There were 14 patients with a mean age of 65 years (ranged from 47 to 77) who had severe coronary artery disease associated with cancer of other organs. Stomach cancer was in 8 patients, breast cancer in 2, lung cancer in 2, rectum cancer in 1, and esophagus cancer in 1. They were treated with sequence of coronary artery revascularization and surgery for cancer; 9 patients in a two-staged fashion and 5 recent patients in a simultaneous one. In the series of simultaneous procedure, A-C bypass was combined with left ventricular aneurysmectomy in one patient and closure of ASD in another. There was no cardiac complication in both series, but one death from the complication of surgery for cancer in the staged series. Three patients in the staged series could not help being postponed the surgery for cancer by slow recovery from A-C bypass. In conclusion, coronary artery revascularization should anticipate surgery for cancer and simultaneous combined procedures should be chosen when possible.

The mortality experience of 1392 lead-zinc-silver miners (Gorno, Northern Italy) employed in the period 1/1/1950-31/12/1980 and followed-up to 31/12/1986 was examined. Two separate estimates of the radon exposure level are available: 0.60 and 0.36 working levels respectively. The silica exposure level was not assessed. Vital status was ascertained for 95.6% of the cohort members and their mortality was compared with expected deaths based on national rates. Significant excess mortality from esophageal cancer, stomach cancer, lung cancer, respiratory tuberculosis, respiratory diseases and deaths from external causes was found among underground miners. Surface workers show significantly increased mortality from liver and bile ducts cancer, hepatic cirrhosis, respiratory tuberculosis and respiratory diseases. Based on the 16.4 excess lung cancer cases among underground miners and their cumulative radon exposure, an attributable risk estimate ranging from 9.78 and 16.31 cases per million person-years and WLM (Working Level Month) was calculated.

Twenty-four patients with advanced gastric cancer and 4 patients with advanced esophageal cancer were treated with cisplatin at a dose of 80-100 mg/m2 for one day or 10-20 mg/m2 for 5 consecutive days every 3-4 weeks. As for gastric cancer, 21 of 24 were evaluable for this study according to the criteria of the Japan Society for Cancer Therapy. Four of 21 patients (19%) showed partial response (PR), 7 displayed no change (NC), and 10 evidenced progressive disease (PD). Among 4 PR cases, only one had effective primary lesion. As for esophageal cancer, all 4 patients were evaluable, while 2 showed no change (NC) and another 2 exhibited progressive disease (PD). Gastrointestinal toxicity occurred in 20 patients despite the use of anti-emetic drugs. Nephrotoxicity and hematological toxicity were observed in 14 patients and 22 patients, respectively. These did not impede the continuous treatment except one case of hematological toxicity. It was concluded that cisplatin is more effective for the metastatic lesion of gastric cancer than primary lesion.

A total number of 113 patients with gastroesophageal cancer (47 esophageal cancer, 66 gastric cancer) were examined by endoscopic ultrasonography (EUS). The depth of tumor infiltration in the wall, involvement of lymph nodes and spread to adjacent organs were the features particularly stressed on. 61 patients (46 with gastric cancer, 15 with esophageal cancer) underwent radical resection thus enabling a comparative study with the histological findings. The accuracy rate of EUS in determining the depth of infiltration was 85% in esophageal cancers and 77% in gastric cancers. Regarding involvement of lymph nodes, EUS was accurate in 80% of esophageal cancers and in 87% of gastric cancers, whereas CT scan was accurate in 47% and 43% respectively. For detecting spread to adjacent organs EUS was also more accurate than CT scan.

A review of data on 548 patients with primary squamous cell carcinoma revealed that 24 (4.4%) had had gastrectomy. Although the interval of the gastrectomy due to peptic ulcer or gastric cancer and esophageal cancer was 13.4 +/- 7.9 and 5.8 +/- 4.2 years, respectively, this difference was thought to be due only to the occurrence of each disease. The incidence of the occurrence of the lower esophageal cancer after gastrectomy was 29.2%, not significantly higher than the 22.4% incidence of lower esophageal cancer in overall cases. Histopathological investigation of the 13 resected esophageal cancer tissues from gastrectomized patients revealed a mild esophagitis in some cases, with no significant histologic characteristics. Thus esophageal squamous cell carcinoma and previous gastrectomy may be incidentally related.

