Factors influencing diagnostic yield by directed endoscopic biopsy and cytology were studied in 50 patients with advanced
gastric cancer
using a forward-viewing panendoscope. The diagnostic yield was higher for exophytic lesions than for infiltrative tumor, and directed brush cytology alone was more productive than directed biopsy alone. The lesions that provided non-diagnostic tissue were mostly recurrent or infiltrative cancers, and were most often in the cardia, antrum, or were of the
linitis plastica
type. The combination of infiltrative character and location in antrum or cardia, especially if recurrent, often resulted in non-diagnostic biopsy and cytology specimens.

We report that the penetrating type of early
gastric cancer
(PEN) is a specific type of early
gastric cancer
and that the poorly differentiated PEN type could be considered an initial lesion of
linitis plastica
-type cancer. We performed an immunohistochemical study to clarify the role of growth factors (epidermal growth factor [EGF] and transforming growth factor-beta [TGF-beta]) in the PEN type of early
gastric cancer
. The results indicated that the PEN type of early
gastric cancer
has a high growth capacity. Moreover, it was suggested that EGF was involved in its specific infiltrative growth and that both EGF and TGF-beta were involved in its specific scirrhous growth. From these findings, it was assumed that the immunohistochemical staining of EGF and TGF-beta in endoscopic biopsy specimens was useful for the diagnosis of the PEN type of
gastric cancer
and also for the diagnosis of the initial lesion of
linitis plastica

Early gastric cancer in the region of the fundic gland was found to occur predominantly in women. Many lesions of this type were identified in the posterior wall of the middle portion of the stomach. Histologically, the lesions were classified as poorly differentiated carcinomas in all cases of early cancer. In 9 patients with cancer of
linitis plastica
type having IIc lesions located in the region of the fundic gland, the tumor focus was also in posterior wall of the middle portion of the stomach. Giant folds were present diffusely in all other portions of the gastric mucosa in these latter cases. These cases also had diffuse infiltration of cancer cells into the submucosa, muscle layer and extraserosal regions. Histologically, the lesions were classified in all cases of
linitis plastica
type as poorly differentiated carcinomas including signet ring cells. Acid mucopolysaccharides (AMPS) and sialic acid were histochemically detected in the interstitial tissues of early gastric carcinomas and
linitis plastica
type gastric cancers. The AMPS digestion rates were higher for early cancer tissue. The present results support the hypothesis that early
gastric cancer
in the region of the fundic gland may occur in conditions favoring tumor growth and may develop into cancer of
linitis plastica
type.

Subtotal gastric resection usually provides the best palliation in advanced
gastric cancer
; however, if total gastrectomy (TG) is required there is doubt about its benefit. The authors reviewed 53 consecutive patients undergoing TG for advanced gastric adenocarcinoma between 1980 and 1989. Indications for TG were tumor location in 30% and extent of tumor in 70%, including nine patients (17%) with
linitis plastica
. Four patients (8%) died postoperatively, and six patients required reoperation for postoperative complications. The median postoperative hospital stay was 13 days. The median survival was 19 months, and 13 patients (24%) lived for more than 2 years. The quality of life was graded in survivors as good in 59%, satisfactory in 28%, and poor in 13% of patients. It was concluded that TG is a worthwhile procedure for selected patients, even in the presence of advanced disease, providing prolongation of good quality of life with low morbidity and mortality.

A 72-year-old woman with signet-ring cell carcinoma of the urinary bladder treated with total cystectomy is described. The bladder yielded
linitis plastica
pattern of infiltration similar to that seen in the
gastric cancer
, i.e., cancer tissue extended almost whole bladder deeply to the serosa, whilst the mucosal surface was only minimally invaded. She received no adjuvant therapy and she is alive without recurrence 8 months after the operation. We review the reported cases and shortly discuss the prognosis and treatment of primary signet-ring cell carcinoma of the urinary bladder.

Despite new diagnostic techniques,
gastric cancer
in its less aggressive stages is not diagnosed more frequently and the prognosis still remains poor. Thus, a possibility exists that there is still a delay in diagnosis. This study was therefore designed to investigate whether a delay in diagnosis of
gastric cancer
exists, to identify factors leading to diagnostic delay, and to settle whether the prognosis is dependent on a delay. Fifty consecutive patients with gastric carcinoma were interviewed and the diagnostic attempts prior to diagnosis were checked. It was found that thirteen patients (26%) had patients' delay (more than three months of symptoms before consulting a doctor) whereas twelve (24%) had doctors' delay (more than three months of investigations before correct diagnosis was made). Doctors' delay was more common in female (7/13) than in male patients (5/37) (p less than 0.05) and in
linitis plastica
(5/8) than in other types of gastric cancers (7/42), (p less than 0.05). Seven of the twelve patients with doctors' delay had been on regular check ups before diagnosis. A negative barium meal was the most common reason for doctors' delay. Survival was influenced only by tumour stage (r = -0.71, p less than 0.05). Our conclusion is that both patients' and doctors' delay are considerable in the diagnosis of
gastric cancer
; it is therefore suggested that improvement in five-year survival requires improved presymptomatic diagnostic methods and screening programmes.

Thirty-two patients with gastric polyps and a mean follow-up period of 18 months were analysed. The principal presentation was epigastric pain accompanied at times by haematemesis or melaena. As in other series, the hyperplastic polyps (21 cases) predominated, followed by the adenomatous type (six patients). Endoscopy was an effective first-line investigative method for 21 patients and for a further five who had initial false negative barium studies. Gastric polypectomy was performed for 60% of patients without any complications. There was discrepancy between the biopsy material and polypectomy specimen in five of seven patients who were recalled for snaring. Although no polyp in this series turned malignant, one diffuse polyposis patient developed
linitis plastica
after 4.5 years of follow-up. Hence, for reasons of recurrence, new polypoid growths and development of
gastric cancer
, long-term surveillance is indicated.

Two hundred eleven gastric adenocarcinomas diagnosed from 1967 to 1982 were analyzed. Thirty-four percent had a proximal location, a proportionate increase from previous decades that suggested a distinctive epidemiology. Diffuse histology occurred in 49% of cases overall and in 55% of unresectable cases, which were also increases from previous decades. No deaths followed curative resections, two (4%) of 50 patients with palliative resections died, and three (6%) of 54 patients who underwent exploration without resection died, indicating improved operative management. Superficial
gastric cancer
constituted 6% of cases; 91% were cured. Seventeen percent of cases were
linitis plastica
and required total gastrectomy in 77% of resections; only 13% of patients had curative operations; none were cured. Seventy-nine percent of cases were polypoid or ulcerated focal cancers. Of operable focal cancers, 72% were resected; 27 (47%) of 57 patients who underwent resection for cure survived five years, a distinct improvement from previous reports, as was the overall survival of 21%.
