Since mid-1993, all

treatments performed in the state of Texas (except for United States government hospitals) must be reported every quarter to the Texas Department of Mental Health and
Mental Retardation
(TXMHMR) on a data collection form provided by the Department. Part 1 of this paper reviewed that data. This paper reviews the responses to questionnaires and contacts made with physicians, hospitals, medical liability insurance companies, and manufacturers of stimulus generating devices regarding their experience with
ECT
in Texas. Questionnaires were sent to physicians and hospitals that had not performed
ECT
during the final two quarters of the review period. Medical liability insurance companies and the manufacturers of the stimulus generating equipment used in
ECT
were contacted regarding their experience with liability claims. The results indicate that medical liability in regards to the performance of
ECT
is extremely low. Physicians and hospitals that stopped performing
ECT
did so for reasons other than medical liability.

Self-mutilation is form of self-injurious behavior which is either low intensity, high frequency form seen in personality disorder and
mental retardation
or low frequency, highly destructive form which occurs in the context of psychosis or acute intoxications. We report a 32 yr-old male with bipolar affective disorder presenting with multiple acts of major self-mutilation during an episode of psychotic depression. At presentation he had broken upper central incisors, circular burn scar on right palm, crush injury of left index finger with avulsed nail and cut injury over glans penis. In view of his extreme self-mutilating behavior, he was administered electroconvulsive therapy on second day following which self-mutilating behavior improved after fourth

sessions, he was started on lithium carbonate with a serum level of 0.82 meq/L and haloperidol 15 mg/day and there was no recurrence of self-mutilating behavior during follow up after four months.

Catatonia is a syndrome of motor and behavioral disturbance. It is a poorly understood condition, which is underrecognized and may go untreated despite intensive medical workup and numerous unsuccessful medication trials. However, with treatments known to be effective, such as benzodiazepines and/or electroconvulsive therapy, patients may return to their baseline functioning. Autism and catatonia have been previously reported together. We report 2 patients with autism and
mental retardation
who developed catatonic symptoms at the onset of puberty. Both patients experienced persistent symptoms over several years and presented with a history of motor disturbance, functional decline, and episodic aggression. Both patients were treated with electroconvulsive therapy resulting in a positive response and functional improvement. Catatonia may persist as a chronic condition, lasting over several months or years, if not recognized and treated.

The mentally retarded subjects show a much higher prevalence of full range of psychiatric disorders than the non-retarded population. Whereas the role of psychotropic drugs in such patients is well discussed, the reports on the use of
ECT
are scarce. Many psychiatrists dealing with the mentally retarded psychiatric patients are reluctant to consider
ECT
due to lack of adequate experience. We report five mentally retarded patients with different psychiatric disorders who were successfully treated with
ECT
after failing adequate trials of pharmacotherapy. No disproportionately higher adverse events on account of
mental retardation
were observed.
ECT
need to be favorably considered in mentally retarded persons with psychiatric morbidity especially when treatment with psychotropic drugs either fail or is intolerable.
