We tested the significance of the
Ki-67
plasma cell growth fraction in 49 bone marrow samples from 42 patients with
multiple myeloma
(MM). As a new approach to study
myeloma
cell proliferation, strong positivity of the CD38 antigen as plasma cell related feature was simultaneously evaluated with nuclear
Ki-67
expression in a flow cytometric double immunofluorescence assay. Mean
Ki-67
values were significantly higher in MM at relapse (22.4 per cent +/- 10.4) as compared with MM at diagnosis (11.9 per cent +/- 8.4, p less than 0.005) and plateau-phase (10.0 per cent +/- 5.5, p less than 0.001), respectively. Serial observations in six patients confirmed this change in cell kinetic behaviour during the course of the disease. Elevated
Ki-67
values correlated significantly with stage III (versus stage I, p less than 0.05), beta-2-microglobulin serum levels greater than 6 (p less than 0.001), plasmablastic morphology (p less than 0.001), and diploid
myeloma
cell DNA-content (p less than 0.005). No correlation was found between
Ki-67
and immunoglobulin isotypes as well as immunophenotypic features (expression of CD10, CD33, and CD56) of
myeloma
cells. Clinically, six of seven patients with
Ki-67
greater than 14 per cent at diagnosis had an unfavourable course (primary resistant disease or early relapse), and three of four patients with elevated
Ki-67
values at plateau-phase relapsed within 3 months. Our results demonstrate the usefulness of
Ki-67
in determining proliferative activity in MM and emphasize its value in the evaluation of the risk profile of MM patients.

Plasma cells isolated from bone marrow (BM) aspirates of 15 patients with active
multiple myeloma
(MM) were cultured and analysed for in vitro proliferative response and Ig-synthesis upon stimulation with interleukin-3 (IL-3), interleukin-4 (IL-4) and interleukin-6 (IL-6). The proliferative response, determined as
Ki-67
positivity in concentrated plasma cells, was increased by IL-6 (Stimulation Index, SI = 1.77 +/- 0.21 (M +/- SEM] but not by IL-3 or IL-4. This proliferation could be blocked by anti-IL-6. In vitro Ig-synthesis was stimulated by IL-4 (SI = 1.62 +/- 0.12 (M +/- SEM) P less than 0.05) but not by IL-6 or IL-3. This effect was not antagonized by anti-IL-6. An inverse correlation was found in this group of patients between the IL-6 induced stimulation of plasma cell proliferative activity and the IL-4 induced increase of Ig-synthesis (P = 0.027). These data indicate in MM that Ig-synthesis and the in vitro proliferative activity may be stimulated by different haematopoietic growth factors, which may reflect the involvement of different responding cells.

In 16 patients with monoclonal gammopathies of undetermined significance (MGUS) and in 49 with
multiple myeloma
(MM, 43 untreated and 6 relapsed) we used immunocytochemistry to determine the percentages of bone marrow plasma cells (BMPC) that incorporate bromodeoxyuridine (BUDR-labeling index, BUDR-LI) in vitro and that label with the
monoclonal antibody Ki-67
(which recognizes an antigen thought to identify the growth fraction of the population,
Ki-67
GF). Both mean and range values were greater for
Ki-67
GF than for BUDR-LI. Most patients with high
Ki-67
GF also had high BUDR-LI, although a linear correlation was not found between the two parameters. MGUS has lower values than MM, and the difference was much greater for
Ki-67
GF than for BUDR-LI (p less than 0.005 vs. p less than 0.05). Differences in
Ki-67
GF but not in BUDR-LI were found between MGUS and stage I MM (p less than 0.0005) and between grouped stage I and II MM and stage III MM (p less than 0.025). Both
Ki-67
GF and BUDR-LI were significantly (p less than 0.005) greater in relapsed than in untreated MM. Determining
Ki-67
GF as a proliferative parameter could be a better way of studying the kinetics of (BMPC) in MGUS and MM than determining the BUDR-LI, since a wider range of values is obtained and this allows patient groups with different clinical characteristics to be separated more easily.

and leukemias and found that H-ras p21 levels in
multiple myeloma
strongly correlated to the prognosis of patients (p = 0.03). When AML cells were stimulated by adding G-CSF, it was found that many cells proliferated but some were dying. The percentage of dying cells was small in one AML case whose myeloblasts showed increased expression of H-ras p21 by G-CSF stimulation. Together with other papers reviewed, it is conceivable that H-ras p21 expression is related to cell proliferation and inhibition of cell autolysis. Thus FCM is useful in the classification of the role of oncogene products in carcinogenesis in clinical cases.

. The karyotype consists of about 50 murine and 20 human chromosomes. The HAB-1 cells grow in suspension and have a doubling rate of about 25-30 h. In fusion experiments with spleen cells from stomach carcinoma patients HAB-1 cells show a 5-7 times higher fusion efficiency than murine Ag8 cells or another heteromyeloma SPM4-0 and give stable antibody producing products. The cell line will be made available to interested scientists.

(MM) colonies are difficult because nonproliferative, but viable, plasma cells can survive for several weeks in culture and because MM cells tend to clump readily, forming pseudo-colonies. The present study describes a method for growing pure
myeloma
colonies in serum-free conditions in which genuine
myeloma
growth is unequivocally demonstrated. Growth was observed in 17 of 32 MM bone marrow samples. After a delay of 3-5 weeks, during which most cells died, Ig light-chain-restricted colonies emerged, expanded for approximately 3 weeks, and then showed no evidence of further proliferation. Flow cytometry (FCM) of oncogene products which opens new avenues of cell biological investigation of human neoplasia is being reviewed. Using H-ras p21/DNA dual FCM, patients with DNA-aneuploid
multiple myeloma
(MM) were examined. The patients whose MM cells expressed high level of H-ras p21 had poor prognosis. Specificity of this assay was appraised extensively. It is not likely that H-ras p21 expressed in MM is of oncogenic form since point mutation of H-ras gene was not reported in B cell chronic lymphocytic leukemia which is closely located to MM in B lymphocyte differentiation lineage. High expression of H-ras p21 in MM seems to be related to cell proliferation and/or differentiation. H-ras p21/DNA dual FCM is applicable to analyse the pathophysiology of tumor cells. This study demonstrates that malignant plasma cells, in analogy to their normal counterparts, have an affinity for the BM stroma and suggests that their long-term survival might be enhanced by their interaction with it.

positive cells may therefore serve as a reliable measurement for the growth fraction in normal and neoplasmic cell populations. We have tested the significance of the MoAb

in the classification of monoclonal gammopathy and compared the results with the plasma cell labelling index. In benign monoclonal gammopathy the percentage of
Ki-67
positive plasma cells (median 1.6%) was significantly lower than in untreated
multiple myeloma
(median 9.6). Among the patients with more than 10%
Ki-67
positive plasma cells there were some very short survivors. The largest growth fractions (median 41.8%) were found in patients with relapsing
multiple myeloma
indicating here a different growth pattern more resembling the high-grade lymphomas. A linear correlation between the proportion of
Ki-67
positive plasma cells and the labelling index was not found. Determination of the plasma cell growth fraction with the
