A phase II study of a new fluoropyrimidine antitumor agent, carmofur (HCFU) fine granules, was performed in 24 institutions. The eligibility of patients and the criteria of response evaluation were based on the "Japanese Criteria for Evaluation of Clinical Effects of Cancer Chemotherapy on Solid Tumors." Out of 119 patients entered in the study, 65 patients were evaluable: 63 patients among them had measurable or evaluable lesions. Positive responses better than PR (Partial Response) were obtained in five (17.9%) out of 28 patients with gastric cancer, four (36.4%) out of 11 patients with colorectal cancer and five (45.5%) out of 11 patients with breast cancer. The total positive response rate was 22.2%. Optimal doses for the clinical use were ranged from 9 to 18mg/kg, and the median duration of PR was 19.6 weeks. In addition to the similar gastrointestinal toxicities observed in other fluoropyrimidines, hot sensation, pollakiuria and frequent defecation were seen in 12.4%, 10.1% and 3.4% of patients, respectively.

A 75-year-old man was admitted to our hospital because of severe dyspnea and pollakiuria in October 2001. We diagnosed his illness as carcinomatous pericarditis caused by the recurrence of primary lung cancer and prostatic hypertrophy. He had undergone surgery for gastric cancer in 1986, and for lung cancer in 1996. Furthermore, he had been treated for hepatocellular carcinoma in 1997. He finally died in February 2002 of respiratory failure due to the carcinomatous lymphangitis that followed the lung cancer. After autopsy, a microscopic view of the prostate revealed that he had prostatic carcinoma. Such a case is rare.

A 57-year-old woman visited a physician with complaints of anorexia and pollakiuria. Because a pelvic tumor and ascites were detected, she was referred to our department. Douglas pouch puncture revealed adenocarcinoma cells. Further examination showed an advanced gastric cancer with peritoneal dissemination. The cancer was judged to be unresectable. Chemotherapy with a combination of TS-1 and CDDP was performed before the operation. After 2 courses of the chemotherapy, her complaints disappeared, although abdominal CT confirmed remaining peritoneal dissemination. After 7 courses of chemotherapy, abdominal CT showed that the peritoneal dissemination had disappeared. Total gastrectomy and lymph node dissection were performed. Histological findings of the stomach revealed complete disappearance of cancer cells in the stomach and the regional lymph nodes. We confirmed that the TS-1/CDDP therapy resulted in a complete response to advanced gastric cancer and peritoneal dissemination. We recommend that chemotherapy be continued until the peritoneal dissemination disappears.
