Esophageal and gastric cancers are highly virulent tumors with an especially poor prognosis. They are rather common tumors in the United States with an anticipated annual incidence of approximately 32,000 new patients in 1991.
Adenocarcinomas
of the proximal stomach and lower esophagus are rapidly increasing in incidence; the reasons for this remain unclear. Endoscopic ultrasonography has offered a new dimension to staging especially of the primary tumor but also shows promise for more accurate identification of nodal metastasis. While stage remains the single most important prognostic variable, biological studies investigating tumor markers are a high priority; aneuploidy and HER-2/neu amplification or overexpression may predict poor outcome in
gastric cancer
. Esophageal and gastric cancers have a high local and distant failure rate when treated with conventional therapy. New developments in chemotherapy in the neoadjuvant and postoperative setting are under intense investigation in an attempt to improve prognosis for these diseases.

A case of 56 year-old man with Ki-1 (CD30) lymphoma is reported. He noticed cervical lymph node swelling and was admitted with temporary diagnosis of gastric
adenocarcinoma
in February 1986. His physical examination showed several from 1 to 4 cm size enlarged cervical and axillary lymph nodes. His first lymph node biopsy demonstrated the histological picture of malignant lymphoma. After 20 days his second lymph node biopsy demonstrated the picture of necrosis. Second gastric biopsy showed the picture of neither
gastric cancer
nor malignant lymphoma, in spite of his first gastric biopsy finding with
adenocarcinoma
that turned out to be malignant lymphoma by later reinvestigation. After word those lymph nodes disappeared and have not been palpable for about ten months. In October 1987, elevated LDH value, lymph node swelling and gastric lesion were again observed and sixth gastric biopsy demonstrated the picture of malignant lymphoma. He was treated with anti-lymphoma drugs. After his partial remission, he died of gastrointestinal bleeding in April 1988. Specimens of both first lymph node biopsy and sixth gastric biopsy were examined with cell markers for infiltrated cells and were positive for Ki-1/Ber-2H. His final diagnosis was Ki-1 lymphoma.

Autoinoculation is suggested as an adjuvant means to augment the effect of surgery in
gastric cancer
which may prevent the development of recurrences and metastases. Surgery alone is not a curative method and 5-year survival in gastric cancer stage III ranges from 9.2 to 28%. The data have been obtained on comparative survival in a control group and a group of inoculated patients. The greatest effect was achieved in patients with poorly differentiated
adenocarcinoma
. Further investigation of the two groups adjusted for the patients' age, kind of surgery and morphological structure of the tumor will help to work out precise criteria for inoculation.

Helicobacter pylori infection, thought to be causally related to chronic gastritis, may also be associated with an increased risk of
gastric cancer
. To determine whether an association with
gastric cancer
does exist, we retrospectively evaluated serum samples from 69 patients with histologically confirmed gastric
adenocarcinoma
(32 with cancer at the cardia and 37 with cancer at other sites) and from 218 patients with one of three categories of nongastric cancers, with other gastric cancers, or with benign gastric neoplasms. These samples were compared with samples from 252 cancer-free control subjects, a group comprising 76 asymptomatic volunteers and 176 persons with nonmalignant disorders. Serum samples collected from cancer patients prior to surgery and from cancer-free controls were tested for antibodies to H. pylori by using a highly sensitive and specific IgG enzyme-linked immunosorbent assay. The risk of H. pylori infection in the case patients relative to the control subjects was estimated with the use of multivariate logistic regression analysis to adjust for potential confounding variables. Antibodies to H. pylori were detected in 65% of the patients with noncardia
gastric cancer
but in only 38% of the patients with
gastric cancer
located at the cardia. A significant association was found between H. pylori infection and noncardia
gastric cancer
(odds ratio = 2.67; 99% confidence interval = 1.01-7.06). Within the subset of patients with noncardia
gastric cancer
, a statistically nonsignificant tendency existed for those with the intestinal versus the diffuse histologic type of noncardia
gastric cancer
to have a higher risk of H. pylori infection. Our results support the hypothesis of a relationship between H. pylori infection and the development of noncardia gastric
adenocarcinoma
.

Gastric cancer
, especially
adenocarcinoma
, is variable in incidence on the world. In this paper, there is a review of the epidemiology and the etiopathogenic factors of the disease: genetics, hereditary, immunologic and environmental and, also, of the so called precursor diseases: atrophic gastritis and intestinal metaplasia, gastric adenoma, gastrectomized patients, pernicious anemia and Menetrier's disease. There is an explanation about the changes of the gastric epithelium related both with the intestinal and diffuse type of
adenocarcinoma
; the anatomo-pathological notion of macroscopic advanced-Borrmann and early cancer-Japanese classification and the clinical and diagnostic procedures are included with the fundamentals of therapeutic management.

