We evaluated the clinical usefulness of intraperitoneal cancer chemotherapy using an implantable reservoir in patients with peritonitis carcinomatosa as compared with conventional chemotherapy. The cases were comprised of 9 gastric cancers, 6 colorectal cancers, 1 appendiceal carcinoma, and 1 malignant tumor of retroperitoneum, all of which were observed to have peritoneal dissemination during surgical operations. Medications infused were CDDP in 14 cases, MMC in 4 cases, and 5-FU in 1 case. The patients with P2 and P3 findings (group A) were compared with other cases treated with conventional chemotherapy (group B). The average survival periods were, for
gastric cancer
patients, 6 months in group A and 8.6 months in group B; for the colorectal cancer group, 10.5 months in group A and 13.6 months in group B. The 6-month and 1-year cumulative survival rates for the
gastric cancer
patients were 100% and 0%, respectively in group A; 52% and 17%, respectively in group B. For the colo-
rectal cancer
group, the 6-month, 1-year, and 2-year cumulative survival rates were 75%, 75%, and 0%, respectively, in group A; 79%, 45%, and 14% respectively, in group B. These results did not show significant differences between the two groups. On the other hand, the average outpatient clinic visiting rates in the
gastric cancer
patients in groups A and B showed a significant difference; 72.6% (group A) and 46.8% (group B). Also, the rates in the colo-
rectal cancer
patients showed a significant difference; 69.6% (group A) and 61.5% (group B). As a result, the extension of the period of stay at home and visiting of our outpatient clinic for treatment was obtained in the group A. Consequently, this intraperitoneal cancer chemotherapy using an implantable reservoir was extremely effective in patients with peritonitis carcinomatosa for outpatient treatment over the long term without loss of quality of life for each patient.

To evaluate trends in cancer incidence adjusted for completeness of registration in a population-based cancer registry, we analyzed the data of incidence and mortality in the Aichi Cancer Registry from 1979 to 1986 and recalculated age-adjusted incidence rates (AAIRs) at the lowest completeness of registration through the study period by excluding reported cases from the registration randomly. The results were as follows: 1) The AAIRs for male

in both sexes, which did not decrease or rather increased using the original incidence figures, showed a clear decrease after adjustment. 2) The AAIRs for female stomach and uterine cancers, which also showed decreases in the original incidence, showed more marked decreases. 3) The marked increases observed for the AAIRs for colon and breast cancers in the original incidence were attenuated by about 50%. 4) The AAIRs for liver and lung cancers were also affected somewhat by the adjustment for completeness of registration. 5) Trends in the AAIRs after adjusting for completeness of registration were virtually the same as those for age-adjusted mortality rates.

The purpose of this study was to investigate the relationship between life style factors and adult diseases among three ethnic groups, Chinese living in Japan, Koreans living in Japan and Japanese. The mortalities of major cancers and other adult diseases of Chinese and Koreans in Japan were compared with those of Japanese by calculating Standardized Mortality Ratios (SMR) of the two groups using death rates in the Japanese population as the standard. Life style data on smoking, drinking and dietary habits of the three groups were collected by self-administered questionnaire surveys, and age-adjusted proportions were calculated with the truncated world population as the standard. In September 1985, chemotherapy with 3 courses of CDDP (50 mg x 1) plus VDS (3 mg x 3) and liniac irradiation (60 Gy in total) were performed for pulmonary large cell carcinoma. Restiform shadows and fibrosed foci were left behind, but thereafter only the former was persisted without anticancer therapy. Now in December 1989, he has still been surviving without any sign of local recurrence of remote metastasis. Neither anticancer therapy was tried for the first and second cancers nor a cancer family history was found about him.

Adjuvant treatment is based on the concept that surgery is only potentially curative and that apparently localized disease has extended beyond surgical resection or is already disseminated. Although death might be related to local recurrence as well as to disease dissemination, most of the trials have tested only one adjuvant modality. Among many negative and non-contributory studies, very few positive results were obtained: in

it seems that pre-operative and perhaps postoperative radiotherapy may reduce the incidence of local recurrences, and in colon cancer patients treated with Methyl-CCNU, vincristine and 5-fluorouracil (5-FU) had a significant increase in survival. In colon cancer, the lack of active drug might at least partly explain negative studies, but in
gastric cancer
the most active combination in advanced disease has failed to demonstrate an improvement of survival in the adjuvant setting. Future trials should take account of this succession of negative trials.

