Hyperferritinemia in various diseases, mainly hematological, was confirmed by immunological methods. For

antiserum were used and the result was compared with each other. Leukemia, malignant lymphoma,
multiple myeloma
, and aplastic anemia are hematological diseases which showed a positive reaction in this test, among which leukemia showed the highest positivity. Cases of hepatic diseases and non-hematological malignant neoplasms also showed a positive reaction. The positivity was quite low and almost negligible in other diseases and healthy individuals. ferritin
antiserum seemed to be suitable for cancer diagnosis and, antihuman hepatic
ferritin
antiserum for hepatic diseases. The results of analysis of purified human hepatic and placental ferritins highly suggested the presence of immunological heterogeneities between them. Also, a possibility was pointed out that one of the components of the so-called leukemia-specific antigens might sometimes be the isoferritin of leukemia cells.

was found in patients with breast cancer and untreated Hodgkin's disease. No such lymphocytes were demonstrated in normals or in patients with benign breast disease. The appearance of such subpopulation in the circulation is an early manifestation of the neoplastic disease, and its identification may provide a tool of potential diagnostic and prognostic importance in the management of Hodgkin's disease and breast cancer.

The specific antiserum against a type of
ferritin
that is especially common to leukemia cells and the placenta was used to test, by countercurrent immunoelectrophoresis, sera from humans with various diseases. The best results were obtained with leukemia; patients with chronic myelogenous leukemia in blastic phase, acute myelogenous leukemia, lymphogenous leukemia, and unclassifiable juvenile leukemia frequently showed a positive reaction, but patients with chronic myelogenous leukemia in static phase did not. The average incidence of positive reaction among all leukemia patients was 54.0%. Patients with other malignant tumors (i.e.,
multiple myeloma
, malignant lymphoma and carcinomas of the stomach, rectum, and liver) also often showed a positive reaction. The average incidence of positive reaction among all the patients with malignant diseases of the hematopoietic system, except for leukemia, was 34.3%, and that among patients with nonhematologic malignant neoplasms was 36.8%. However, the incidence of a positive reaction in patients with benign diseases and healthy individuals was less than 3%.

Spontaneous variants of the IgA immunoglobulin secreting mouse
myeloma
, S194-2, were isolated by cloning the line on soft agar and screening for the loss of secreted S194 immunoglobulin. Because S194 IgA possesses DNP binding activity, the screening method was designed to test for clonal secretion of antibody which specifically precipitated DNP-
ferritin
conjugates. Precipitates formed over IgA secreting S194 clones, whereas none were evident over nonsecreting XCl clones nor IgG secreting MOPC 21 clones (MOPC 21 IgG does not bind DNP). In addition the method was sensitive to the amount of immunoglobulin secreted. By continual selection of exceptionally reactive clones with this assay, a S194 culture was obtained which secreted five to six times as much IgA as the original mass culture. Spontaneous variants were isolated from six independent subclones of this parent line with an overall frequency estimated at 2.7 X 10(-5) per cell per generation. Biochemical analysis of these variants showed that all of them secreted reduced or undetectable amounts of IgA. No variants were obtained which secreted IgA molecules altered at the DNP binding site, or which secreted immunoglobulin subunits alone. Variants of the latter class have, however, been obtained in high frequency in other
myeloma
strains by other investigators.

In this study, we investigated a possible association between the degree of macrophage activation - as measured by serum neopterin concentrations - and disturbances of iron metabolism, determined by the concentrations of
ferritin
and serum iron, in patients with malignant disorders. Additionally we evaluated correlations between these factors and the degree and type of anaemia. Seventy-three patients, who suffered from non-Hodgkin's lymphoma (NHL) (n = 43), Hodgkin's disease (n = 11),
myeloma
or monoclonal gammopathy of unknown significance (n = 9), myelodysplastic syndrome (n = 1), and solid tumours (n = 9), were examined. Mean neopterin levels were raised in all groups, patients with NHL showing the highest concentrations. Ferritin but not neopterin concentrations were higher in males than in females. A significant correlation was found between neopterin and
ferritin
concentrations (p less than 0.01). Considering only female patients the strength of the correlation was the same (p less than 0.02). In addition, we found inverse correlations of neopterin with haemoglobin and iron concentrations (all p less than 0.01). Similar relationships existed in patients during follow-up. Our results support the hypothesis of an association between the degree of activation of macrophages and the development of anaemia by a shift or iron towards the storage sites.

