Bezoars are conglomerates of food or fiber in the alimentary tract of humans and certain animals, mainly ruminants. In adults, bezoars are most frequently encountered after a gastric operation, but in children, bezoars are associated with pica, mental retardation, and coexistent psychiatric pathologic disorders. We describe a 4-year-old girl with trichotillomania in whom a gastric trichobezoar resulted in failure to gain weight, iron deficiency anemia, and a painless epigastric mass. The causes of gastric bezoars are discussed, with emphasis on diagnostic considerations, treatment options, and preventive measures.

We present the case of a 16-year-old girl with a 12-year history of trichotillomania and trichotillophagia in combination with mental retardation and early childhood deprivation, all contributing to the growth of a stomach ulcer and an oversized, stomach-shaped trichobezoar which had to be removed by gastrotomy. Included are a discussion of psychodynamic aspects, therapeutic strategies, and significant literature, concluding with a short, historic view on the varieties and therapy of bezoars.

An unusual form of bezoar extending from the stomach to the small intestine or beyond has been described as Rapunzel syndrome. Bezoars are concretions in the gastrointestinal tract that increase in size by continuous accumulation of non-absorbable food or fibers. Most bezoars in children are trichobezoars from swallowed hair from the head, dolls, or brushes. Trichobezoars typically cause abdominal pain and nausea, but can also present as an asymptomatic abdominal mass, progressing to abdominal obstruction and perforation. Trichobezoar with Rapunzel syndrome is an uncommon diagnosis in children with less than 40 cases reported. It is predominantly found in emotionally disturbed or mentally retarded youngsters. We present the youngest case of Rapunzel syndrome in the United States, a 5-year-old girl with mental retardation who presented with abdominal pain, vomiting and a non-tender abdominal mass.

A rare case of 13 years old female child with recurrent trichobezoar stomach which needed reoperation for its removal is reported. Patient also had trichotillomania and mental retardation. She showed satisfactory response to therapy with fluoxetine.
