The use of technetium-99m in the noninvasive investigation of the upper gastrointestinal tract is discussed with particular reference to the evolution of a method of assessing gastric function or gastric acid secretion non-invasively and to the applications of this method in the investigation of surgical patients with disease of the upper gastrointestinal tract. We report a case of adenocarcinoma in
Barrett's esophagus
following a total resection of the gastric remnant. A 52-year-old man had undergone a distal gastrectomy for
gastric cancer
at 33 years of age and a total resection of the gastric remnant for local recurrence of the
gastric cancer
at 35 years of age. Repeated endoscopic examinations revealed the sequence of reflux esophagitis and
Barrett's esophagus
. Furthermore, adenocarcinoma in
Barrett's esophagus
was detected in December, 1989. A subtotal esophagectomy was performed in January, 1990. The elevated lesion in the lower esophagus showed coarse lobulation and measured 7.4 x 3.2 cm. The histologic type was that of well-differentiated adenocarcinoma, with the invasion limited to the muscularis mucosae without lymph node involvement. Severe dysplasia was seen adjacent to the definite carcinoma. The case supports the acquired theory of pathogenesis for
Barrett's esophagus
and suggests that reflux esophagitis after total gastrectomy may result in a dysplasia-carcinoma sequence.

's metaplasia (esophagocardia group). They compared results of that investigation with the cigarette and alcohol use of 303 patients with squamous carcinoma of the esophagus, a cancer in which an etiologic association is proven, and of 338 patients with
stomach cancer
not involving the cardia, a cancer in which there is little or no association with cigarettes or alcohol. Controlled for other variables, patients with squamous cancer used more cigarettes and alcohol than the other two groups. There was no significant difference in cigarette or alcohol use between patients with esophagocardia and
stomach cancer
or between those in the esophagocardia group with or without
Barrett
's metaplasia. Because cigarette and alcohol use was not greater in patients with esophagocardia than in those with

The accumulating data show that endoscopic ultrasonography (EUS) is highly compatible with the UICC/AJCC staging classification for esophageal and
gastric cancer
, based on the TNM system expressing anatomical extent of disease. The great strength of EUS in staging these cancers is its ability to image the gut wall and adjacent structures in unique detail. EUS is more accurate than computed tomography in staging the depth of primary tumor invasion (T) and regional lymph node metastases (N). High frequency EUS is not useful in staging for distant metastases (M) due to limited depth of the field. EUS also has limitations in reliably distinguishing between neoplastic and inflammatory tissue. Thus, the major use of EUS is in staging rather than in diagnosis. Most countries with adequate statistical infrastructure have registered declines in
gastric cancer
mortality and incidence rates. Such a trend is dominated by the most frequent variant, namely the so-called intestinal type of adenocarcinoma, usually ulcerated and occupying predominantly the antrum and the antrum-corpus junction. This variant is considered the endstage of a prolonged precancerous process with gradual progression from (a) chronic active gastritis to (b) multifocal atrophic gastritis to (c) intestinal metaplasia, first resembling the phenotype of the small intestine and later that of the colon, to (d) dysplasia and (e) finally to invasive carcinoma. Major trends in dietary habits, namely lower intake of salt and increased and more frequent consumption of fresh fruits and vegetables, have been linked to the decline. In parallel with those trends, improved sanitation and more adequate housing may be responsible for the declining rates of infection with Helicobacter pylori, the major cause of chronic active gastritis. A decline in the frequency of papillary adenocarcinoma of the oxyntic mucosa, associated with the pernicious anaemia syndrome, appears to have taken place much earlier. Although the frequency of the pernicious anaemia syndrome seems to have remained at similar levels, its complications in terms of papillary adenocarcinoma have decreased in populations of northern European extraction. This may be related to time trends in dietary habits. The secular decline in diffuse carcinoma has been either of much less magnitude or non-existent. Few clues are available on this tumour variant. It is somewhat predominant in women, in subjects of blood group A phenotype, and less frequent in older subjects. Cell lines derived from diffuse carcinomas lack functional calcium dependent adhesion molecules ("cadherins"). Recent increases in incidence rates have been registered for adenocarcinoma of the gastric cardia. This increase parallels that of lower oesophageal adenocarcinoma, frequently linked with

, reflux oesophagitis, a history of duodenal ulcer and gastric hypersecretion. New developments in molecular biology are being used to study the process of gastric carcinogenesis. A selection of landmark articles for a given year in any subject risks being somewhat subjective, and subjectivity is best avoided in scientific endeavor. However, the very nature of such a selection process invites judgment. Like most judges, I, too, claim to avoid conscious bias, but no one who has ever graced the bench can claim that at the subconscious level personal bias has never crept into a decision. Similarly, deep down in the vault of my subconscious, I love a maverick. That perhaps explains why so many articles that challenge long-held beliefs have especially found favor. Among them are those that question the strength of the association of Helicobacter pylori with

, the usefulness of surveillance endoscopy in patients with
Barrett's esophagus
, a randomized trial that casts doubt on the preeminence of laparoscopic cholecystectomy, and a metaanalysis that concludes that corticosteroids may not be nearly as good for alcoholic hepatitis as we were once told. I have tried to resist the temptation to be too laudatory of technologic advancement, unless the benefit to the patient of such technology has been defined clearly. Thus, of all of the new technologies (endoscopic retrograde choledochopancreatography is no longer a new technology), only endoscopic ultrasonography finds a place. Articles that assess preventive strategies and are in the realm of epidemiology have received mention. All in all, 1996 was not a spectacular year for major therapeutic advances. In contrast, some notable advances have been made in the laboratory, and perhaps the most important has to do with the role of nitric oxide both in the regulation of normal function and in the genesis of disease.

A delta 5-aminolevulinic acid (ALA) bioadhesive gel has been developed and evaluated in an in-vivo mouse model for photodynamic treatment of
gastric cancer

Barrett's oesophagus
. Four gels were tested: noveon AA-1, keltrol T, lutrol and blanose. An initial in-vitro study of gel adhesion showed that noveon and keltrol had longer polyethylene transit times than lutrol and blanose. In-vivo assays indicated that protoporphyrin IX was synthesized by gastric mucosa when ALA-noveon and ALA-lutrol were used (preferable results for noveon). Keltrol was eliminated from the study after these investigations. Only ALA-noveon gel was retained for studies of the relationship between ALA dose and fluorescence. Fluorescence measurements in-vivo showed that ALA concentration and application time had an influence on protoporphyrin IX synthesis. Maximum intensity (2091 counts s-1) was found with 2 mg mL-1 ALA, and fluorescence intensities differed with application time, reaching 1805 counts s-1 after 240 min. ALA-noveon, showing good adhesion and enabling efficient diffusion of ALA at a pH < 6, was considered the best formulation for maintaining ALA stability.

, and adenocarcinoma of the distal esophagus. Recent studies suggest this phenomenon may be related to the simultaneous fall in the prevalence of Helicobacter pylori (H. pylori) colonization, especially by the virulent cagA + strains. H. pylori infection with the cagA+ strain is potentially protective against the spectrum of gastroesophageal reflux disease because it lowers intragastric acidity as the result of a pangastritis, frequently with multifocal gastric atrophy and possibly increased intragastric ammonia production. Assuming that some types of H. pylori strains are protective, our entire approach to the worldwide elimination of this organism, sometimes indiscriminately, will need critical reevaluation.
