Forty-two cases of hematological malignancy (18 cases of CML, three cases of
polycythemia vera
, 10 cases of malignant lymphoma and 11 cases of
multiple myeloma
) were treated with MCNU. The results obtained were as follows. MCNU was markedly effective on CML cases, being especially useful during the chronic phase, and a partial remission was observed in one of three patients with CML blastic crisis. A good response was observed in all cases with
polycythemia vera
. In some cases of malignant lymphoma, a fair response was observed. No response was observed in any of the cases of
multiple myeloma
. Myelosuppression was a major side effect of MCNU, but other side effects other than melena were not severe.

Five patients with myeloproliferative disorders and paraprotein are reported. The diseases included acute myelomonocytic leukemia, chronic myeloid leukemia, sideroblastic anemia with excess of blasts,
polycythemia vera
and myelofibrosis. In four cases, the paraprotein was of the IgG k type and in one, IgM k. No evidence of
multiple myeloma
or excessive plasmacytosis was noted. The literature records 19 other myeloid disorders with this unusual association. Although the pathogenesis remains unclear, three possible explanations are suggested: disturbance of the pluripotent stem cell resulting in a combined myeloplasmatic disorder, coexistence of two diseases, or a fortuitous association.

Adenosine deaminase (ADA) has been assayed in plasma, erythrocytes, and lymphocytes from 29 patients with haematological and autoimmune diseases. ADA activity was uniformly low in erythrocytes and lymphocytes from patients with non-Hodgkin lymphoma and
multiple myeloma
(p less than 0.001). High levels of ADA activity was found in plasma, erythrocytes, and lymphocytes from patients with myeloid leukemia (p less than 0.001). ADA was high in plasma but low in erythrocytes and lymphocytes from patients with autoimmune diseases treated with immunosuppressive drugs (p less than 0.05). 4 adults with congenital immunodeficiency showed decreased ADA activity. In the control group of normal blood donors we found a 34-year-old female with low ADA activity in plasma, erythrocytes, and lymphocytes without any immunological abnormalities. Management of hematologic disorders in older patients must often be weighed in a setting of decreased physiological reserves and concurrent illnesses. Anemia in the elderly should never be attributed to old age. Even a mild anemia in collusion with multiple physical and mental problems may tip the balance for those previously able to cope with their disabilities. Iron deficiency anemia and the anemia of chronic disease are the most common types of anemia in the elderly. Nutritional anemias due to folate or vitamin B(12) deficiency are treatable and should not be overlooked. Newer chemotherapy regimens for acute nonlymphocytic leukemia have been effective in many older patients. Decisions to treat are sometimes difficult, often depending on the aggregate of coexistent physical and mental disorders. The most prevalent type of leukemia in the elderly is chronic lymphocytic leukemia. A benign asymptomatic course requires no therapy, but aggressive disease requires treatment.
Multiple myeloma
should be suspected in an elderly person who has both unexplained anemia and bone pain. After definitive diagnosis, phlebotomy therapy should be considered for both
polycythemia vera
and secondary erythrocytosis to reduce blood viscosity and increase cerebral blood flow.

While radiotherapy and antineoplastic chemotherapy often control malignancies they may, paradoxically, cause new cancers to develop as long-term complications. Although almost any type of neoplasm can occur, radiation-induced malignancies are most likely to affect the myelopoietic tissues and the thyroid gland. The former tissues are also most frequently involved by chemotherapy. The combination of intensive radiotherapy and intensive chemotherapy is particularly leukemogenic. Acute myeloid leukemia has occurred with increased frequency following treatment of Hodgkin's disease, non-Hodgkin's lymphoma,
multiple myeloma
, ovarian cancer,
polycythemia vera
, carcinoma of the thyroid gland, and carcinoma of the breast. Radiation-induced malignancies usually occur in the field of irradiation. For example, radiotherapy for carcinoma of the cervix may be followed by the development of carcinomas of the endometrium, vagina, urinary bladder, colon , rectum, and anus, as well as mesotheliomas of the peritoneum and osteosarcomas of the pelvis. Tumors developing in an irradiated field include a substantial number of soft tissue sarcomas or osteosarcomas. There is a 20-fold increase of second cancers following treatment of childhood malignancies, mostly sarcomas of bone and soft tissues, but including leukemia, and carcinomas of the thyroid gland, skin, and breast. The latent period between radiotherapy and the appearance of a second cancer ranges from 2 years to several decades, often being 10-15 years. With chemotherapy the mean latent period is shorter, approximately 4 years. The mechanism of oncogenesis by radiotherapy or chemotherapy is poorly understood and probably involves a complex interplay of somatic mutation, co-oncogenic effects, depression of host immunity, stimulation of cellular proliferation, and genetic susceptibility. The danger of developing second malignancies following radiotherapy or chemotherapy emphasizes the need for lifelong follow-up of patients given these forms of treatment; particularly in those with a long life expectancy as are those treated for childhood neoplasms.

, agnogenic myeloid metaplasia, primary thrombocythemia, rheumatoid arthritis with thrombocytosis,
multiple myeloma
, and erythrocytosis associated with polycystic renal disease. Portal hypertension was suspected in three and features of hypersplenism were present in four. 2. Nodular regenerative hyperplasia occurred in livers which had widespread obliteration of portal vein radicals (obliterative portal venopathy). Morphometric analysis indicated that the portal vein lesions were predominately located in veins up to 0.2 mm in diameter and were significantly more frequent than similar lesions occurring in elderly persons. 3. The following pathogenesis of nodular regenerative hyperplasia is proposed: Thrombi, perhaps largely composed of platelet aggregates formed in the portal venous circulation or spleen, embolize to the liver and results in obliterative vascular lesions. Atrophy and regenerative nodule formation occur in response to the interruption of the portal blood supply.
