A 52-year-old man had a generalized seizure resulting in fracture of two vertebrae 10 years after excision of a superficially spreading
melanoma
(Clark level III) on the left flank. Additional to
pain
on pressure over the right upper abdomen, the erythrocyte sedimentation rate was increased (41 mm) and the activity of the liver enzymes was raised. Neurological examination indicated a loss of short-term memory, signs of meningeal irritation, unsteady gait and falling tendency. Cerebrospinal fluid showed mild pleocytosis and cells suspicious of tumour. Upper abdominal ultrasonography revealed an enlarged gallbladder with contents of soft-tissue density. Magnetic resonance imaging showed numerous round lesions in the brain which looked nodular after contrast-medium injection. Cholecystectomy revealed metastases of a
malignant melanoma
. It was likely that the cerebral lesions also represented metastases. The confusional state increased in severity. Neither cytostatic drugs nor radiotherapy were administered, because of the poor prognosis. The patient, now fully disoriented, died 54 days after hospital admission.

In this retrospective study plain radiographs, radionuclide bone scans, computed tomography (CT) and magnetic resonance (MRT) examinations of 115 patients with metastatic carcinoma of the spine were analyzed. In 32 patients metastases were proven histologically and in the remainder by follow-up studies. Altogether, 513 vertebrae were evaluated. Forty-one patients had histologically proven breast cancer, 14 renal cell carcinoma, 11 prostate cancer, 8
melanoma
. 8 tumors of the gastrointestinal system and 7 bronchial carcinoma. Evaluation of the plain films showed that the initial site of metastasis (n = 463) was the vertebral body in 441 cases and the pedicles in 294 cases. In CT scans most of the lesions confined to one part of the vertebral body (36 of 98) were localized in the posterior part. Twelve percent of the metastases were diagnosed with conventional radiography and 17% of those diagnosed with CT were not detected in skeletal scintigraphy. MRI was rarely used in diagnosing occult vertebral metastases (n = 37); 22% of the metastases demonstrated by MRI were not detected in skeletal scintigraphy. We concluded that only in 63.8% was the pedicle sign the initial site of metastasis on plain films. Bone scans and plain films are the most important diagnostic procedures for detecting and monitoring vertebral metastases. CT and MRI are only needed in patients with neurological symptoms and persistent

. Fluorescein-angiography revealed signs of double circulation. Abnormal choroidal vessels inside the tumour showed up clearly on indocyanine-green angiography. By ultrasonography we were able to detect scleral perforation and extra-scleral extension. Computed tomography confirmed the extra-scleral extension. As there were no signs of metastasis, an orbital exenteration with sparing of the eyelids was performed. Histopathology is compared with the results of clinical investigation.

malignant melanoma
, aspects of quality of life (QL) were measured prospectively by patients using linear analogue self assessment (LASA) scales including the GLQ-8 and by doctors using Spitzer's QL Index. QL scores and performance status at the time of randomisation were available for 152 of 170 eligible patients. These scores carried significant prognostic information. In univariate analyses, Spitzer QL Index assessed by the doctor and LASA scores for physical wellbeing (PWB), mood,
pain
, appetite, nausea and vomiting, GLQ-8 total and overall QL were significant (P < 0.01) predictors of subsequent survival. QL Index and LASA scales for mood, appetite, and overall QL remained independently significant (all P < 0.05) in multivariate models allowing for significant prognostic factors other than QL (liver metastases and performance status). These findings closely parallel those in patients with metastatic breast cancer. They add further validity to the QL Index and LASA scores, provide the first evidence of the prognostic significance of the GLQ-8, and argue strongly for the routine assessment of QL in future therapy trials.

(MM) module and the Hospital Anxiety and Depression (HAD) scale. The questionnaires displayed good psychometric qualities and were able to document the florid symptomatology of disseminated

. They were well accepted by the patients. Before treatment the patients reported a relatively low symptom burden, good physical and social functioning, moderate psychological distress and a high overall QOL rating during the past week. Fatigue and
pain
were the most frequent symptoms reported. The QOL measurement differentiated between subgroups of patients differing in performance status and the tumour burden. We conclude that the EORTC questionnaire technique is feasible and clinically relevant in generalized

in the back. Computed tomography, bone scans and immunohistochemical study of a liver biopsy specimen revealed metastases from the

in the liver, spleen and skeleton. Her Karnovsky score was 50%. Chemoimmunotherapy given for five cycles consisting of dacarbazine (800 mg/m2 body surface area every 4 weeks) together with ambulatory cytokine therapy (interferon alpha-2b subcutaneously, 5 million I.U. on days 1 and 3 to 7 in the first week, days 1, 3 and 5 in the second and third weeks and interleukin 2.9 million I.U. on days 1 to 5 in the second and third weeks; no treatment was given in the fourth week) raised the Karnovsky score to 90% and abolished the
pain
. Lactate dehydrogenase activity, originally 3372 U/l, dropped to normal. Computed tomography demonstrated definite shrinkage of the liver metastases and some regression of the splenic and bony deposits. This improvement has now been maintained for a total of eight cycles of therapy.
