This report describes a 73-year-old male with early
gastric cancer
(type IIc) superimposed on infestation of a parasitic larva. Eosinophilic granulomas surrounding a dead worm were seen in the submucosa just beneath the intramucosal tubular adenocarcinomatous lesion measuring 1 X 1cm at the anterior wall in the acid-secreting area. The cancer cells showed lowered and altered
mucin
production in comparison with the surrounding non-cancerous fundic mucosa. The parasite was identified as an Anisakis-like larva by the presence of typical lateral chords. An immunohistochemical examination revealed that the cancer cells were more strongly positive for IgA and secretory component than the surrounding non-cancerous mucosa, and that IgG, IgA and IgM were detected in plasma cells around the granulomas but IgE and IgD were not. The possibility that the Anisakis-like larva preferentially infested the cancerous mucosa because of the change in
mucin
, local defect in acid secretion and/or other structural alterations in the area is discussed.

A total of 171 gastric carcinomas comprising 69 advanced cancers and 102 early cancers were examined immunohistochemically for lysozyme. Tumour cells containing lysozyme were detected in 65 cases or 38% of the 171
gastric cancer
cases. The incidence of these cells did not differ remarkably by histological type and infiltrative growth of gastric carcinoma. Of the foregoing 65 cases, two well-differentiated adenocarcinomas and three signet ring cell carcinomas had numerous lysozyme-containing tumour cells, 13 had many argentaffin or argyrophil cells, and 40 had various amounts of several types of
mucin
. In addition, tumour cells containing both lysozyme and
mucin
could be identified. No correlation could be observed between lysozyme immunoreactivity in the tumour cells and cellular infiltration of granulocytes or macrophages around the tumour. The lysozyme appeared to be produced by tumour cells. The two year survival rates indicate a tendency for advanced gastric cancers containing lysozyme to have a poor prognosis.

The "borderline lesion" of the stomach (severe dysplasia of the protruded type) is nowadays considered to be a true precancerous lesion. Histologically, this lesion is characterized by a proliferation in the area of the glandular neck, combined with severe cellular atypia. Investigation by means of transmission electron microscopy reveals the structure of these cells to be very similar to the structure of cells of an intestinal-cell-type carcinoma of the stomach. In addition to signs for a loss of differentiation of the cells, such as the absence of rough-surfaced endoplasmatic reticulum,

granules, and of the polarity of the cells, remarkably frequently luminal bulges and "blebs" developed to various extents can be detected. Morphometric analysis is delineating the tendency of structural features of the borderline lesion to develop toward the
gastric cancer
cell. These irregularities might be expressions of the beginning of expansive growth and signs pointing to differentiation into malignant neoplasia.

A total of 164 gastric carcinomas comprising 97 advanced cancers and 67 early cancers were examined immunohistochemically for human chorionic gonadotropin (hCG). Tumor cells containing immunoreactive hCG was found in 7 cases or 10.4% of the early cancer cases and in 11 cases or 11.3% of the advanced cancer cases. Moreover, the incidence of hCG-beta subunit positive cells was 8.2% in the advanced cancer cases and 6.0% in the early cancer cases. The incidence of these cells did not differ significantly by histological type and by infiltrative growth of gastric carcinoma. However, hCG-containing tumor cells were frequently detected in poorly differentiated or undifferentiated area within the tumor. In addition, these cells did not contain
mucin
or argyrophil granules. The three year survival rates indicated a tendency for advanced
gastric cancer
containing hCG to have a poor prognosis.

showing serum AFP over 20 ng/ml, 14 cases (3.2%) showed AFP positive reaction in cancer cells by immunostaining. AFP localization in cancer cells classified into marginal type, granular type, diffuse type and inclusion type. Mixed combination of these types increased in high serum AFP level. Cytomorphology of AFP positive cells was classified to clear cell type, hepatoid cell type and round cell type. Clear cell type showed endodermal sinus like pattern in histomorphology. Previous studies from our laboratory have shown that benzyl-alpha-GalNAc inhibits the glycosylation of
mucin
in colon cancer cells. In this study, we determined whether benzyl-alpha-GalNAc inhibits
mucin
glycosylation in KATO III
gastric cancer
cells. We also examined its effects on expression of
mucin
antigens, and compared the mucins made by KATO III with those of a colonic cancer cell line, Caco-2. Results of these experiments suggest that benzyl-alpha-GalNAc (2 mM) inhibited [3H]glucosamine labelling of mucins by 82% in KATO III and by 70% in Caco-2. For both cell lines, the
mucin
secreted in the presence of benzyl-alpha-GalNAc was less acidic. Both cell lines secreted benzyl-oligosaccharides, but those from KATO III (8-9 sugars) were larger than those from Caco-2 (6-7 sugars). In mucins purified from the medium of treated cells, peripheral carbohydrate antigens (sialyl Lex in KATO III and terminal fucose in Caco-2) were decreased (compared with control), while core carbohydrate antigens (T antigen in both cell lines and sialyl Tn in Caco-2) were increased. Western blots of cell homogenates showed differences between KATO III and Caco-2 in MUC 1 apomucin protein antigens, in sialyl Lex and in sialyl Tn antigens. We conclude that benzyl-alpha-GalNAc does inhibit the glycosylation of
mucin
in KATO III
gastric cancer
cells as in human colon cancer cells, but that alterations in

antigens occur in a cell line-specific manner.

