Carcino-embryonic antigen (CEA) was determined in 206 patients with
gastrointestinal cancer
(131 colonic and 75 stomach) at the 2nd Department of Surgery of the University of Vienna. The value of CEA in predicting tumour recurrence and/or metastatic spread has been assessed within the framework of regular follow-up control examinations after surgery. A postoperative rise in CEA titre corresponded with cancer recurrence or metastasis both in patients with stomach (91%), as well as colonic cancer (86%), whilst low postoperative CEA values correlated with a negative clinical report only in patients with carcinome of the colon to any degree of accuracy (91%). The correspondence was much lower in patients with
stomach cancer
(67%).

We evaluated "BALL ELSA CYFRA21-1" kit, an immunoradiometric assay kit for cytokeratin 19. Monoclonal antibodies KS19-1 and BM19-21 are used for immunoadsorbent and indicator, respectively. There was no problems in reproducibility, dilution test and recovery test. Minimum detectable concentration was 0.42 ng/ml. The antigen measured by this kit was immunologically cross-reactive with tissue polypeptide antigen (TPA) and CYFRA21-1 concentration was closely correlated with TPA concentration in patient's serum. One of twenty-six healthy subjects showed serum concentration over a cut-off value of 2.0 ng/ml. Serum CYFRA21-1 concentration elevated in 5 of 10 esophageal cancer patients, 5 of 10

The new proliferation marker, tissue polypeptide-specific antigen (TPS), representing the specific epitope M3 of tissue polypeptide antigen, and three conventional biochemical markers, CEA, CA 19-9 and CA-195, were analysed in 69 patients with advanced gastrointestinal tumours. The aim of our study was to assess the clinical relevance of these markers and to determine whether their use in monitoring the course of the disease can reduce the need for serial imaging procedures. At baseline, pathologically elevated TPS levels occurred in 90% of patients. CEA was elevated in 73%, CA 19-9 in 59% and CA-195 in 68%. With a detection rate of > 90% in both advanced colorectal (n = 37) and pancreatic cancer (n = 20), and of 75% in
gastric cancer
(n = 12), TPS was the most sensitive marker in all three tumour types included in this analysis. Serial evaluations of TPS and other biochemical markers were available in 39 patients undergoing palliative systemic chemotherapy. Treatment with a fluorouracil-based regimen resulted in a partial response in 5/27 patients with colorectal cancer, whereas 2/12 patients with pancreatic cancer responded to therapy with a high-dose epirubicin combination regimen. All other patients had disease stabilisation or suffered from progressive disease. When compared with the results of serial CT scanning, the TPS correlated best with the course of the disease, the positive predictive value being 75% for a partial response, 96% for stable disease and partial response combined and 100% for progressive disease. The corresponding values for CEA were 50%, 81% and 62% and were similar to those for CA 19-9 and CA-195. In summary, TPS seems to represent a sensitive, clinically relevant and specific marker of proliferative activity in
gastrointestinal cancer
. According to our preliminary results in colorectal and pancreatic cancer, TPS may be considered as the primary means of monitoring treatment, and imaging reduced to confirm the response.

A new operative procedure, called subtotal peritonectomy (SP), in combination with chemohyperthermic peritoneal perfusion, was developed for the treatment of peritonitis carcinomatosa in

. SP includes resection of primary lesion, colon, small bowel, spleen, and gall bladder and parietal peritonectomy. Six patients with
gastric cancer
and two patients with colon cancer underwent these procedures. A great deal of discharge from the peritoneal cavity, an increase in systemic vascular resistance index, and a decrease in central venous pressure represented much decrease in circulatory volume on days 1 to 2 postoperatively. This state improved at 3 to 4 days after operation. Histopathological study revealed multiple peritoneal seedings with negative surgical margins in all patients. There were no related deaths though bleeding, perforation, and abscess occurred in two patients each. One patient died of peritoneal recurrence after one year, but the other have survived.

No new sound evidence pointing to the effectiveness of screening for any
gastrointestinal cancer
has become available in the past year. Although recommended in Japan, screening for
gastric cancer
is expensive, and in a study in Venezuela it was found to be ineffective. The sensitivity of the standard hemoccult test for fecal occult blood is low; variants have improved sensitivity but relatively poor specificity remains. A major review of the evidence on colorectal cancer screening concluded that it was not recommended as part of the periodic health examination. Colorectal cancer mortality is falling in North America, possibly due to prevention (dietary modification) rather than screening. As dietary modification has benefits on other cancers and diseases, it is preferable as a colorectal cancer control strategy. Screening for other gastrointestinal cancers cannot be recommended.

The purpose of this study was to investigate the peculiarities of hormonal regulation of adenylate cyclase (AC) of blood lymphocytes in colorectal cancer patients and to compare these peculiarities with hormone sensitivity of AC of colorectal tumors and normal colonic mucosa. Basal and stimulated lymphocyte AC activity was studied in 51 healthy persons and 52 cancer patients (14 with colon cancer, 21 with rectal cancer and 17 with

) aged 20-75 years. In 31 of 35 patients with colorectal cancer the AC activity was studied simultaneously in lymphocytes, tumor tissue and normal colonic mucosa. To evaluate basal and stimulated AC activity the measurement of c-AMP (Amersham kits) formed in the presence of ATP regenerating system was used. patients was lower than in lymphocytes of healthy subjects of similar age. Stage dependence of the parameters under study was not found. There was a tendency for higher basal and stimulated lymphocyte AC activity in colon cancer patients as compared to stomach and rectal cancer patients. In colorectal cancer patients the peculiarities of lymphocyte AC reactions to stimulation were closer to those in tumor tissue but not to those in normal colonic mucosa. The reaction of lymphocyte AC to VIP and glucagon coincided more frequently with tumor AC reactions to the same hormones in case of hormone nonsensitive tumors. Thus, basal and stimulated lymphocyte AC activity in colorectal cancer patients was modified to some degree by tumor factors. Lymphocyte AC reactions to VIP and glucagon may be considered as indirect markers of hormone sensitivity of colonic tumors. Moreover, the probability of discovery of hormone nonsensitive tumors by this way is more reliable than hormone sensitive ones.

more than six months after operation for the breast cancer. This paper presents these six cases. The patients ranged from 56 to 78 years of age at the time of breast cancer surgery, and the interval after surgery until diagnosis of the second cancer ranged from 7 months to 5 years 1 month. The second cancer was
gastric cancer
in 3, esophageal cancer in 2, and hepatic cancer in 1. All of the 6 patients had received postoperative adjuvant chemotherapy for breast cancer. The most frequent histological type of breast cancer was solid-tubular carcinoma (3 patients). Three patients died, due to the second cancer, 4 days, 2 months, and 6 months, respectively, after diagnosis of the second cancer, and the other patients are alive 2, 3, and 4 years after diagnosis. The Japanese literature regarding multiple cancer among breast cancer patients is reviewed. It is concluded that care should be taken to examine breast cancer patients with gastrointestinal symptoms, which are likely to be dismissed as a side effect of postoperative chemotherapy.

This difference reached statistical significance at the 5% level. Additionally, a significant positive correlation (rs = 0.8, p < 0.05) was observed between ornithine and urinary albumin loss in this group of patients.
