Myeloma in the left temporal region was seen in a 71-year-old male who also had facial palsy, hearing loss and tinnitus on the left. In this case, none of the laboratory test results had indicated myeloma. Diagnosis of myeloma in this case was made from findings of a CT scan of temporal bone and the results of biopsy of the external auditory canal lesion. In the CT scan, marked destruction of the bone was seen, extending from the mastoid air cell to the posterior wall of the external auditory canal. The biopsy of the mass lesion in the left auditory canal revealed proliferation of lymphoplasma-cytoid cells. This case is reported along with laboratory test findings, clinical findings, and a review of the literature.

The manifestations of multiple myeloma are protean and related to bony osteolytic lesions, and to medullar and renal insufficiency. We report a patient who presented with otalgia as the inaugural symptom of multiple myeloma. Local irradiation combined with systemic chemotherapy led to the disappearance of the temporal bone mass and the accompanying symptoms. To date, 24 months after the diagnosis, the patient is still in remission. The literature on otological involvement in multiple myeloma is reviewed. Symptoms are non-specific and include hearing loss, tinnitus, dizziness, facial paralysis, and otalgia. The diagnosis of multiple myeloma should be considered in the presence of a temporal bone mass.
