The neurological complications of malaria--an almost universal problem although it occurs mainly in the tropics--constitute the commonest cause of death and morbidity. These complications usually occur in P. falciparum infections and in those who are non-immune, such as children, pregnant women and visitors to areas endemic for malaria. They are rare in children suffering from protein-calorie malnutrition. Febrile convulsions, the commonest childhood emergency in the African and cerebral malaria are the commonest neurological complications. Spinal cord disorders and polyneuropathy have been described but are rare. It has been suggested that various forms of mental illness and psychoses may be related to malarial infections, but much research is needed to establish cause and effect relationship. The neuropathology and pathogenesis, clinical presentations, treatment and mortality of the neurological complications are reviewed.

Various types of neuropsychiatric manifestations are described in P. falciparum malaria of which peripheral neuropathy has been described mainly from India. We are reporting such a case who presented with seven days history of fever and weakness of two days duration. On investigations it turned out to be acute inflammatory demyelinating polyneuropathy (AIDP) with peripheral blood showing heavy parasitaemia of P. falciparum. All other causes of acute polyneuropathy were ruled out by history and relevant examination. Patient improved with quinine and other supportive therapy.

We report two cases of people from different states in India who developed polyneuropathy following intake of mefloquine for treatment of malaria. To the best of our knowledge this has not been reported before.

We describe a case of a woman who developed a peripheral polyneuropathy shortly after completing 4 weekly doses of mefloquine hydrochloride (250 mg) malaria prophylaxis. Although mefloquine-related central nervous system neuropathy is well described in the literature, peripheral polyneuropathy similar to this case has been documented only once before, to our knowledge.

There are four neurological complications that can occur after malaria treatment at a time when the patient is aparasitaemic: delayed cerebellar ataxia, acute inflammatory demyelinating polyneuropathy, post-malaria neurological syndrome and acute disseminated encephalomyelitis (ADEM). The authors describe a case of a 54-year-old male who presented with encephalopathy and generalized seizures forty-three days after complete recovery from acute malaria by Plasmodium falciparum. Diagnosis of post-malaria ADEM was made based on the acute onset of the neurological symptoms, characteristic findings in magnetic resonance imaging of the brain and prompt response to steroid therapy. ADEM is an autoimmune demyelinating disease of the central nervous system that usually arises after an infection or vaccination. Its occurrence after malaria infection is relatively rare, and to the best of our knowledge there are only thirteen cases described in the literature.