A case of triple cancer involving a tubular adenocarcinoma of the stomach, a squamous cell carcinoma of the esophagus, and hepatocellular carcinoma of the live is reported. These three cancers had been diagnosed while the patient was alive and later were confirmed by autopsy. The esophageal cancer and hepatocellular carcinoma were found almost simultaneously, 6 years after surgery for the gastric cancer. Although many cases of triple cancer have been reported, a triple cancer of this combination is very rare (0.3% of all cases of triple cancer), our case being the fifth such case in Japan.

The effectiveness of polychemotherapy (platidiam + chemoradiation treatment) for extensive cancer of the esophagus, stomach and lung was studied in 71 patients. Response rate in esophageal cancer patients after platidiam, methotrexate and prospidin was 31%, which increased to 65% after irradiation. In stomach cancer patients treated with platidiam, 5-fluorouracil and vincristine, antitumor effect was registered in 30% and a 50% regression of tumor following radiotherapy--in 37%. In small cell lung cancer group, chemotherapy (platidiam + cyclophosphamide + vincristine) was effective in 37.5% and in 83% when combined with radiotherapy.

An enzyme-linked immunosorbent assay for human immunoreactive gamma-glutamyl transpeptidase (gamma-GTP) was developed. This assay was found to be simple, reproducible, and sensitive to 10 ng of the enzyme. Serum immunoreactive gamma-GTP content was significantly elevated in patients with various malignant tumors including liver cell cancer, lung cancer, gastric cancer, esophageal cancer, and colorectal cancer. On the other hand, in sera of patients with nonneoplastic diseases, the immunoreactive gamma-GTP content was not significantly elevated. No correlation was found between the serum levels of gamma-GTP determined by enzymatic assay and enzyme-linked immunosorbent assay, which indicates that, due to the presence of endogenous inhibitors and/or activators in sera, the enzyme activity may not reflect the true amount of enzyme protein. The measurement of immunoreactive gamma-GTP protein in sera appears to be useful for the detection and monitoring of certain malignant tumors.

The number of cancer deaths and age-adjusted cancer death rates up to 2000 in Japan were predicted based on cancer death rates in 1972-1986. A simple linear regression model (y = a + bt, where t is calendar year and y is cancer death rate per 100,000 population) was fitted to the sex-age specific cancer mortality rates from 1972 to 1986 and cancer death rates in 1990, 1995 and 2000 were predicted by extrapolation method. The number of future cancer deaths was estimated after taking into account future population in Japan. The age-adjusted cancer rates up to 2000 were also estimated. The present study revealed that the numbers of deaths from stomach cancer (both sexes), uterine cancer and esophageal cancer (females) would keep declining, while all other cancers would increase in the future. The total number of cancer deaths in 2000 was estimated to be about 310,000 which is 1.62 times that of the total cancer deaths in 1986 (191,654). It was estimated that in 2000, lung cancer would rank top accounting for 22.2% of all cancer deaths, followed by cancers of the large intestine, liver, stomach, pancreas, biliary tract, leukemic, lymphoma and breast.

To elucidate the role of the spleen on immunosuppression of gastric and esophageal cancer, suppressor cell activities of spleen cells (SCs), splenic vein lymphocytes (SVLs) and peripheral blood lymphocytes (PBLs) were investigated. Concanavalin-A induced suppressor cell (Con-AS) activity of SCs was significantly higher in patients with gastric cancer than in those with benign diseases. Higher Con-AS activity of SCs was observed in esophageal cancer patients with tumors located in the lower portion of the esophagus. In comparison with suppressor activities of SCs and SVLs, the decrease of the predominance of suppressor precursors in SCs and the increase of the spontaneously activated suppressor cells in SVLs were noted with the advance of the tumors. Culture supernatants from splenic adherent cells significantly induced suppressor cell activities as well as did sera from splenic venous blood. From these results, it is concluded that the generation of suppressor precursors in the spleen is dependent on the location of tumors and that the maturation of suppressor cells occurs in the spleen by factors released from splenic adherent cells, then migrates into the peripheral blood.
