The congenital varicella syndrome is a rare disorder in which affected infants developed characteristic ocular and systemic manifestations. We present an infant who showed this syndrome, following maternal varicella during pregnancy. The infant had low birth weight, cicatricial skin lesions, limb contractures, hypotonia, areflexia and chorioretinal scarring. Fetal infection was demonstrated with the presence of varicella-zoster-specific IgM in the baby.

Following maternal chicken pox in the 14th week of pregnancy, a male infant was born with low birth weight, muscle wasting and limb contractures, hypotonia and areflexia. A rising titre of varicella-zoster-specific IgM (by enzyme-linked immunoabsorbent assay) confirmed congenital infection, and electromyogram showed widespread denervation. Death occurred at 8 days. Neuropathological examination revealed destructive and inflammatory lesions of cerebral cortex and white matter, thalamus, brain stem and spinal cord. In addition there were cerebellar heterotopias and bilateral polymicrogryic malformations of the insular cortex.

The actual incidence of central nervous system complications with varicella ranges from 0.1 to 0.7% in several series and the time interval between the rash and the type of neurological manifestation varies widely. Cerebellar ataxia and hypotonia are the most common neurological abnormalities associated with varicella. We present a report on a 6-year-old girl with an unusual course of disease finally presenting a probable diagnosis of acute disseminated encephalitis.
