The clinical and morphologic findings of three patients with metabolic acidosis, methylmalonic aciduria, and homocystinuria are presented. The clinical evolution of the patients was similar and was characterized in the first weeks of life by failure to thrive,
hypotonia
, and lethargy associated with pancytopenia and hepatic dysfunction, eventually progressing to severe respiratory insufficiency and renal failure consistent with a hemolytic-uremic syndrome. The patients died at 40, 45, and 75 days of age. Biochemical analyses and complementation studies revealed a congenital anomaly of vitamin B12 metabolism (cobalamin C disease). Postmortem morphologic findings in all three cases were dominated by a thrombotic microangiopathy of the kidneys and lungs, diffuse hepatic steatosis, and megaloblastic changes in the bone marrow. A severe
gastritis
with striking cystic dysplastic mucosal changes and total absence of parietal and chief cells was a consistent finding in all three cases, the rest of the gastrointestinal tract appearing essentially normal. Cobalamin C disease is an intracellular defect of cobalamin metabolism with possible recessive inheritance that can result in multiorgan failure early in life, with a thrombotic microangiopathy and unusual changes in the gastric mucosa.

The author has examined the possibility of a simultaneous combined examination of liver exosecretion and gastric secretion. A two-channel gastroduodenal tube has been employed. The findings evidence that pentagastrin (6 micrograms/kg) stimulation during the third phase of bile excretion potentiates the effects of cholecystokinetics administered in succession: 33% magnesium sulfate solution (0.5 ml/kg) and, after C bile is obtained (the fifth phase), olive oil (0.4 ml/kg). Continuous aspiration of the gastric juice prevents pancreatic stimulation and permits the measurements of gastric acid production and of the bile debit. gastritis
often presented with an increased volume of the residual B bile (the IVA phase), this evidencing the gall-bladder
hypotonia
. The 14 patients with inadequate gastric secretion have developed dyskinesias of the gall-bladder and bile ducts twice more often than the rest patients. The suggested method helps detect combined abnormalities of the stomach and the biliary system; it is satisfactorily tolerated by the patients.

The aim of this study was to assess the incidence of oesophageal abnormalities and to determine their nature in patients with retrosternal chest pain and normal coronary angiography with a negative coronary spasm provocation test. Oesophageal manometry was carried out in all cases with or without a spasm provocation (usually alkalosis) test. Forty consecutive patients were studied: 19 men (47.7 +/- 10.0 years) and 21 women (54.7 +/- 7.5 years). A history of gastro-intestinal disorder was obtained in 57 p. 100 of cases (hiatal hernia and/or gastro-oesophageal reflux, biliary lithiasis and/or cholecystectomy,

). Seventeen patients had broad based powerful oesophageal contractions which are an established cause of pain; they were recorded under basal conditions in 5 cases and after a provocation test in 12 cases. Two patients had a megaoesophagus without giant waves. Thirteen patients had manometric signs of reflux (malposition and
hypotonia
of the lower oesophageal sphincter) of whom 7 had giant waves on provocation. In addition, three patients experienced pain during gastro-oesophageal reflux (1 case) or
