A retrospective cohort study was carried out on asbestos workers who had received health examinations in 1972 to 1974 conducted by the Osaka Health Center. The subjects, total of 789 (329 males, 460 females) were followed-up for 10 years (Jan. 1, 1975-Dec. 31, 1984). There were sixty-one deaths in the cohort--4 tuberculosis, 12 malignant neoplasms (4 stomach cancers, 8 respiratory cancers including one case of pleural mesothelioma), 18 circulatory diseases, 24 respiratory diseases, and 3 other causes of death. Standardized mortality ratio (SMR) was calculated age and sex-specific death rates for the general population in Osaka between 1975-79 and 1980-84. SMR for all causes of death, stomach cancer, respiratory cancer, circulatory diseases, and respiratory diseases were 1.15, 3.29, 0.75, 3.88, 0.93 and 8.63 respectively. Respiratory cancer and respiratory diseases showed statistically significant (p less than 0.01) excess death with a mean death age of 59 and 56 years old respectively.

Mortality rates from different cancers in migrants to Argentina from 11 individual countries and 6 groups of countries were compared with those in the Argentina-born population and in their countries of origin. Almost all countries of origin had higher mortality rates from gastric cancer than Argentina, but the risk declines in migrants, and for European migrants becomes similar to that of the Argentina-born. In contrast, mortality from oesophageal cancer is significantly lower in European countries than in Argentina. For cancer of the colon and breast, most countries have lower mortality rates than the Argentina-born, the exceptions being Uruguay and Germany, and migrants demonstrate a convergence of risk towards that of Argentina-born. These results suggest that migrants to Argentina undergo changes in some environmental exposure, probably dietary, which give rise to substantial alterations in cancer risk within their lifespan.

Proliferating cell nuclear antigen (PCNA) is a 36 kD nuclear protein associated with the cell cycle. A monoclonal antibody, PC10, that recognizes a fixation and processing resistant epitope has been used to investigate its tissue distribution. Nuclear PCNA immunoreactivity is found in the proliferative compartment of normal tissues. PCNA immunoreactivity is induced in lectin stimulated peripheral blood mononuclear cells in parallel with bromodeoxyuridine incorporation and the number of cells with PCNA immunoreactivity is reduced by induction of differentiation in HL60 cells. In non-Hodgkin's lymphomas a linear relation between Ki67 and PCNA staining was demonstrated. These data suggest that in normal tissues and lymphoid neoplasms, PCNA immunolocalization can be used as an index of cell proliferation. However, in some forms of neoplasia, including breast and gastric cancer and in vitro cell lines, the simple relation between PCNA expression and cell proliferation is lost. In some breast and pancreatic tumours there is apparent deregulation of PCNA with increased expression in tissues adjacent to the tumours. The over-expression in some tumours and in adjacent morphologically normal tissue may represent autocrine or paracrine growth factor influence on PCNA gene expression.

The exact incidence of gastric stump cancer is not known. The reported incidence figures vary greatly, even in studies using similar methods. The aim of this study was to re-evaluate the risk of gastric stump cancer after gastric surgery for peptic ulcer in the Finnish population. A total of 285 patients (252 men and 33 women) operated on for benign peptic ulcer between 1948 and 1954 were followed-up till the end of 1984 (mean for men = 19.4 years, for women = 22.9 years). At the end of 1984 58 patients (20%) were still alive with no verified gastric cancer. One patient developed gastric cancer within five years after the operation and was excluded from the series. Nine patients (3%) were lost to follow-up. Six patients (four men and two women) of the total 285 had developed gastric cancer 6, 7, 8, 21, 25 and 27 years after operation. The risk of getting gastric cancer in a control population (no operation) of equal size and age during a similar follow-up period was 6.71 cases for men and 0.86 cases for women (total = 7.57). The observed number (6 patients) does not differ significantly (P greater than 0.5) from the expected number (7.57). According to this study the risk of gastric cancer is not significantly increased by partial gastrectomy for benign peptic ulcer.

