A 31-month-old female mongrel dog was orally administered with 50 mg or 100 mg of N-nitrosobutylurea (NBU) in gelatin-capsule 3 times per week for 19 months with interposing periods of complete suspension. Thirty-four foci of signet ring cell carcinoma were found in the antral region of the stomach. The majority of the foci (31 foci) were early cancer, and the remaining foci were invasive cancer. In addition to these lesions, there was "a single gland cancer" in which a row of cancer cells was confined to a single gland. The whole gland was composed of two cell layers; the inner layer facing the lumen was normal gastric cells and the outer layer was atypical or neoplastic cells underlaid by the basement membrane. Mitosis was frequently observed on the bottom of the gland. Atypical or neoplastic cells seemed to mature gradually through a process of upward migration with increase in cytoplasmic Alcian blue-PAS and HID-AB positive mucin. Some of the cells rich in mucin moved into the lamina propria. The other cells remained in the flow of the regular cell renewal system of the normal gastric cells and reached the top of the gland. This observation revealed a mode of incipient gastric cancer growth, which starts and spreads within a single gland, before it invades the surrounding lamina propria.

Studies of the ultrastructure of stomach cancer were carried out: adenomacarcinoma (4 observations), signet ring cell carcinoma (1 observation), nondifferentiated cancer (1 observation). In 5 out of 6 observations cells of each tumour were homogeneous in principle. Signs of differentiation were revealed in them in a various degree of manifestation in any one direction: according to the type of additional or surface cells in 3 adenocarcinomas; according to that of goblet cells in signet ring cell carcinoma. In the case of nondifferentiated cancer signs of differentiation were absent. In one observation the cells of adenocarcinoma had ultrastructural features of various cells: mucoid, chief, parietal, endocrine, enterocytic, goblet cells. In a part of cancer cells there were noted combined ultrastructural signs of various cells of the gastric mucosa.

We have investigated the cell kinetic effect of four carcinostatic agents (MMC, CDDP, ADR and 5-FU) on the human gastric cancer cell line (KATO-III; signet ring cell carcinoma) by means of flow cytometry (FCM), using bromodeoxyuridine (BrdU) and its monoclonal antibody. Cancer cells in the S phase were first labelled with BrdU and then the bivariate DNA/BrdU distribution was examined to analyze the effect on the cell cycle. Furthermore, cells were reincubated at 24 hours after labelling to evaluate the cell turnover during FCM. MMC, CDDP and ADR assembled the cells into late S phase and G2M phase, while 5-FU assembled them into S phase. after 24 hours, cells with cessation of cell cycle had inhibited their proliferation. We conclude that this technique can be usefully applied as a susceptibility test of carcinostatic agents, since it could define the phase where carcinostatic agents acted on cancer cells.

A newly developed scanning acoustic microscope (SAM) system operating in the frequency range of 100-200 MHz has been employed to measure the attenuation and the sound speed of formalin-fixed specimens of five different types of gastric cancer. Signet-ring cell carcinoma specimens exhibit attenuation constant and sound speed values significantly lower than other types of gastric cancer tissues. Tubular adenocarcinoma specimens exhibit a trend toward higher attenuation and sound speed values as the cell type became differentiated. Our measurements and observations suggest that the ultrasonic properties are influenced by cellular arrangement, intercellular junction and intracellular chemical components.

The patterns of recurrence and the prognosis of 24 gastric cancer patients with peritoneal seeding and/or serosal invasion, who underwent gastrectomy followed by intraperitoneal hyperthermic perfusion (IPHP), were studied in comparison with 23 gastric cancer patients given surgery alone (controls). With respect to 22 patients with peritoneal seeding, all of 8 patients in the control group died of re-accumulation of ascitic effusion, whereas of 14 patients in the IPHP group, 3 died of peritoneal recurrence, 2 of intraperitoneal tumors, 1 from pleural metastasis, and 1 of liver metastasis. The remaining 7 cases are alive without a sign of recurrence. Again, with respect to 25 patients with large serosal penetration of cancer, 7 of 15 patients in the controls died of peritoneal recurrence, whereas this was never observed in the IPHP group. As to the relation of histology and prognosis, while the poorly differentiated adenocarcinoma and signet ring cell carcinoma had poor prognosis in the controls, the same patterns in the IPHP group proved not to be so unfavorable. Since a few given IPHP resulted in hepatic metastasis and/or nodal involvement, adjuvant treatment will be required. The survival rate for the IPHP group was significantly better at p = 3.43 x 10(-2) than in controls.

