The effects of chemotherapy for lung metastasis in 284 cancer patients using various anti-tumor drugs, including classic ones and modern active agents for the past 18 years, were presented. Lung metastasis for lung cancer was excluded. The response was achieved in cervical carcinoma of the uterus (17/62, 27%), endometrial carcinoma of the uterus (1/7, 14%), colorectal cancer (6/39, 15%), breast cancer (5/28, 18%) and
stomach cancer
(4/28, 14%). A high response was achieved in myosarcoma (5/12, 42%), testicular cancer (5/11, 45%) and also in ovarian cancer (3/10, 30%). Though there were few cases, a high response was achieved in malignant melanoma (2/3), choriocarcinoma (2/4) and esophageal cancer (1/3). In total patients the response rate was 20%. In these cases a complete response was achieved in 4 cervical cancers; one testicular cancer, ovarian cancer, esophageal cancer and
renal cancer
, respectively. However, the effect was temporary and no longterm survivor was observed except for one case of
renal cancer
treated continuously with interferon (3 X 10(6) units daily) and showing complete remission after 7 months of therapy. The effect of chemotherapy for lung metastasis was compared between nodular metastasis (NM) and lymphagiosis carcinomatosa (LC). In cervical carcinoma of the uterus, the response rate in NM (39%) was higher than in LC (11%). However, no difference was observed in breast cancer (NM 15%, LC 13%) nor in

We have utilized a recently developed human tumor cloning system to screen for antitumor effects in vitro of a new anthracene derivative, CL216,942. The object was to determine whether the system is useful for pinpointing the types of tumors in patients which should be studied in early phase II clinical trials. Tumors from 684 patients were placed in culture (27 different histologic tumor types). Two hundred seventy-three tumors both grew and formed enough colonies for drug sensitivity assays. In vitro antitumor activity was noted for CL216,942 against human breast cancer, ovarian cancer,
renal cancer
, squamous cell, small cell and large cell lung cancer, lymphoma, acute myelogenous leukemia, melanoma, adenocarcinoma of unknown origin, adrenal cancer,
gastric cancer
, pancreatic cancer, and head and neck cancer. The drug definitely showed no in vitro activity against colon cancer. These data indicate that CL216,942 has a wide spectrum of in vitro antitumor activity. A comparison of these in vitro results with the results of phase II clinical trials with the drug should allow an evaluation of the utility of the human cloning system for predicting clinical activity of a new compound.

Docetaxel has been evaluated in six tumour types in a total of 189 patients entered into phase II studies. Treatment consisted of a 1 h intravenous infusion of docetaxel 100 mg/m2 repeated every 3 weeks. No premedication was administered for possible hypersensitivity reactions. Docetaxel was found to be effective as first-line chemotherapy for head and neck cancer (response rate 44%)

(23%) and melanoma (14%) and as second-line chemotherapy for soft tissue sarcomas (21%; 95% confidence interval: 7.5%-43.7%). The results in colorectal and
renal cancer
were disappointing, with response rates of less than 10%. The most frequent adverse effects were alopecia (81%), grade III-IV leukocytopenia of short duration (66%) and skin reactions (52%). Hypersensitivity reactions were mild and occurred in 26% of patients. Docetaxel is an important new drug in the treatment of solid tumours.

New material was presented from pending publications arising from the follow-up to 1988 of the Quebec cohort of over 10,000 chrysotile miners and millers born 1891-1920. In reviewing these and previous findings, the following conclusions were drawn; they are supported, insofar as this is possible, by the only other relevant information, that from Balangero, in Northern Italy. There is strong evidence that the risk of lung cancer as a result of exposure to chrysotile at concentrations of less than 15 million particles per cubic foot is vanishingly small. At higher concentrations, the relative risk of lung cancer is elevated, but less so in smokers of 20 or more cigarettes a day than in others. The magnitude of this risk cannot be evaluated with any certainty, but this is unimportant as these higher concentrations (above about 50 f ml-1) are well outside the range of experience nowadays. There is no evidence that the risk of laryngeal cancer or of
stomach cancer
are adversely affected by exposure to chrysotile. Nor is there evidence of increased risks of other abdominal malignancies or of
kidney cancer
among chrysotile miners and millers. The risk of mesothelioma in chrysotile miners and millers is very low compared with the risks in populations exposed to amphiboles or to mixtures of fibres including even small proportions of amphiboles.

