-year-old female was admitted to the Fukui Red Cross Hospital because of abnormal genital bleeding in Feb., 1987. She underwent gastrectomy for early
gastric cancer
(IIc) 5 years earlier, mastectomy for early breast cancer (stage I) 3 years earlier and radiation therapy (60Co 350rad x 25 days) and chemotherapy (5FU, 200 mg/day, orally) for uterine cancer (stage IIb) 2.5 years earlier. These 3 cancers had quite different histological features one from the other. Laboratory investigation revealed leukocytosis (216, 100/microliters) with 90% blasts. The bone marrow was hyperplastic with 87% blasts. Although 90% of blasts had myeloperoxidase activity, surface marker analysis of the blasts revealed T-cell (CD2, CD3) phenotypes suggesting that the disease was so-called hybrid acute leukemia. She died of intracranial hemorrhage on the 2nd hospital day. An autopsy showed generalized leukemic infiltration and residual uterine cancer without metastasis to other sites. In our country, quadruplicate malignancies including hematological malignancy have been rarely described. The possible pathogenesis of multiple cancers has been discussed.

gastric cancer
associated with metastatic carcinomatosis of the bone marrow, microangiopathic hemolytic anemia (MAHA) and disseminated intravascular coagulation (DIC) was reported.

-year-old woman complained of lumbago and general fatigue. At the time of admission, besides anemia, jaundice and a tendency to bleeding, the laboratory data showed, hyperbilirubinemia, elevated FDP and hemolytic anemia with fragmented red cells. The bone marrow was infiltrated by carcinoma, and an upper GI series examination showed Borrmann II type
gastric cancer
on the greater curvature of the antrum. After remission of both MAHA and DIC by mitomycin C, neothramycin and FOY, the patient successfully underwent subtotal gastrectomy. However, she died of cerebral hemorrhage on the 96th postoperative day.

is presented. Histological examinations of gastric and colonic polyps which resembled juvenile type revealed no malignant cells. The patient slowly recovered after subtotal gastrectomy and elemental diet supplement with antiplasmin agents. As for as is known to the authors, a total of 69 cases with this syndrome were reported in Japan including our patient. Of these cases 8 had cancer of the gastrointestinal tract. Two of them, including our case, had

. Another one was associated with
gastric cancer
but not intestinal polyposis. The remaining 5 cases were associated with colonic cancer. Caution must be paid to the fact that cachexia or malabsorption in some patients with this syndrome results from gastrointestinal malignancy. The prognosis of this syndrome consisted of 20 deaths including 2 postoperative deaths, 33 of improvement including 4 of natural remission and 4 of remission after gastrectomy or colectomy, 7 unchanged and the remainder not described.

We present herein an extremely rare case of metastasis from a gastric carcinoma to the mesocolon.

-year-old woman underwent a laparotomy for
gastric cancer
with an intra-abdominal mass. Her serum alpha-fetoprotein level was very high at 3,560 ng/ml. The abdominal mass was subsequently revealed to be a metastatic tumor of the transverse mesocolon derived from an alpha-fetoprotein-producing gastric carcinoma, but no other metastatic focus was found. An immunohistochemical study revealed alpha-fetoprotein-positive cells in both lesions. The serum alpha-fetoprotein level became normal after the operation which was followed by a course of chemotherapy, and no recurrence has been observed thus far in 6 months of follow up.

-year-old woman underwent subtotal distal gastrectomy for II a+ II c type early cancer of the gastric antrum. Histological type was poorly differentiated adenocarcinoma with medullary proliferation, and the lesion invaded the submucosal layer. Two years and 6 months after the operation, multiple liver tumors were found on the CT scan. A surgical resection of the liver tumor was performed. Microscopically, the liver tumors were compatible with
gastric cancer
. The remnant liver metastases were treated by intrahepatic infusion chemotherapy with Angiotensin II human (Delivert) using a subcutaneous implanted pump. The liver metastases disappeared on the CT scan after 3 courses of chemotherapy, but bone metastasis occurred after 2 months. This mode of chemotherapy was therefore considered a useful treatment for liver metastasis in
gastric cancer
. We concluded that not only intrahepatic infusion chemotherapy with Angiotensin II human but also another systemic chemotherapy was necessary to treat patients with liver metastasis in

with multiple liver metastases had been treated with 5'-DFUR 1400 mg/m2/day, p.o. day 1-day 4/2w and CDDP 80 mg/m2 i.v. day 5/4w, which was repeated for 4 cycles. After 2 cycle treatment, all metastatic lesions in the liver disappeared on the computed tomography scan, indicating a complete response. The primary gastric lesion was reduced, indicating a partial response. There was no significant side effect during the 4 cycles of this therapy. He is alive 6 months after the therapy with a partial response. This 5'-DFUR and CDDP combination therapy seemed to be effective for advanced

and laparoscopic examination revealed peritoneal metastasis. Her abdominal cavity was treated with MTX and CDDP and she was given intravenous administration of 5-FU/LV. Repeated laparoscopy revealed complete disappearance of peritoneal seeding, and a partial gastrectomy was done. The histopathological findings of resected specimen showed the significant effects of preoperative chemotherapy. She has been living for more than 2 years with no recurrence after initial chemotherapy.

were found. The patient thereafter underwent an operation for those pathologies after the endoscopic removal of a choledochal stone; cholecystectomy, and a distal gastrectomy with regional lymph node dissection for

The proposed procedures of gastrectomy and cholecystectomy were completed without any major difficulty because no markedly enlarged collateral veins were found in the area where the regional lymph node dissection was carried out. such as that seen in the present case may be increasingly encountered in the future. The greatest caution, however, needs to be exerted at operation to minimize any unexpected bleeding and to avoid any interruption of the porto-portal shunts in such cases. Further, the reestablishment of the portal blood supply to the liver might be required in advanced cases of

gastric cancer
and peritonitis carcinomatosa was treated with 5'-DFUR 600 mg/body p.o. daily, CDDP 50-80 mg/m2 i.v. on day 1 and MMC 6 mg/m2 i.v. on day 2 after intraperitoneal injection of CDDP and OK-432. The cycle of CDDP and MMC was repeated every 4 or 5 weeks. After 15 cycles of treatment, the primary gastric lesion disappeared and regional lymph node metastasis was reduced. Because of relapsing of the original lesion and concomitant renal dysfunction, a changed regimen of CDDP and Carboplatin was tried. Then the gastric lesion was reduced and lymph node metastasis disappeared. The patient has lived for about 3 years after diagnosis, but his gastric lesion is gradually worsening with duodenal invasion. This case indicates that these combination chemotherapies are quite promising as an effective treatment for advanced
gastric cancer
and good quality of patient's life.

-year-old female noted a tumor in the right orbital region. She had had bilateral breast cancer 2 years before and

5 months before. Histopathology had shown stage II invasive ductal cancer (scirrhus) in the right breast and stage III invasive lobular cancer in the left. Signet-ring cells were present in the breast and gastric cancers. Biopsy of the right lower eyelid showed poorly differentiated adenocarcinoma with signet-ring cells. Indian file pattern, which is specific for invasive lobular cancer, was also present, suggesting that the orbital tumor had metastatized from the left breast cancer. Genetic analysis of the
gastric cancer
using polymerase chain reaction showed a mutation at exon 8 of the p53 tumor suppressor gene, indicating the cancer to be metastatic. These results led to the conclusion that invasive lobular cancer of the left breast was the primary lesion for the gastric and orbital metastases. This case also illustrates that signet-ring cells, which are usually seen in
gastric cancer
, may be present in invasive lobular breast cancer and in orbital metastasis.