Multi-institutional studies on clinical hyperthermia of deep-seated tumours were undertaken using 8 MHz radiofrequency capacitive heating devices (Thermotron RF-8) at seven institutions. Each institute was designated to treat specific organs. This paper contains the accumulations of the results obtained at different institutions charged for different tumours. Deep-seated tumours in the lung, stomach, pancreas, liver, urinary bladder and rectum were treated. A total of 177 cases examined from January 1985 to December 1988 included 96 cases (54%) treated with radiotherapy plus hyperthermia, among which 14 cases were pre-operative. Of 177 cases, 81 (46%) were treated with chemotherapy plus hyperthermia. Complete response (CR) and partial response (PR) were obtained in 80% of the cases with lung cancer, 39% with

, 56% with liver cancer, 35% with pancreas cancer, 71% with urinary bladder cancer, 100% with primary

, and 47% with recurrent rectal cancer. Thermometry was performed using two techniques; one is direct measurement of intratumour temperature in lung and liver cancers, the other is indirect measurement of intracavitary temperature for stomach, pancreas, urinary bladder and rectal cancers. Intratumour temperatures were measured in 30 of the 43 tumours of the lung and liver. The maximum tumour temperature was greater than 42 degrees C in 23 (77%) of the 30 tumours. Intracavitary temperatures were measured in 133 (99%) of the 134 tumours of stomach, pancreas, urinary bladder and rectum. An intracavitary temperature greater than 42 degrees C was obtained in 98 (73.7%) of the 133 tumours. The contribution of hyperthermia in improving the quality of life of patients under terminal care was also investigated. It was indicated that hyperthermia was one of the most effective treatment techniques for advanced or inoperable cases. In this study local control rate (LCR) was mainly discussed because the period of follow-up was only 3 years. Side-effects were observed in 37 cases (21%); main side-effects were fatty induration, pain during treatment and burn. However, no side-effects were severe enough to interrupt therapy.

Cancer is a disease predominantly seen in the older age group. The most frequent forms are in males: lung, prostatic, stomach, colonic and bladder cancer. In females: breast, colonic,

, lymphoma, leukaemia and

. In view of the expected demographic figures a dramatic increase in the incidence of cancer is expected. The malignancies seen in the elderly respond generally poor to chemotherapy. Most cytotoxic drugs are excreted by the kidneys. Especially the renal clearance of anticancer drugs will therefore be compromised in the elderly, this should be considered when giving cytostatics. Mucositis, bone marrow toxicity, pulmonary and neurotoxicity are quite often enhanced in the older patient group. The indications for chemotherapy are limited. Chemotherapy should not be withheld from patients with advanced breast cancer and certain haematological malignancies. Further clinical research focussed on the elderly is warranted. Drugs with a mild spectrum of side effects deserve priority. Hormonal treatment is an important modality in breast, prostatic and endometrial carcinoma. The burden for the patients is limited and the advantages are well documented.

In vitro thermosensitivity of various human tumors including 90 esophageal, 10 gastric and 40 colo-rectal cancers were evaluated using the succinate dehydrogenase inhibition (SDI) test. Tumor fragments minced with scissors were incubated at 43 degrees C as heat treated cells and at 37 degrees C as controls for 20 hrs, and assayed for the succinate dehydrogenase (SD) activity using 3-(4, 5-dimethyl-2-thiazolyl)-2, 5-diphenyl-2H tetrazolium bromide (MTT) as a hydrogen acceptor. The thermosensitivity was estimated by the percentage of SD activity of heat treated cells compared to that of each control. A variation in the thermosensitivity was noted between patients. The SD activity was 60.1 +/- 20.3% (mean +/- standard deviation) for esophageal cancers, 34.9 +/- 21.7% for gastric cancers, 50.3 +/- 20.6% for colo-rectal cancers. Significant differences were noted between esophageal cancers and gastric cancers, colo-rectal cancers (p less than 0.01 and p less than 0.05, respectively). When the thermosensitivity was arbitrarily defined as reduction in the SD activity to 50% of control or less, the positive rates were 31.1% for esophageal cancer, 70% for
gastric cancer
and 62.5% for colo-

University hospitals perform total gastrectomy to a greater extent than other hospital and have greater faith in radical operations for pancreatic cancer. Otherwise there are no conspicuous differences of opinion between the different categories of hospitals.
