Tasmania, an island state of the Australian Commonwealth with a population of 400,000 of predominantly Anglo-Saxon heritage, has relatively centralized oncology services. A study was undertaken of all patients known in December 1971 and of all new cases diagnosed since January 1972 with all forms of leukemia, Hodgkin's disease, non-Hodgkin's lymphoma,
myeloma
, and other myeloproliferative and lymphoproliferative disorders. Data were obtained with respect to lifetime residential and occupational history, schools attended, and known familial cases of any of the myeloproliferative and lymphoproliferative disorders.

Brain tumors have been tested for their glial fibrillary acidic protein (GFAP) content by means of the rocket electrophoresis technique. Meningiomas and neurinomas were low in GFAP. Metastases had a low level of GFAP except when contaminated with surrounding tissue. Non-nervous tumors such as
myeloma
, myeloplaxoma and adenocarcinoma gave negative results. More detailed correlations with histological observations have been looked for in glial tumors. Low levels of GFAP were always associated with signs of
malignancy
such as mitoses and giant or atypical cells, whereas high levels of GFAP were correlated with the presence of well-preserved astrocytes.

An increased mortality from lung cancer, cardiovascular disease, haematolymphatic
malignancy
and cirrhosis of the liver has been reported among smelter workers and others exposed to arsenic. This study uses the case-referent (case-control) technique and is concerned with workers in a copper smelter in a complex work environment, characterised by the presence of trivalent arsenic in combination with sulphur dioxide and copper, and also with other agents. Lung cancer mortality was found to be increased about five-fold and cardiovascular disease about two-fold, showing a dose-response relationship to arsenic exposure. Mortality from malignant blood disease (leukaemia and
myeloma
) and cirrhosis of the liver was also slightly increased. This mortality pattern among the smelter workers is consistent with earlier reports. An increased mortality from cardiovascular disease in this type of industry is of particular interest as it has been reported only once before.

is very rare. On review of 248 cases, we found 4 cases in which pathological evidence of
plasma cell neoplasm
in the thyroid was verified. This was a heterogeneous group of patients; the thyroid involvement was clinically recognized as a site of extramedullary
plasma cell neoplasm
in one patient and as a part of generalized disease in two patients. In another patient with generalized disease, the thyroid involvement was discovered at autopsy.

Multiple myeloma
is often associated with humoral immunodepression in both man and mouse. When mice bearing the humorally immunodepressive plasmacytomas TEPC-183 and SPQC-11 were injected with SRBC, the rise of serum haemolysins was significantly less than that of non-tumour-bearing mice. Mice with the plasmacytomas MPC-11 and MOPC-315 have an antibody response similar to normal mice when injected with SRBC. Following immunization, normal mice and those bearing MPC-11 showed a 2- to 3-fold increase in total spleen lymphocytes. Mice bearing TEPC-183 or SPQC-11, the plasmacytomas causing an impaired antibody response, has significant increase in spleen lymphocytes under the same conditions. Mice bearing MOPC-315 had a very high initial count of spleen lymphocytes, which did not further increase upon immune stimulation.Incubation of lymphocytes from plasmacytoma-bearing mice with PHA did not produce an increase in TdR incorporation and in some cases even caused a decrease in TdR incorporation.Lymphocytes from mice bearing TEPC-183, SPQC-11, and MOPC-315 incorporated less TdR in response to LPS than did normal mice. On the other hand, mice bearing MPC-11 incorporated about as much TdR as did normal mice following LPS stimulation. Thus, the defect in the ability to respond to LPS in vitro correlated with the lack of an increase of spleen lymphocytes in mice bearing these tumours following antigenic stimulation in vivo.No immunodepressive properties of serum from mice with plasmacytoma could be detected.

A survey was carried out in 2 country areas of New South Wales with the aim of defining contacts among patients with lymphoma, leukemia and
myeloma
, and of determining whether these were more numerous than among matched controls from the same localities. Cases were identified from records of the N.S.W. Central
Cancer
Registry and of local doctors and hospitals. 184 cases were found, corresponding to the expected number, and 145 patients, as well as the same number of controls, were interviewed. Of the 290 patients and controls surveyed 111 (38.3%) had had one or more contacts with other patients or controls (37.9% of patients and 38.6% of controls). There were 24 case-case pairs involving contacts among 33 individual patients, 23 control-control pairs involving 36 individuals, and 38 case-control pairs involving 66 individuals. A statistical analysis using a weighting system showed that numbers, closeness and duration of contacts among patients and patients did not differ significantly from those expected. These results thus provide no evidence in favor of the hypothesis that the diseases were transmitted from patient to patient in the survey areas.

, while four patients had "benign" monoclonal gammapathy. In all the cases where bone marrow aspiration was done, an increase in plasma cells was revealed. A majority of the

patients had osteolytic lesions, while all the patients with "benign" monoclonal gammapathy had normal bone survey. A relationship between the solid tumors and monoclonal gammapathy is discussed. The difficulty of detecting
multiple myeloma
in patients who are previously known to have
cancer

Fourteen patients with
multiple myeloma
resistant to melphalan plus prednisone were treated with BCNU 50 mg/m2 plus cyclophosphamide 200 mg/m2 on day 1, adriamycin 20 mg/m2 on day 2 and prednisone 60 mg orally, daily for days 1 through 5. Therapy was repeated every four weeks. Depending upon criteria used, objective antitumor responses were achieved in five to nine of the 14 patients. Mean survival was 9.5 months and actuarial median survival was 7.0 months. Six patients are alive, four to 35 months after initiation of therapy. This preliminary report indicates that this combination may be a useful treatment program in the management of patients with advanced
multiple myeloma
. A review of studies employing adriamycin plus BCNU suggests that these regimens currently offer the most effective treatment of melphalan-resistant patients.

Multiple, firm, nontender, violaceous, cutaneous and subcutaneous nodules of the abdomen and thighs developed in a 53-year-old man with proven IgD myeloma. A biopsy specimen showed dermal infiltration by large atypical undifferentiated cells similar to those previously found on bone marrow aspiration. Despite intermittent improvement with radiotherapy and chemotherapy, the patient died and an autopsy showed multiple osteolytic lesions and massive involvement with tumors that encased the pelvic organs, infiltrated and replaced portions of the psoas muscles, infiltrated the kidneys and intestines, and replaced the bone marrow by small and intermediate-sized malignant lymphoid cells. IgD myeloma is a rare hematologic
malignancy
that occurs at a younger age than other forms of
myeloma
, has a high incidence of involvement of soft tissues, and is a particularly dedifferentiated and vicious form of

Plasmacytomas occasionally may arise in extraosseous sites, of which the upper respiratory tract is the most common. In our series of 256 nonepithelial tumors involving the nasal cavity, paranasal sinuses and nasopharynx, 10 (4%) were apparently primary extramedullary plasmacytomas. Of our patients with adequate follow-up information, four died of disseminated disease 2, 4, 5 and 6 years after the initial diagnosis. One was alive 6 years after diagnosis with locally persistent tumor. Another was living without recurrence 12 years later. Based on our findings and on the cases reported in the literature, extramedullary plasmacytomas arising in the head and neck area may evolve into one of several different patterns. 1) Some patients have localized disease which is apparently controlled (by surgery, radiotherapy or both) and which never recurs locally or becomes disseminated. 2) In some cases, the tumor recurs locally and is controlled by further therapy. 3) Other patients have a locally persistent and aggressive lesion which cannot be eradicated and which eventually leads to the patient's death by uncontrolled local growth. 4) Still other patients eventually develop evidence of