Three cases of colonic carcinoma coexisting with early gastric cancer, are presented. The etiopathogenesis of intestinal-type gastric cancer and colonic cancer are discussed in the light of a possible common mechanism.

A cooperative study on surgical adjuvant immunochemotherapy for prevention of postoperative recurrence of gastric cancer was carried out by Kondo's group from July 1974 to December 1977. A total of 848 patients with gastric cancer underwent curative resection were eligible. Of 848 patients, 819 cases with complete description of patient's background were evaluable: Group A (surgery + MMC + 5-FU:chemotherapy) 253 cases, group B (surgery + MMC + 5-FU + OK-432 or PSK:immunochemotherapy) 282 and group C (surgery alone) 284. The 5-year corrected survival rates of total cases using Cox's proportional hazard model were 76.2% with group A, 73.6% with group B and 73.3% with group C. And the 10-year survival rates were 67.6% with group A, 64.3% with group B and 63.9% with group C. The difference in survival rate among groups A, B and C was not statistically significant. In the evaluation of prognostic factors by Cox's proportional hazard model, level of curative resection, extent of resection, histological stage and Kajitani's classification had an influence upon survival rates, in which degrees of metastasis in lymph node and serous infiltration were also involved. The significance of postoperative adjuvant immunochemotherapy has not been clarified.

By pre-stimulating the lymphocytes of cancer patients with BCGF plus anti-mu chain specific antibody, we have generated human X human hybridomas. The resulting hybridomas preferentially produced IgG class antibody compared with those obtained from non-stimulated lymphocytes. With non-stimulated lymph-node lymphocytes, from stomach cancer patients we selected a hybrid (TOS/H8) producing IgM with broad reactivity against human tumor cells. In contrast, with pre-stimulated lymph-node lymphocytes from hepatoma patients we selected a hybrid (TOH/D5) producing IgG having a specific reactivity with HuTu80 (duodenum carcinoma).

To elucidate the effects of parenteral nutrition on the immunocompetence, we administrated parenteral nutritional support to the malnourished gastric cancer patients during perioperative periods. Changes of peripheral blood natural killer cytotoxicity (NKC) activity (LDH enzyme-release assay) and T lymphocyte subsets (OKT series monoclone antibody indirect fluorescent assay) were monitored before and after nutritional support. The results showed that one week of pre or postoperative parenteral nutrition significantly increased the NKC activity, T-helper, and T-helper/T-suppressor ratio. The total T lymphocytes count may also increase. T-suppressors remained unchanged no matter whether nutritional support was given or not. The authors believe that perioperative nutritional support could improve the immunocompetence of gastric cancer patients to some extent, and promote the restoration of immune depression caused by operation, but it could not eliminate the effects of immune depressing factor produced by the tumor. Intralipid can be used properly as non protein energy source without immunodepressing effects.

Blood magnesium (Mg) and calcium (Ca) concentrations were determined in patients with stomach cancer and in normal subjects. The patients were classified into four stages of malignancy and three metastatic categories. High Mg levels were found in plasma and whole blood of patients in the early stages of stomach cancer, while low Ca levels were found in blood plasma in most stages and in all metastatic groups. By contrast, high Ca levels were observed in whole blood in the later malignancy stages and in two of the metastatic categories. The calcium level in blood plasma does not therefore seem to depend on the stage of malignancy and metastasis, while the calcium level in whole blood and the Mg level in blood plasma and whole blood do appear to depend on the stage of malignancy or metastasis.

Helicobacter pylori is a gram negative bacteria which has recently been associated to tissular changes of the upper digestive tract, however, the causal role has not yet been determined. Of 150 patients studied, 63 had tissular changes associated to Helicobacter pylori (Hp), 8 had Hp without tissular related changes (of whom 3 suffered bulbar ulcus and 1 gastric ulcus); the rest of the patients had hiatus hernia associated to distal esophagitis or pyloric stenosis; and only one patient was found to have normal tissue. A clear associated to distal esophagitis or pyloric stenosis; and only one patient was found to have normal tissue. A clear association between Hp and chronic or atrophic gastritis was determined, but no association was found between Hp and gastric cancer.
