Using Avidn-biotin-peroxidase complex (ABC) immunohistochemical method, 68 gastric cancer specimens and 20 non-cancerous stomach specimens were examined for ER. In 38 of 68 gastric cancer specimens, ER concentrations were also biochemically determined by Dextran-coated charcoal (DCC) assay. The results showed that ER was absent in noncancerous stomach tissues. ER was present in 21 (30.9%) of gastric cancers. The ER positive cases were chiefly poorly differentiated gastric cancers. There were no significant statistical differences in the patient's age or sex, size of primary tumor and the ER positive rates. The results indicate that endocrine therapy might be useful for the ER positive gastric cancer patients.

We examined whether or not preoperative impaired bactericidal activities of polymorphonuclear neutrophils (PMN) are associated with infections following surgery for esophageal cancer. Intracellular killing (KI), superoxide anion-producing capacity (SOP), and myeloperoxidase (MPO) activity were measured in 22 patients with esophageal cancer, 27 with gastric cancer, and 13 age-matched controls. The average level of KI was significantly depressed in patients with esophageal cancer or with gastric cancer, to a similar extent, compared to findings in controls, but SOP was not. In esophageal cancer patients, the SOP level was significantly higher in those with postoperative septic complications than in those without such problems, whereas the KI level was depressed to a similar extent in both. Therefore, a depression of KI with elevation of SOP of PMN may serve to predict complications of infection following surgery in patients with esophageal cancer.

We compared three different strategies for measuring basic fibroblast growth factor (bFGF) by fluorometric enzyme immunoassay (EIA). After optimizing conditions, we found that a primary anti-bFGF MAb directly conjugated with peroxidase gave the best detection limit for recombinant bFGF (approximately 30 ng/L, 3 pg/assay tube) in a two-site sandwich assay. The detection limit of methods based on biotinylated primary MAbs or on secondary antibodies followed by streptavidin-conjugated peroxidase was slightly lower than that of the above method. Using the most sensitive EIA examined in this study, we made a preliminary measurement of immunoreactive bFGF in sera of apparently healthy people and found it to be 190 (SD 32) ng/L (n = 48), in agreement with an earlier reported value (30-206 ng/L). Also, the concentration of immunoreactive bFGF in sera was above normal in 19 of 31 patients with stomach cancer.

The authors used helix pomatia agglutinin (HPA) staining to examine 163 primary gastric carcinoma isolates, using the avidin-biotin-peroxidase complex method. The positive HPA staining rate was 59% (96/163) for the primary tumors, and the positive staining correlated well (with statistical significance) with tumor enlargement, penetration, lymphatic invasion, and metastasis (P less than 0.01), and with infiltrative spread (P less than 0.05). Patients with gastric cancer showing positive HPA staining had a lower survival rate (P less than 0.001), particularly when the cancer cells were present on the serosal surface. Careful follow-up and intensive postoperative therapy are required for patients with an HPA-positive advanced gastric carcinoma.

An ecological survey on diet, life style and cancer mortality was carried out in 65 rural counties in China, using a questionnaire comprising 285 questions on environmental factors, dietary practices and other life style characteristics; in addition, blood, urine and food were collected and analysed. Two interpretations of preliminary data are presented. One is for the finding of a positive correlation between a latent variable, namely general nutritional status, and cancer mortality rates; the other is for a positive correlation between lipid peroxidation (plasma lipid peroxidase and copper) and oesophageal and gastric cancer mortality, and a negative correlation between dietary oxidants (plasma selenium, ascorbic acid and retinol) and mortality from oesophageal and gastric cancer.

We examined the localization of epidermal growth factor (EGF) in 185 specimens of primary human gastric cancer using the avidin-biotin peroxidase complex immunohistochemical method on formalin-fixed paraffin-embedded sections. Thirty-four per cent of the gastric cancer specimens were positive for EGF, which was mainly located in the cytoplasm of the cancer cells and occasionally in the stromal cells, but was not detected in non-cancerous gastric epithelium. Moreover, the presence of EGF in gastric cancer was correlated with gastric wall invasion and lymph node metastasis. EGF was found more often in advanced cancers than in early ones (p less than 0.01), and also more often in cancers with lymph node metastasis than in those without (p less than 0.05). The five-year survival of patients with EGF-positive tumors was worse than that of patients with EGF-negative tumors (p less than 0.05). The presence of EGF in human gastric cancer may thus represent higher malignant potential.

