We studied 161
gastric cancer
patients with P0, H(+) and 51 colorectal cancer patients with P0, H(+) from among cancer patients of the digestive organs and obtained the following conclusions. The effective treatment for synchronous hepatic metastasis was regarded as the group with surgical removal of the primary lesion plus hepatic resection plus chemotherapy, demonstrating most favorable prognosis in both gastric and colorectal cancer patients. Prognosis of the group treated with surgical removal of the primary lesion plus hepatic resection plus chemotherapy, was the most excellent and was followed by the group with surgical removal of the primary lesion plus chemotherapy and group with surgical removal of the primary lesions and group surgical removal of the primary lesion in this order. Concerning chemotherapy after surgical removal of the primary lesion, continuous intraarterial infusion therapy with FML regimen combining
Lentinan
revealed more favorable prognosis also in both gastric and colorectal cancer patients. Hepatic resection with aggressive reduction surgery was of significance in the treatment for the patients with hepatic metastasis of H1 and H2. Long-term survival is also expected for the patients with metachronous hepatic metastasis of H1 by hepatic resection plus chemotherapy.

Five patients with distant metastasis from colorectal cancer who had good nutrition were treated with continuous intravenous administration of 5-FU infusion (400-450 mg/body/day) by infusor. The other five patients with advanced
stomach cancer
who could not have so good nutrition were receiving home parental nutrition (HPN, 1,200 kcal/day) and continuous or intermittent 5-FU infusion (250-500 mg/body/day) using
Lentinan
at the same time. In colorectal cancer, one partial response (PR) and one complete response (CR) were obtained. And in
stomach cancer
, only one NC were obtained but we could perform these HPN and chemotherapy for 13 months at home. According to our trial, we thought these chemotherapy and nutrition at home were useful to improve the quality of life of patients with GI tract cancer.

Lentinan
was administered for
gastric cancer
in order to determine what type of changes would occur as a consequence. In an experimental study, the intraperitoneal administration of lentinan on cancerous tissues caused a marked development of reticular fibers, along with an enhanced interstitial response. Findings which support the relationship between a marked development of reticular fibers and an anti-tumor effect, were also obtained. Furthermore, lentinan was administered for human
gastric cancer
to evaluate whether or not an increase in interstitial response would be observed. As a result, reticular fibers developed in tumor sites with a resultant fragmentation of cancer cell nests. Many T lymphocytes infiltrated cancer sites where the lentinan was administered. These findings suggest that the intratumor administration of lentinan enhanced an interstitial response and also activated an anti-tumor immune response in the human
gastric cancer
tissues.

This report describes the effect of intraperitoneal administration of CDDP for the treatment of peritoneal dissemination (P) in 20 patients with
gastric cancer
. Eight patients after palliative resection, 4 after non-resection and 8 with recurrent cancer were given 25 mg of CDDP through a reservoir (Infuse-A-Port) implanted in the abdominal wall once a week. Simultaneously, 400 mg/body/day of UFT and 2 mg/body/week of
Lentinan
were administered. Twelve of the patients died of disease at 6-19 months (median 7 months), but 8 have survived 3 to 15 months (median 6 months). Complete and partial responses were obtained in 7 of the 8 patients with recurrence (88%) for malignant ascites. No patient showed any toxicity from treatment. All patients receive the treatment in the outpatient clinic after discharge, and 17 patients have shown a long hospital-free survival.

Three cases of postoperative
gastric cancer
with peritoneal dissemination were treated by combined administration of CDDP, 5-FU and
Lentinan
. CDDP was given intravenously at 20 mg/body on days one to five. At the same time, 5-FU was infused continuously 250-500 mg/body per day for more than two weeks, and
Lentinan
was given at a dose of 2 mg per week. Two courses were repeated every 3 weeks. This combination treatment was very effective because the median survival rate for the three patients was 13 months with no sign of recurrence, and the quality of life has been good.

Eleven patients with postoperative-stage IVb
gastric cancer
were treated by combined administration of CDDP, 5-FU and
Lentinan
. CDDP was given intravenously at 20 mg/body on days one to five. At the same time, 5-FU was continuously administered 250 or 500 mg/body/day via the central venous catheter for more than two weeks, while
Lentinan
was given at the dose of 2 mg per week. Two courses were repeated every 3 weeks. The mean response duration and survival period for the eleven patients were 13 and 21 months, respectively, and one- and two year survival rates were 82% and 27%. This combination therapy was thus considered to be very effective. Next, the pharmacokinetics of long-term CDDP and 5-FU was studied. Our method could maintain the optimum serum concentration of total CDDP. Furthermore, the continuous infusion of 5-FU at a daily dose of 250 mg/day could sufficiently maintain the effective plasma concentration, although its peak fluctuated over time. Thus, in the plasma level of 5-FU in patients in poor condition requires periodic measurement.

