Long-term adjuvant immunochemotherapy carried out on the gastrectomized patients with stomach cancer was reported. The protocol comprises the administration of large-dose of Mitomycin-C (20+10) mg just after gastrectomy and the long-term administration of PSK, FT-207 or (PSK+FT-207). Almost no side effects were observed. According to the studies on the immunological parameters, the increased reactions in PPD skin test and lymphocyte blastogenesis induced by PHA and PWM were observed remarkably in group (P+F) 3 or 6 months after gastrectomy. As for the survival rate, group (P+F) showed the most preferable results at one year in stage IV, at two years in stage III and at three years in all the stages, respectively after gastrectomy.

A combination of MFC (Mitomycin-C, 5-FU and Cytosine arabinoside) was administered to 32 gastric cancer patients and study was made of the therapeutic efficacy in relation to the histological pattern. In evaluation based on the degree of cancer cell infiltration (INF), chemotherapy was more effective in cases with INF alpha and beta than those with INF gamma. As for the grade of follicular hyperplasia in regional lymph nodes of gastric cancer, the higher was the grade, the more effective was the chemotherapy. However, a significant relationship could not be observed between histological pattern of stromal reaction of the gastric cancer and clinical effects of chemotherapy. The grade of lymphocyte infiltration or connective tissue reaction in the stroma was not related with the therapeutic effects. No relationship was also obtained between the grade of sinus histiocytosis of lymphoid reaction in the lymph nodes and therapeutic effects.

A postoperative long term chemotherapy was carried out against stage IV gastric cancer with Mitomycin-C, Futraful and a plant polysaccharide, PS-K. The 2-year survival rate was 15 per cent in patients who received an intraoperative Mitomycin-C alone, while it increased to 34 per cent in those receiving the anticancer agents for a prolonged period. The postoperative long term chemotherapy is assumed to be an effective means of improving the therapeutical results of gastric cancer.

We have evaluated the effect of 5-fluorouracil 600 mg/m2, doxorubicin ('Adriamycin') 40 mg/m2, and Mitomycin-C 4 mg/m2 (FAM) in two groups of patients with adenocarcinoma of the oesophagus, as either a preoperative or primary treatment. Response was assessed by barium swallow, CT scan, and measurement of metastases where present. Toxicity was as reported for FAM in gastric cancer. In the operated group 8 of 22 patients (36%) showed a partial response following two courses of FAM. Resection was completed in 20 patients, with six hospital deaths (30%). Of the 14 patients who were discharged from hospital, 8 have died (median 8 months) and 6 are alive at 12 to 27 months, with known recurrence in 1. In the non-operated group 6 of 17 patients (35%) showed a response, one complete, following one to six (mean 4.2) courses of FAM. Fifteen patients have died (median 5 months), and 2 are alive and free from disease at 12 and 17 months. Neoadjuvant therapy with FAM in adenocarcinoma of the oesophagus offers no advantage over surgery alone, although with inoperable disease FAM may be of use in palliation.

Feasibility of utilizing human gastric cancers as first transplant generation xenografts in nude mice for determining tumor sensitivity to chemotherapeutic agents was demonstrated by applying subrenal capsule (SRC) assay. A total of 55 human gastric tumors from patients were tested in this assay. Mitomycin-C (MMC) and hexycarbamyl-5-FU (HCFU, 5-FU derivative) were selected for the treatment of these patients after surgery and also for this assay as first transplant. Evaluable rate of MMC in this assay was 92.7% and that of HCFU was 90.9%. Sensitivity of tumors to MMC was 25% and to HCFU was 32%. Correlation between response to chemotherapy of human tumors in patients and in nude mice was 78.6%. These results indicate that this assay could predict effective drugs for patients with gastric cancer.

The effectiveness of Bestatin combined with Mitomycin-C, FT-207 for treating gastric cancer was investigated in a prospective randomized controlled study. All patients had undergone gastrectomy during the period from November 1980 to July 1983. Three-year survival rates revealed no difference between groups given or not given Bestatin. The effectiveness of Bestatin was confirmed by postoperative changes revealed by liver function test, especially GOT and GPT. No side effect due to Bestatin was detected.

