive symptoms and inattention. Family environment had a significant main effect on depressive symptoms; effect on disabled children was not significantly different from effect on controls. Family environment had no significant effects on symptoms of inattention in disabled children. The findings provided no support for the hypothesis that brain dysfunction renders children vulnerable to environmental stress.

Congenital dysplasia of the hip after conservative treatment does not always lead to satisfactory results. Remanent hip dislocation causes severe problems for the gait and static of the vertebral column. Here surgical treatment in early childhood (open reduction, intertrochanteric osteotomy and roof plasty, may produce an essential improvement, even if for a long term arthritis may be expected. But nowadays we have satisfactory techniques of arthroplasties with wear resistant ceramic selflocking prostheses for these cases. Besides on the CDH valuable surgical methods are available also in other hip diseases in early childhood (

, Perthes,
cerebral palsy
). Indication and surgery need special experience.

This study measures continuity in behavioral disturbance over a 5-year period among 255 children with physical disabilities who were 6 to 18 years old at first assessment. The pattern of the results differed from that reported previously for a sample of the general population of children. Disabled children showed little stability in aggression, an area in which stability over time had been consistently reported for the general child population. Furthermore, important differences were observed between children with cystic fibrosis and children with

, or multiple physical handicaps, all conditions involving brain abnormality. Children with cystic fibrosis followed the trend toward better adjustment seen among the general population of children. In contrast, among children with conditions involving the brain, the majority of those who had been classified as psychologically severely impaired remained at this level of impairment 5 years later. In two behavioral domains, Mentation Problems and Isolation, children with conditions involving the brain maintained, on the average, the same markedly high score they had initially. Among children with physical conditions involving the brain, Mentation Problems and Isolation signify persistent problems that do not subside as the children mature.

One hundred forty-nine Grice procedures performed between 1955 and 1982 were reviewed. The valgus deformities were secondary to poliomyelitis,

, flexible flatfoot, clubfoot, congenital vertical talus, and other neuromuscular diseases. Review consisted of radiographic and clinical follow-up. Overall evaluation revealed 46% unsatisfactory results because of both graft failure and residual clinical deformity. Previous unrecognized ankle valgus and instability prejudiced the results in the

and flexible flatfoot. Recommendations include consideration of the Grice procedure in poliomyelitis and

coupled with appropriate tendon balancing, but not in

We examined the impact of child disability on psychological distress in mothers, by comparing scores on two indexes of psychological distress of 369 mothers of children with cystic fibrosis,
cerebral palsy
,
myelodysplasia
, or multiple physical handicaps, with those of 456 mothers from a randomly selected sample of families ("control" subjects). Mothers of disabled children scored significantly higher than control subjects on both indexes of psychological distress. This finding persisted when the mothers' education, family income, and racial composition were controlled for. Type of disability, ie, the diagnostic classification of the disabled children, was unrelated to the mother's level of psychological distress. In contrast, the disabled child's dependence on others in daily activities had a significant effect on both measures of psychological distress; the more dependent the child, the greater was the mother's distress.
