A 53-year-old male presenting with anorexia, intermittent diplopia, general fatigue, headache and vertigo was admitted to our hospital. He was diagnosed as having
gastric cancer
by endoscopy of his upper gastrointestinal tract. Brain computed tomography (CT) showed no abnormalities, but magnetic resonance imaging (MRI) showed slight enhancement in the cerebellar sulcus. Cytological examination of cerebrospinal fluid revealed malignant cells. He became blind one week after hospitalization. We diagnosed his condition as meningeal carcinomatosis (MC) and started radiotherapy.
His
vision improved after four weeks of treatment, and then he became totally blind again. Since his general condition remained poor, we did not perform chemotherapy. He died on the 127th day of hospitalization. MC is a rare pathosis of
gastric cancer
in comparison with leukemia and malignant lymphoma. This disease does not often show characteristic pictorial images, and early diagnosis is difficult. Moreover, it usually manifests itself in its late stages after several months or more of treatment, and it is rare for MC to be present at the time of initial diagnosis. We present a case of
gastric cancer
with meningeal signs present when the primary tumors were diagnosed. Radiotherapy alleviated some of the symptoms, and the patient survived for as long as patients undergoing enforced chemotherapy.

We herein report two cases showing long-term complete remission (CR) in response to S-1 monotherapy. Case 1 was a 65-year-old male diagnosed with an advanced poorly differentiated adenocarcinoma of the stomach with paraaortic lymph node metastases, which disappeared after S-1 monotherapy. Subsequently a total gastrectomy was performed, and histological CR was evident.
His
progress is presently uneventful without recurrence 50 months after surgery. Case 2 was a 59-year-old female who underwent a total gastrectomy with a jejunal pouch. The resected tumor was a medullary type poorly differentiated adenocarcinoma infiltrating the serosa and involving the regional lymph nodes. One year after surgery, endoscopy revealed a recurrent tumor in the jejunal pouch. After the administration of S-1, this recurrent tumor completely disappeared, and she has since maintained CR for 39 months. These cases suggest that a subgroup of patients with advanced
gastric cancer
may attain CR with S-1 monotherapy.

Hereditary Nonpolyposis Colorectal Carcinoma (HNPCC) is the most frequent inherited disease which can lead to the development of tumors in the colon and other locations. Its genetic basis is related to the germline mutation of the Mismatch Repair (MMR) genes. Muir-Torre syndrome is considered one of the subtypes of this disease, in which the HNPCC tumor spectrum is frequently associated with sebaceous carcinoma of the skin or keratoacanthoma. A 57 years old male patient is presented with a mucinous carcinoma of the caecum and an adenocarcinoma of the pancreas head. A malignant sebaceous carcinoma was removed from his left neck area.
His
family history was significant for two cases of colon carcinoma, two cases of
stomach cancer
and a case of metacron endometrial and skin tumor as well. Both the colon carcinoma and the skin tumor proved to be microsatellite unstable. An Arg>Pro switch missense mutation was found in codon 265 of the hMLH1 gene. This error was found in 4 other members of his family. The detected genetic alteration was considered pathogenic and was not published yet in English literature. The significance of this particular case is the rare tumor association in a patient with Muir-Torre syndrome (MTS). In cases of sebaceous skin lesions, evaluation of family history is of utmost importance in the early detection of HNPCC and in the follow up care of family members with the particular mutation.

A 56-year-old man was admitted to our hospital for
gastric cancer
. He had dyspnea before admission. Chest CT scan showed massive pericardial and pleural effusion. He was treated by cardiac drainage, and cytology of the effusion showed class V. We injected mitomycin C 10 mg into the pericardiac cavity, and also administered S-1 100 mg per day.
His
dyspnea improved and he was discharged. Afterward the dyspnea re-appeared, and he died 9 months after the diagnosis. We report this rare case of cardiac tamponade induced by
gastric cancer
responding to S-1 with a review of the literature.

post-operative course was complicated by acute pancreatitis and intra-abdominal sepsis. The patient eventually succumbed to multiple organ failure despite surgical debridement and drainage, together with aggressive antibiotic therapy and nutritional support. For patients with pancreas divisum or dominant duct of Santorini who fail to follow the normal post-operative course after gastrectomy, clinicians should be alert to the possibility of PGAP as one of the potential diagnoses. Early detection and aggressive treatment of PGAP might improve the prognosis.

A 69-year-old male patient has been diagnosed hepatitis B a couple of years ago at our hospital.

follow up CT revealed multiple hepatocellular carcinomas (HCCs) in May 2007. Transarterialchemoembolization was selected for treatment. A couple of months later, carcinoma in segment eight was diagnosed viable by CT. Radiofrequency ablation (RFA) therapy was performed for further treatment but was incomplete. On the other hand,
gastric cancer
was discovered at the same time. Total gastrectomy with regional lymph node dissection and splenectomy were performed. Pathological examination of the dissected lymph nodes revealed not only the metastases of the
gastric cancer
but also HCC. Recently, RFA treatment is growing in number for HCC. Although lymph node metastases in HCC are seldom seen, our case suggests that RFA may have led to the unexpected metastases to the lymph nodes.

The patient is a 62-year-old male who was treated for macroscopic-type 3
gastric cancer
by total gastrectomy (D2) and splenectomy.
His
disease was recorded as pT3, pN2, Stage IIIB, curability B, and S-1 was started as postoperative adjuvant chemotherapy. One year later, during the adjuvant chemotherapy, CT revealed para-aortic lymph node enlargement. Recurrence was diagnosed, and S-1 (100mg/body, days 1-28) + CPT-11 (80 mg/body, days 1, 8, 15, and 22) combination therapy was started. After 4 courses, the lymph node had markedly regressed (regression rate: 72. 2%), and CPT-11 administration was changed to biweekly (days 1, 15, 28). A total of 9 courses were administered, and during the 9th course it disappeared. Adverse events during the 9 courses consisted of only grade 1 alopecia and grade 2 diarrhea and leukopenia, and none of them were serious. At the patient's request, only oral S-1 was continued thereafter, however, CR has been maintained for 30 months. S-1+CPT-11 combination therapy can be conducted safely on an outpatient basis, and it has been superior in terms of continuity of treatment.

clinically diagnosed as cT3N1H0P0M0, stage IIIA.
His
treatment was supposed to be daily oral administration of 120 mg S-1 for 28 days. At 21 days, this treatment was stopped due to severe appetite loss. The histological diagnosis of the resected stomach revealed complete disappearance of cancer cells in the stomach and the regional lymph nodes.
