A 42-year-old man presented with a two-week history of right-sided
otalgia
, deafness and nocturnal tinnitus. Examination revealed a black mass on the posterior meatal wall. Two years before presentation, he had a
malignant melanoma
removed from the left hip region. Biopsy of the lesion in the posterior canal wall and another which had developed within two weeks on the anterior wall, confirmed metastatic disease. Treatment with radiotherapy and later chemotherapy were unsuccessful and the patient died 15 months later.

We report the first case (to our knowledge) of
malignant melanoma
of amelanotic type arising from the mucosa of the middle ear. An 81-year-old man who presented with right hearing loss was diagnosed as having otitis media with effusion at an otolarygology clinic. Despite subsequent treatment, he developed otorrhea and
otalgia
. Furthermore, a granular mass was found in the pars flaccida of the tympanic membrane. A biopsy of the mass was performed, and the findings were suggestive of
malignant melanoma
. The disease was already in an advanced stage, because metastases were noted in the patient's lungs and liver. Although chemohormonal therapy was performed and was slightly effective in reducing the primary tumor, the patient died of acute renal failure. Autopsy confirmed the diagnosis of amelanotic melanoma of the middle ear.

Primary involvement of the middle ear and mastoid mucosa by
malignant melanoma
(MM) has been rarely reported. We report an 81-year-old Caucasian woman presenting with
otalgia
, left-sided aural fullness, and hearing loss. Otoscopic examination revealed a purple-colored polyp that appeared to be vascular, occupy left external auditory canal (EAC). The computed tomography scanning showed a mass in soft tissue density, filling the tympanic and mastoid cavities, eroding the mastoid cortex, and invading medial and central parts of the EAC. A biopsy was performed with the diagnosis of MM. The patient underwent subtotal temporal bone resection with radical neck dissection. Dacarbazine was given postoperatively. The case presented herein emphasizes the need to be aware that MMs can arise in the middle ear and mastoid cavity. An index of suspicion in the differential diagnosis of temporal bone tumors is necessary for early recognition.

The patient, a 52 year old male was admitted to the hospital, because of right hearing loss before three months. Six months ago; the patient had the right former group sinusitis and nasal polyps, and had the right former group sinus open and polypectomy operation outside the hospital. The surgery was uneventful and the postoperative dressing was done. He has no history of tinnitus,
earache
, ear pus, epistaxis, headache, dizziness. Physical examination on admission shows the right external auditory canal was clean, tympanic membrane integrity, pale yellow, mild depression, and poorly eardrum movement. The electronic nasopharyngoscopy show a black mass in the edge of the anterior lip of the right eustachian tube. The mass has a smooth surface, and only seen partly. Nasopharynx magnetic resonance shows in the right pharyngeal orifice visible there was a round short T2 node, maximum diameter of 13 mm, the border was clear. The parapharyngeal space had been compressed which close to the right eustachian tube torus. After the scan enhanced, the lesions was strengthened. The pure tone audiometry shows right mild conduction deafness, and the acoustic impedance showing right type B tympanogram curve. Eardrum puncture extracted got about 0.2 ml yellow liquid. Otitis media with effusion is considered. A biopsy is taken by means of the nasal endoscopic. The pathology report is the right eustachian orifice
malignant melanoma
. The immunohistochemical examination (Horton-Magath-Brown 45) showed a positive reaction.

Primary
malignant melanoma
of the external auditory canal is rarely reported.
Malignant melanoma
of the ear is estimated to occur in 1-4% of all skin melanomas and about 7-20% of melanomas of the head and neck region. The pathophysiology of these tumours is different from other skin lesions because of their anatomical and functional characteristics. The case presented is of a 11 year old female child with
malignant melanoma
of the external auditory canal confined to the right side, who initially presented with right
ear pain
, bleeding, post auricular swelling and also a mass in the external auditory canal which was thought to be an aural polyp in the right ear. Excision of the tumour was accomplished by a radical mastoidectomy. It was confirmed to be
malignant melanoma
after histopathological examination and Immunohistochemistry. Despite all efforts, the patient succumbed to the disease after receiving three cycles of chemotherapy. Even though this malignancy is rarely found in the external auditory canal, it should be expanded into the differential diagnosis of an aural polyp and a post aural abscess. The incidence, symptoms, investigations, treatment and prognosis of
malignant melanoma
of the external auditory canal is discussed in this article.

Melanoma
of the ear canal is extremely rare; due to its hidden area, most would present late in their history. We present the first case reported, of a black pigment foreign body mimicking a
melanoma
in the external auditory canal. A 14 year old Chinese male presented with right sided
otalgia
and itch. Otoscopic examination revealed an irregular black naevus in the right auditory canal. An intraoperative excision showed a black pigment foreign body giant cell reaction. Clinicians should be aware of the possibility of a foreign body granuloma of the ear canal that might mimic a
melanoma
.

Mucosal
melanoma
arising in the middle ear or eustachian tube is uncommon. We present a patient with hearing loss and
otalgia
found to have mucosal
melanoma
which occurred in the eustachian tube with extension into the middle ear cavity and external ear canal. Otologic clinics was consulted and biopsy of the mass located at the external canal was performed to ascertain the pathological diagnosis. The patient refused immunotherapy and surgery instead of undergoing radiotherapy and died from hepatic metastasis 8 months later. The mucosal
melanoma
originated from the eustachian tube with extension into the external ear canal is exceedingly rare, and the differential diagnosis should be considered for tumors in external ear canal.