Intestinal metaplasia of the gastric mucosa is an important lesion in the pathogenesis of
gastric cancer
. The expression of a
mucin
-associated antigen, sialosyl-T antigen, in the tissue of
gastric cancer
was investigated immunohistochemically to assess its biological role. Sialosyl-T antigen, detected by the monoclonal antibody TKH2, was present in 30 (23.6 per cent) of 127 lesions, and expressed sporadically in intestinal metaplasia. Patients negative for sialosyl-T had a significantly better 5-year survival than those who were positive (70.4 per cent versus 47.2 per cent). Grading by sialosyl-T antigen expression and the DNA ploidy pattern had a predictive value for prognosis in patients with
gastric cancer
. The biological role of sialosyl-T antigen is not clear but the
mucin
alteration reflects upon the biological behaviour of
gastric cancer

The finding of free tumour cells within the peritoneum at the time of laparotomy for gastrointestinal cancer is an important indicator of prognosis and may help select treatment. The aim of this study was to improve the methods whereby these cells could be retrieved and identified. Free peritoneal cancer cells were found in 6 out of 18
gastric cancer
patients. All 6 patients had tumour invasion through to the serosa and subsequently died of tumour recurrence. None of the 18 Dukes' B and C colorectal cancer patients had free peritoneal cancer cells. Only 2 of a further 5 patients with extensive colorectal tumour spread had positive cytologies. The conventional
mucin
stain Periodic Acid Schiff (after diastase digestion) and the monoclonal antibody stain B72.3 were the most useful in identifying free peritoneal cancer cells. Peritoneal brushings did not offer any advantages over conventional peritoneal washings in the retrieval of free tumour cells.

We measured the concentration and distribution of tumor associated antigens, TAG-72 and CEA, in
stomach cancer
by in vitro quantitative autoradiography (IV-QAR). Frozen sections of 33 specimens were incubated with varying concentrations of 125I-labeled CEA-79.1 and B72.3 antibodies specific for carcinoembryonic antigen (CEA) and tumor-associated glycoprotein-72 (TAG-72), respectively. Computer analysis of specific antibody binding gave maximal binding values which were equal to the concentrations of the antigen or epitope. TAG-72 was detected in 25 specimens, at a concentration ranging from 8.4 to 562.9 pmol/g. CEA was detected in 32 of the 33 specimens and its concentration ranged from 8.8 to 525.3 pmol/g. The distribution of TAG-72 by IV-QAR coincided with that of the tumor cells in 41.4% of the pathologic lesions. The distribution of CEA coincided with the tumor cells in 80.5% of pathologic lesions, nearly twice the TAG-72. The concentration of TAG-72 was significantly higher in mucinous adenocarcinoma and
mucin
containing adenocarcinomas than other types of adenocarcinomas. There was no significant difference in the concentration of CEA among the pathologic types of
stomach cancer
. In summary,
stomach cancer
exhibited wide variations in TAG-72 and CEA expression. CEA expression was more frequent and homogeneous than TAG-72.

We succeeded in establishing a human gastric carcinoma cell line (KE-97) from oncocytes obtained from the mesentery disseminated metastatic focus of a 52-year-old male
stomach cancer
patient. From a histopathological point of view the gastric carcinoma was clearly a
mucin
-producing mucinous carcinoma, portion of which were mixed with poorly differentiated adenocarcinoma and signet-ring cell carcinoma. The oncocytes were fused into a mass and exhibited a suspended proliferating system, with a doubling time of about 28.8 hours. Transplantation of the cancer cells into skid mice resulted in a tumor system in all cases, and histologically, mucinous vacuoles were found in the cell membranes. With immunological staining they were found to be positive for anti-CEA antibodies, anti-CA19-9 antibodies and anti-ICAM-1 antibodies. Autopsy found extensive hematogenous metastasis (lung, liver) and cancerous peritonitis. KE-97 is mucinous carcinoma, and it was reported with the belief that it is a useful cell line upon the investigation of its cancer metastasis mechanism and cytological characteristics.