A 61 year old male with a high serum alpha-fetoprotein level (2750 ng/ml) is reported. He had undergone curative surgery for early gastric cancer in June 1979, and palliative resection of a primary rectal lesion in December 1981. The postoperative alpha-fetoprotein level decreased to 1300 ng/ml but gradually increased despite the intraarterial infusion of 5-FU (9.25g) and Mitomycin-C (30 mg). He died of cachexia in February 1982. Histological examination revealed moderately differentiated adenocarcinoma; alpha-fetoprotein positive substances were found in the cytoplasm of rectal cancer cells by the enzyme labeled antibody method(peroxidase-antiperoxidase immune complex method). This rectal cancer patient had multiple primary cancers and gastric cancer.

Postoperative long-term cancer chemotherapy (PLCC) with a combination of Mitomycin-C (MMC), FT-207 and PSK (an immuno-stimulant) was prescribed for gastric cancer patients subjected to curative resection. The 5 year survival rates for patients with stage III and stage IV cancer were 58.3 per cent and 50.0 per cent in the PLCC groups, 48.0 per cent and 15.4 per cent in MMC groups, and 46.3 per cent and 13.3 per cent in no chemotherapy groups, respectively. In stage IV, the survival rate in PLCC group was significantly higher than that in MMC or no chemotherapy group (p less than 0.05). In the PLCC group, there was a tendency toward a dose-dependent effect in each group, and 5 year survival rate of stage III group administered over 60 mg of MMC, 60 g of FT-207 and 270 g of PSK was 70 per cent, such being remarkably higher than 46.3 per cent in those given no chemotherapy (p less than 0.07). There was no drug related death and only a slight leukopenia and hepatotoxicity occurred in some patients.

In order to investigate the hormone dependency of gastric cancer, the presence of estrogen receptor (E.R.) in surgically resected carcinomatous tissues was studied. E.R. assay was performed by dextran coated charcoal method. We have found 10% E.R. positive cases in gastric cancer (4/40). These 4 patients were all female, and showed Borrmann 3 or 4 type macroscopically. The histological types of these cases were signet-ring cell carcinoma or poorly differentiated adenocarcinoma. Furthermore, experimental studies suggest that the growth of signet-ring cell carcinoma transplanted on nude mice depends on sex hormone. Based on these observations, the clinical trial of chemo-endocrine therapy after gastrectomy for female patients with diffuse carcinoma of the stomach has been performed in our hospital since 1980. The therapy consists of Mitomycin-C plus subsequent Tegafur, with or without Tamoxifen 20 mg/day given orally, twice a day starting 2 weeks after surgery. The results are as follows: The cumulative 3-year survival rate in 21 cases receiving chemo-endocrine therapy (TAM+) after gastrectomy revealed higher (43.3%) than that (5.6%) in 23 cases receiving chemotherapy alone (TAM-). Furthermore, 2 and 3 years survival rates of TAM + in curatively resected cases (8 cases) were both 100% including 2 recurrent cases. In TAM- 10 cases, 2 and 3 years survival rate showed 68.4% and 16.3% respectively with statistically significance (p less than 0.01). Chemo-endocrine therapy for non-curatively resected and recurrent cases were also effective. This result suggests that the chemo-endocrine therapy after gastrectomy may be a new hopeful adjuvant in female patients with diffuse carcinoma of the stomach.

The effectiveness of Levamisole combined with Mitomycin-C, FT-207 and PS-K for treating gastric cancer was investigated in a prospective randomized controlled study. All patients had undergone gastrectomy during the years from August 1977 to November 1980. Five year survival rates revealed no difference between groups given or not given Levamisole. In those with positive lymph node metastases plus obvious serosal invasion, there was a significant elevation in the survival rate at four years after gastrectomy. The effectiveness of Levamisole was confirmed by postoperative changes in the PPD skin test, lymphocyte blastogenesis to PHA and serum inhibition index.
