A 25-year-old woman with active systemic lupus erythematosus and infective endocarditis was seen initially with porcine aortic bioprosthetic stenosis, perivalvar

, and native mitral regurgitation 9 years after aortic valve replacement for lupus endocarditis. Double-valve replacement was performed with St. Jude Medical mechanical prostheses. After operation the patient developed fever and an elevated white blood cell count. One month later she had increasing mitral and aortic perivalvular

and intermittent complete heart block. At reoperation both annuli showed evidence of continued infection, and she underwent annular reconstructions with pericardium and double-valve re-replacement. Cultures grew

hominis. Despite long-term therapy with appropriate antibiotics, within 2 months she developed recurrent perivalvar

with congestive heart failure. Orthotopic heart transplantation was performed. The postoperative course was notable for significant leukocytosis and spontaneous culture negative hemothorax that required thoracotomy for drainage. The patient recovered and is now well 14 months after operation.

Four cases of oesophageal stricture subsequent to doxycycline administration are reported. All cases were young to middle age (median age 3 years; range 1-7 years), and either domestic shorthair or domestic longhair breed. In all cases the predominant clinical sign was
regurgitation
, which developed at variable times after doxycycline administration. In all cases the reason for doxycycline use was treatment or prophylaxis of suspected infections (
Mycoplasma
haemofelis, Chlamydophila felis or Bordetella bronchiseptica), and the duration of therapy was variable. In one case the stricture was definitively diagnosed at post mortem examination, in the three other cases, definitive diagnosis was by endoscopy. Balloon dilation was successful in the three cases that were treated. This is the largest case series, to date, of oesophageal disease in cats associated with doxycycline administration. Caution should be exercised when administering oral medication to cats, especially doxycycline, and should be accompanied either by a water or food swallow.

A one-year-old, castrated male domestic short hair cat was admitted with a history of anorexia,
regurgitation
and pyrexia for two days. Fever and leukocytosis were identified. There were a large soft tissue density oval mass in the caudal mediastinum on thoracic radiographs, a fluid-filled oval mass in the caudal mediastinum on ultrasonography, and left-sided and ventrally displaced and compressed esophagus on esophagram. On esophageal endoscopy, there were no esophageal abnormalities. CT findings with a fluid filled mass with rim enhancement indicated a caudal mediastinal paraesophageal abscess. The patient was treated with oral antibiotics, because the owner declined percutaneous drainage and surgery. The patient was admitted on emergency with severe respiratory distress; and ruptured abscess and deteriorated
pleuropneumonia
were suspected. With intensive hospitalization care and additional antibiotic therapy, the patient had full recovery.