Established human colon cancer cells with distinct degrees of differentiation (LoVo, well-differentiated; SW620, intermediate differentiation; and SW1116, poorly differentiated) were used to produce monoclonal antibodies (MoAbs) by standard hybridoma techniques. Specificity was tested by an enzyme-linked immunosorbent assay against human foreskin cells, 7 established human colon cancer lines, a panel of 17 established human tumor lines of different histological origins, purified carcinoembryonic antigen, panels of red blood cells, and a suspension of lymphocytes obtained from 30 random normal donors. MoAb LoVo-F4 3E4/1A1/2E10 (MoAb F4/2E10) reacted with five colon cancer lines and only slightly with MCF-7 cells (estrogen receptor positive breast carcinoma). MoAb LoVo-F4 3E4/1A1/5C10 also reacted with the previous five colon cancer lines and with two gastric cancer lines. A MoAb obtained with a LoVo 3 M KCl membrane extract reacted exclusively with LoVo cells. MoAb SW620-F1 4E5/1A3 reacted with only three colon cancer cell lines and an estrogen receptor negative breast cancer line. MoAb SW1116-F2 1E3/1A1 reacted with four colon carcinoma cell lines, one gastric cancer line, MCF-7 cells, and a lung cancer line. MoAb SW1116-F2 1F3/1B1 reacted intensely with purified carcinoembryonic antigen and with every carcinoembryonic antigen-producing cell line available in our laboratory. Further studies concentrated on the immunoglobulin G1 MoAb F4/2E10. We demonstrated that the purified MoAb did not inhibit binding of MoAb CA19-9 to any colon Ca lines and reacted with fresh human colon carcinoma specimens regardless of whether they were processed by cryostat or paraffin embedding after fixation in formalin for 24 through 96 h. Using the peroxidase-antiperoxidase technique, MoAb F4/2E10 did not react with 23 normal adult and 18 fetal (less than 3 months old) human tissue specimens. When tested on 312 specimens of diverse histological origins and diseases, the MoAb was positive in 57 of 62 colorectal cancers, in 12 of 19 villous adenomas, in 5 of 7 adenomatous polyps, and in 10 of 12 cases of ulcerative colitis. With the exception of 2 of 15 cases of Crohn's disease that were slightly positive, all tissues from nonmalignant diseases (regardless of histological origin) were consistently negative. There was only weak reactivity in 2 of 18 breast cancers, 7 of 21 squamous cell carcinomas, 4 of 27 lung tumors, 1 of 13 kidney carcinomas and in 7 miscellaneous tumors.(ABSTRACT TRUNCATED AT 400 WORDS)

A case of primary gastric cancer without hepatic metastasis showing extremely high alpha-fetoprotein (AFP) levels is reported. This case illustrates the application of the immuno-peroxidase technique to ascitic fluid cytology. Papanicolaou-stained smears of the ascites permitted the diagnosis of a metastatic carcinoma. A positive reaction to AFP was demonstrated in the tumor cells in the ascitic fluid cellular samples as well as in the paraffin-embedded tissue section of the primary gastric carcinoma. Rising AFP levels were also detected in ascitic fluid. AFP fractionation using lectin-affinity-crossed-line immunoelectrophoresis showed the hepatic rather than yolk sac type. Reports of such occurrences are few; no study, to the best of our knowledge, has previously documented cytological and immunocytochemical diagnosis in ascitic fluid. AFP-producing gastric cancer should be considered in the differential diagnosis.

We used anti-scirrhous carcinoma monoclonal antibody (MAb) S202, which reacted strongly with scirrhous gastric cancer, to evaluate the depth of cancer cell invasion into the stomach wall. Sixty-eight scirrhous gastric cancer specimens were stained with the avidin-biotin peroxidase complex method using MAb S202. There were some discrepancies between the histological evaluation of the immunoperoxidase (IP) and hematoxylin-eosin (HE) staining regarding the depth of tumor invasion. In two of the 14 HE staining cases in which carcinoma had extended as far as the proper muscular layer, IP staining revealed that the lesions had actually penetrated this layer. As well, in two of the 18 HE staining cases in which carcinoma cells had not penetrated as far as the serosa, IP staining revealed that they had, in fact, reached the serosal surface. The invasive carcinoma cells were more easily discerned using IP staining with MAb S202 rather than using histological staining with HE. Thus, MAb S202 may be useful for precise identification of carcinoma cells.

A 71-year-old female was admitted to the Fukui Red Cross Hospital because of abnormal genital bleeding in Feb., 1987. She underwent gastrectomy for early gastric cancer (IIc) 5 years earlier, mastectomy for early breast cancer (stage I) 3 years earlier and radiation therapy (60Co 350rad x 25 days) and chemotherapy (5FU, 200 mg/day, orally) for uterine cancer (stage IIb) 2.5 years earlier. These 3 cancers had quite different histological features one from the other. Laboratory investigation revealed leukocytosis (216, 100/microliters) with 90% blasts. The bone marrow was hyperplastic with 87% blasts. Although 90% of blasts had myeloperoxidase activity, surface marker analysis of the blasts revealed T-cell (CD2, CD3) phenotypes suggesting that the disease was so-called hybrid acute leukemia. She died of intracranial hemorrhage on the 2nd hospital day. An autopsy showed generalized leukemic infiltration and residual uterine cancer without metastasis to other sites. In our country, quadruplicate malignancies including hematological malignancy have been rarely described. The possible pathogenesis of multiple cancers has been discussed.