The addition of 5 years of follow-up and over 411,000 person-years of observation to a cohort of 34,081 men and women employed in U.S. furniture and other related industries allowed the investigation of mortality patterns among women and minority races in addition to white men. A significant excess of pleural mesotheliomas occurred among white men (standardized mortality ratio [SMR] = 3.7, 95% confidence interval [CI] = 1.2-8.7) but could not be linked to a particular type of furniture manufacturing. SMRs for myeloid leukemia and chronic nephritis were elevated among white men employed in the wood furniture industry but were not statistically significant. Males in the black/other race categories in wood furniture plants showed nonsignificant mortality excesses for infectious diseases and cancers of the prostate and colon and rectum. Among white women employed in wood furniture plants, mortality was elevated for cancers of the pancreas and lung during the most recent follow-up period. In metal furniture plants, mortality was raised among men in both race groups for

Stomach cancer
mortality was significantly elevated (SMR = 3.3, 95% CI = 1.3-6.8) among white men in metal furniture plants and was of the same magnitude over both the previous and the most recent follow-up periods. Among those working with textiles, SMRs were significantly elevated for leukemia (SMR = 6.1, 95% CI = 1.2-7.8) and cancers of the colon and rectum (SMR = 3.2, 95% CI = 1.3-4.5) for white women. Lung cancer mortality was increased for white men and women in textile operations, but SMRs were not statistically significant. SMRs for a number of other causes of death that were elevated at the end of the earlier follow-up period were not increased during the new follow-up period.

The antitumor efficacy of IL-2 is limited to
renal cancer
and melanoma. Several cytokines have been associated with IL-2 in an attempt to improve its activity, without, however, any clear benefit. Recent experimental and clinical studies have suggested the possibility to manipulate the host biological response by immunomodulating neurohormones, such as the pineal hormone melatonin (MLT). On the bases of these considerations, we have designed a neuroimmunotherapeutic protocol with low-dose IL-2 subcutaneous therapy (3 million IU/day for 6 days/week for 4 weeks) plus MLT (40 mg/day orally, starting 7 days before IL-2) in advanced solid neoplasms other than
renal cancer
and melanoma, which are generally resistant to IL-2 alone. The study included 82 patients, 72 of whom showed distant organ metastases. Tumor histotypes were, as follows: non-small cell lung cancer: 19; hepatocarcinoma: 16; colon cancer: 15;

Objective tumor regression were achieved in 17/82 (21%) patients, consisting of CR in 4 (liver: 2; pancreas: 1; stomach: 1) and PR in 13 (lung: 4; liver: 4; stomach: 2; pancreas: 1; breast: 1; colon: 1). The median duration of response was 8+ months. A stabilization of disease was obtained in 30 patients, while the other 35 patients progressed. The lack of progression was associated with a significantly higher increase in lymphocyte and eosinophil mean number and with a significantly lower increase in neopterin mean levels. has been observed in some mining populations but the role of radon is unclear. A few correlation studies and a case-control study have indicated that exposure to indoor radon could be of some importance as a cause of other tumours, especially acute myeloid leukaemia, melanoma and

. Also prostate cancer and some other cancer types have correlated with estimated radon exposure but the relatively few studies are not quite consistent with each other. Cancer incidence in the Dalgety Bay area of Fife, Scotland, was examined following the detection of radium-226 particles by routine radiation monitoring. The study was confounded by rapid population growth, demographic change and the relatively high socioeconomic status of the Dalgety Bay population. Health Board Primary Care Division records were used to calculate population estimates and Carstairs deprivation score was used to adjust for socioeconomic characteristics. In the period 1975-90, 211 residents were registered as having cancer compared with 214.21 expected from Scottish national rates. Of specific cancers possibly associated with radiation, the incidence of stomach, liver, lung, bone, prostate, bladder and
kidney cancer
and lymphoma were lower than expected while colon, rectum, pancreas, skin, breast and thyroid cancer and multiple myeloma and leukaemia were higher. There were three cases of childhood leukaemia compared with 1.22 expected. The only statistically significant differences observed were for pancreas (11 cases, O/E 2.28), lung (25 cases, O/E 0.65) and non-melanoma skin (36 cases, O/E 1.50).
Stomach cancer
was of borderline statistical significance (four cases, O/E 0.40). Adjustments for socioeconomic factors accounted for the apparently low incidence of stomach and lung cancer and, to a lesser extent, skin cancer, which remained of borderline statistical significance. Results in relation to pancreas cancer were unchanged. The observations of raised incidence of pancreas and skin cancer arose in the context of a survey of 17 cancer sites, from which the finding of two or more statistically significant results is not unusual (P = 0.21), and the numbers of cases involved were small. The epidemiological evidence for an association between radiation exposure and pancreas cancer risk is weak. Stronger evidence exists for an association with skin cancer. In the present study the anatomical distribution of the 36 cases was similar to that found elsewhere in Scotland.

In order to evaluate the results on successful adjuvant chemotherapy in resected
gastric cancer
we performed a randomised trial on 134 patients in two arms: a control one with no further treatment after surgery versus a treatment arm given mitomycin-C (MMC), 20 mg/m2 intravenously one day every 6 weeks for four courses, starting before the sixth week after surgery. The median follow-up was 105 months. In the control arm, 49 out of 66 patients died due to recurrence, versus 40 out of 68 patients in treatment arm. Actuarial survival curve was statistically significant (P < 0.025) in favour of the treatment group. Liver metastases were lower in adjuvant group than in the control group (8/68 versus 19/66). Toxicity was mild. Main toxic effects were thrombocytopenia, leukopenia, nausea and vomiting. A pelvis