A 57 year old female had a tubular gastric
adenocarcinoma
confirmed by endoscopy and radiologic studies. Echotomography of the abdomen revealed multiple adenopathies. Immunoscintigraphy with 99mTc labeled anti-CEA BW 431/26 monoclonal antibody was performed. Images obtained at 6 and 24 h showed a significant uptake at the primary lesion and at periaortic adenopathies both in conventional gamma-camera and single emission photon tomography. This immuno-scintigraphic technique may be helpful in the management of patients with
gastric cancer
.

Anatomic and surgical characteristics of 233 pts with early, and 120 with intermediate
gastric cancer
, from 13 hospitals in Chile are analyzed. 63% had depressed or excavated lesions. Most of the tumors were located in the inferior or middle third, while only 17% of the early carcinomas were found in the superior third of the stomach, specially in the lesser curvature. 5% had residual tumor in the gastric remanent. Histologically, tubular
adenocarcinoma
was the most common finding. According to Lauren's classification intestinal carcinoma was more frequent than the diffuse type. Surgeons underestimated lymphatic node involvement (macroscopic appreciation) in up to 58% of the cases when compared to the pathologists observation (microscopic appreciation). This was specially true in muscular carcinomas. The incidence of lymphatic metastases was 6% in mucous, 12% in submucous and 57% in muscular carcinoma. The need of complete lymph node dissection of all the lymphatic barriers is emphasized.

Despite the importance of in vitro study of
gastric cancer
, there are very few established cell lines derived from human gastric carcinoma. We have recently established a new cell line derived from human
gastric cancer
which has the ability to produce tumor markers. This cell line has been designated JR-St. This cell line was derived from the cerebrospinal fluid of a 37-yr-old female patient who had metastatic brain tumor of signet ring cell gastric
adenocarcinoma
. This cell line has been maintained for more than 24 months through 80 passages with stable growth. PAS staining showed intracellular mucin granules. Transmission and scanning electron microscopy revealed cells with numerous microvilli and fine projections as well as intracellular granules, indicating mucin. This cell line had the ability to produce high concentrations of tumor markers such as carcinoembryonic antigen (CEA) and carbohydrate antigen (CA) 19-9. Thus Thus this cell line should provide a very useful tool for the investigation of
gastric cancer
such as analysis of tumor markers as well as effects of anti-cancer drugs or growth factors.

Gastric cancer
is the leading cause of death from cancer in China. Samples of fish sauce, a traditional seasoning, were collected in the high-risk area for
gastric cancer
in the Fuzhou area, Fujian Province. When fish sauce samples were nitrosated at pH 2.0, direct mutagenicity and high contents of N-nitrosamide were detected (30.9-78.0 microM); the N-nitrosamide content of three samples of fish sauce made in Guangdong and purchased from a market outside Fujian were low (2.1-6.0 microns). When the nitrosated fish sauce extract was given to newborn rats by gavage, dysplasia and
adenocarcinoma
were induced in the glandular stomach in the 4th and 16th experimental week, respectively. N-Nitrosamides were also found in fasting gastric juice from patients with chronic gastritis in the high-risk area of Putian. The mean concentration of total N-nitrosamides in the extracts correlated with the severity of gastritis in the stomach. These findings indicate that N-nitrosamides may play an important role in causing
gastric cancer
in China.

A novel mouse monoclonal anti-fucosylceramide antibody was established by using neutral glycolipids from a human pancreas cancer tissue as the immunogen. Mice were immunized with the neutral glycolipids in the form of liposome-containing lipid A. Mouse monoclonal antibodies were screened with enzyme-linked immunosorbent assay and by thin layer chromatography-immunostaining. The latter technique showed that a mouse monoclonal antibody, designated PC47H, specifically reacts with a ceramide-monoglycoside fraction of the neutral glycolipids. The effects of various monosaccharides on the reactivity of PC47H with the neutral glycolipids were tested, and it was found that only fucose was able to inhibit the binding of PC47H to the neutral glycolipids. We also examined the direct binding activity of PC47H against galactosylceramide, glucosylceramide, fucosylceramide, and ceramide. This showed that the antigen specificity of PC47H was exclusively directed against fucosylceramide. In thin layer chromatography-immunostaining experiments with neutral glycolipids prepared from various human tissues, we observed that fucosylceramide was highly expressed in human colon and
gastric cancer
tissues. PC47H recognized the human
adenocarcinoma
cell lines of colon, stomach, pancreas, and lung but did not react with other tumor cells or with several nontumorous human cells.