Satisfactory therapeutic effects are rarely obtained with oral chemotherapy for
gastric cancer
. We have experienced successful treatment for synchronous hepatic metastasis of
gastric cancer
with 5'-DFUR and
Lentinan
. The patient was a 78-year-old female, diagnosed as having
gastric cancer
with multiple hepatic metastases, who underwent gastrectomy. Immunohistochemistry of the resected specimens with anti-thymidine phosphorylase (dThdPase) antibody yielded positive results for dThdPase in the primary tumor as well as the hepatic metastases. Two months after surgery, administration of 400 mg of 5'-DFUR per day and 2 mg i.v. of
Lentinan
every other week was started. Four months after discharge, carcinoembryonic antigen (CEA) in plasma showed an abrupt logarithmic decline. Furthermore, a 99% reduction in hepatic metastases was demonstrated by abdominal CT. At present, 22 months after surgery, the patient is managed on an outpatient basis with no complaints of any side effects. Immunochemotherapy using 5'-DFUR and
Lentinan
may be effective against gastric malignancies expressing dThdPase activity.

Six patients with
gastric cancer
, stage IIIA to IV, received intraabdominal cisplatin (CDDP) at laporotomy. This was followed by postoperative intravenous infusion of mitomycin C (MMC), CDDP or fluorouracil (5-FU). When these patients recovered to the extent that permitted oral medication, an immunochemotherapeutic regimen containing either oral UFT (uracil and tegafur) or 5'-deoxy-5-fluorouridine (5'-DFUR), plus intravenous
Lentinan
(
LNT
) was administered for a period ranging from one and a half to two years. This postoperative immunochemotherapy was successful in all 6 patients. No relapse has been observed in any of them for at least 4 years. These findings indicate that the chemotherapeutic strategy of administering intraabdominal CDDP immediately after surgery, followed by postoperative immunochemotherapy with pyrimidine-fluoride products plus
Lentinan
on an outpatient basis may be useful in the treatment of T3 or T4 gastric carcinoma with metastasis in the regional lymph nodes.

The patient was a 50-year-old female with peritoneal metastasis of Type 4
gastric cancer
. She underwent a relative curative resection with total gastrectomy and peritonectomy. Postoperative chemotherapy with 5'-DFUR following 5-FU and CDDP was performed. Thirteen months after surgery, cancer recurrence was suspected due to elevated levels of the serum tumor markers carcinoembryonic antigen (8.9 ng/ml) and alpha fetoprotein (85.8 ng/ml). She was additionally treated with UFT 300 mg/day and
Lentinan
2 mg/week. The serum tumor markers decreased gradually returned to normal levels. At 5 years and 8 months after surgery, she is alive without any sign of recurrence.

It is reported that TS-1 administered orally shows a significant anti-neoplasm effect on advanced
gastric cancer
, and, furthermore, approximately 70% or greater effectiveness is reported for combination chemotherapy with cisplatin (CDDP).
Lentinan
is reported to extend the survival period in advanced cancer, and in combination with Tegafur. In the present study, combination chemotherapy with TS-1/CDDP/
Lentinan
was conducted for patients with inoperable advanced
gastric cancer
, and the validity, safety and resultant QOL of the treatment were evaluated. TS-1 was administered for 3 weeks at 80 mg/m2, followed by withdrawal for 2 weeks, and CDDP was prescribed once for patients at 70 mg/m2 on the 8th day after starting TS-1 administration. For patients aged 80 or above, however, the dose was reduced, and given separately to the patients.
Lentinan
was administered at 2 mg/week. The rate of effectiveness for the 9 registered patients was 100%. This high rate was obtained regardless of changes in the histopathological findings. Critical side effects (grade three or above) were anemia and pigmentation, in one case each. An improvement in QOL was also observed for combination therapy including
Lentinan
. In cases of inoperable advanced
gastric cancer
, TS-1/CDDP combination chemotherapy showed higher efficacy regardless of the pathological alterations, and higher and sustained improvement of QOL was also observed with the addition of
Lentinan
to the protocol.