A signet ring cell carcinoma in the gastric antrum of a Cynomolgus monkey induced by N-ethyl-N'-nitro-N-nitrosoguanidine was sequentially studied by endoscopy, biopsy, and autopsy. The carcinoma was first detected on the angulus of the stomach at the 38th month as a slightly elevated lesion. Sixty-one months later this tumor was found to be still in the "early" (intramucosal) stage. Another, independent, initial gastric cancer was also discovered. This is the first example of an induced gastric carcinoma remaining in the "early" stage during a six-year follow-up period after the initial histologic diagnosis.

In March, 1988, a 65 year-old male underwent a total gastrectomy for a gastric cancer (Borrmann 3: a poorly differentiated adenocarcinoma; IIc: signet ring cell carcinoma). In October that year, however, an anastomotic recurrence of this tumor was found. Thus, from November, 1988 to January, 1989, to treat this recurrence, the patient received a course of radiation therapy frontally, towards the upper abdomen, with a total dose of 46.5 Gy (linac X-ray). His symptoms improved, and a CT, an upper gastrointestinal barium study, and an endoscopic examination indicated that the tumor had disappeared. No further recurrence was found until January, 1990, when the patient died of a metastasis that had spread throughout his body.

Three hundred and one gastric cancer patients have been examined preoperatively to investigate their gastric acid secretions after stimulation by tetragastrin, and serum gastrin stimulation by a test meal, as well as for skin reactions and an evaluation of their serum glycoproteins. The results have indicated that their gastric secretions and serum gastrin response were found to be reduced, according to the advancement of their cancer, and that the gastric acid secretion of patients with signet ring cell carcinoma was higher than that of patients with other histological carcinomas. Gastric acid secretions of patients with an ulcerated type of cancer, that is, type IIc and type III in an early cancer stage and type IIc of an advanced Borrmann V type, was higher than in patients with other types, and there were significant correlationships between gastric secretions and PHA skin test and gastric secretions and the IAP and the sialic acid.

For the purpose of studying the histological findings and prognosis of stomach cancer in atomic bomb survivors, a histological diagnosis of 600 cases of stomach cancer observed between 1964 and 1986 in three medical institutions in Hiroshima City was made by a single pathologist and, furthermore, analysis of 231 cases thereof was performed using the DS86 dose estimated by the Radiation Effects Research Foundation. 1) By histological type of stomach cancer, the frequency of poorly differentiated adenocarcinoma (poorly differentiated adenocarcinoma and signet ring cell carcinoma) was significantly higher in the 1 rad or more group (average stomach organ dose of 30.6 rad) than in the 0 rad group. 2) As for stromal type, the frequency of scirrhous type was significantly higher in the 1 rad or more group, but no difference in infiltrative behavior could be demonstrated. 3) Early cancer tended to be observed at a low rate in the 1 rad or more group. 4) As for average survival time, of the early cancers only well differentiated adenocarcinoma showed a significantly lower survival time in the 1 rad or more group when compared to the 0 rad group.

A case of extensive bone marrow necrosis due to cancer metastasis is reported. A 55-year-old female, who had a history of subtotal gastrectomy for signet ring cell carcinoma of the stomach 7 years ago, was admitted to our hospital with a complaint of lumbago on October 25, 1987. Red blood cell count was 92 X 10(4)/microliters, hemoglobin 2.7 g/dl, hematocrit 8.0%, platelet 6.4 X 10(4)/microliters, and white blood cell count 13,400/microliters with leukoerythroblastosis. Bone marrow aspiration of the sternum, left iliac crest, and bilateral posterior superior iliac supine showed extensive bone marrow necrosis. Serum ALP was increased to 7410IU/l, dominated isozyme of bone type. Hemostatic findings suggested a complication of consumption coagulopathy. Skull, vertebrae, iliac and pelvic bone X-ray showed multiple osteolytic lesions, and irregular isotope uptake was recognized on the bone scintigraphy using 99mTc. Sixth bone marrow examination at the right iliac crest revealed signet ring cell carcinoma metastasis. In spite of detailed examinations, there was no evidence of primary carcinoma, including the remnant of stomach. We speculated that the signet ring cells were originated from the respected gastric cancer. The patient has received anti-cancer chemotherapy with UFT and OK432, and is still alive 9 months after diagnosis.