This clinical trial was performed to study the effects of intravenously (IV) administered recombinant human (rh) erythropoietin (EPO) at escalating doses (150, 300, and 450 U/kg, administered as an IV bolus injection, twice weekly, for 6, 4, and 4 weeks, respectively) in five patients with low-grade non-Hodgkin's lymphoma (Ig NHL) and bone marrow involvement and one patient with
multiple myeloma
(MM). All patients were anemic due to underlying disease. None of the patients had a history of bleeding, hemolysis, renal insufficiency, or other disorders causing anemia in addition to bone marrow infiltrating malignancy. Endogenous EPO serum levels were significantly increased in all patients (74 to 202 mU/mL). Five patients (one MM, four small-cell lymphocytic [SCLC] NHL) showed a dramatic increase of hemoglobin (Hb), hematocrit (Hk) and RBC count becoming obvious on the second EPO dose level. Initial
ferritin
serum values, which were high mostly due to polytransfusion, were significantly reduced in responding patients. Erythropoiesis of one patient with extensive follicular mixed (fm) NHL did not respond to EPO treatment. Platelet (PLT) count increase (greater than 75% above starting levels) during and following EPO therapy was observed in one patient with MM. Adverse events due to EPO therapy have not been recorded. These findings point out a previously unrecognized capacity of EPO given at pharmacologic doses to stimulate erythropoiesis in patients with anemia due to bone marrow infiltration by neoplastic lymphocytes in spite of enhanced endogenous EPO expression.

Anemia is a common complication of
multiple myeloma
. It resolves early in the disease if chemotherapy induces a complete remission, but persists if the disease progresses, causing disabling symptoms and often requiring blood transfusions. We treated 13 patients with
myeloma
-associated anemia by administering recombinant human erythropoietin three times a week for six months. Eleven patients (85 percent) had steady increases in hemoglobin levels and eventual correction of the anemia. Their symptoms of anemia subsided, and they reported a heightened sense of well-being. No patient had any adverse side effects, particularly episodes of hypertension. Monitoring of the serum M component showed a predominantly stable tumor load without apparent interaction between the underlying disease and the response to erythropoietin therapy. The number of erythroid burst-forming units in the bone marrow and peripheral blood and the level of erythropoiesis in bone marrow smears increased significantly during therapy. Pretreatment serum levels of erythropoietin were higher in the patients who did not respond and in those who required more than two months of treatment before they responded. Serum iron,
ferritin
, and transferrin concentrations reflected responses to treatment. We conclude that recombinant human erythropoietin is a promising therapeutic tool for treating
myeloma

M cells of intestinal epithelia overlying lymphoid follicles endocytose luminal macromolecules and microorganisms and deliver them to underlying lymphoid tissue. The effect of luminal secretory IgA antibodies on adherence and transepithelial transport of antigens and microorganisms by M cells is unknown. We have studied the interaction of monoclonal IgA antibodies directed against specific enteric viruses, or the hapten trinitrophenyl (TNP), with M cells. To produce monospecific IgA antibodies against mouse mammary tumor virus (MMTV) and reovirus type 1, Peyer's patch cells from mucosally immunized mice were fused with
myeloma
cells, generating hybridomas that secreted virus-specific IgA antibodies in monomeric and polymeric forms. One of two anti-MMTV IgA antibodies specifically bound the viral surface glycoprotein gp52, and 3 of 10 antireovirus IgA antibodies immunoprecipitated sigma 3 and mu lc surface proteins. 35S-labeled IgA antibodies injected intravenously into rats were recovered in bile as higher molecular weight species, suggesting that secretory component had been added on passage through the liver. Radiolabeled or colloidal gold-conjugated mouse IgA was injected into mouse, rat, and rabbit intestinal loops containing Peyer's patches. Light microscopic autoradiography and EM showed that all IgA antibodies (antivirus or anti-TNP) bound to M cell luminal membranes and were transported in vesicles across M cells. IgA-gold binding was inhibited by excess unlabeled IgA, indicating that binding was specific. IgG-gold also adhered to M cells and excess unlabeled IgG inhibited IgA-gold binding; thus binding was not isotype-specific. Immune complexes consisting of monoclonal anti-TNP IgA and TNP-

Cardiac involvement in haemochromatosis includes development of congestive heart failure and/or cardiac arrhythmias. To elucidate the importance of this disorder among patients with severe cardiac bradyarrhythmias necessitating treatment with a permanent pacemaker, such patients were screened for evidence of iron overload. Serum
ferritin
was determined in 232 men treated with a permanent pacemaker. In six patients (2.6%) the serum
ferritin
values were twice the upper normal limit. In three of these, causes other than iron overload were found (liver cancer,
myeloma
and amiodarone treatment), while in three (1.3%) iron overload was suspected. All the latter three had atrioventricular block of second to third degree. In these patients percutaneous liver biopsy was performed. The specimens were examined by light and electron microscopy and the iron content was determined by atomic absorption spectrophotometry. The results of these investigations showed iron overload in the liver. The prevalence of iron overload (2.0%) among this male pacemaker-treated population with AV block II-III is significantly higher than the rates previously found in urban populations in Sweden. The present data indicate that screening for haemochromatosis is of importance among males with second and third degree atrioventricular-block of unknown etiology.
