An enzyme-linked immunosorbent assay (ELISA) developed using Leishmania donovani promastigote soluble antigen gave positive responses in practically all the clinically and parasitologically diagnosed patients with kala-azar in a preliminary study. Healthy control subjects from non-endemic and endemic areas gave negative results. No cross-reaction was observed with patients having leprosy, filariasis,
malaria

tuberculosis
, or amoebiasis. Blood samples collected on filter paper were adequate for the test. The test appears promising in the diagnosis of kala-azar.

A retrospective survey was carried out on adult medical admissions to Kamuzu Central Hospital, Lilongwe, Malawi during the period January to December 1986, and results compared with those obtained in Queen Elizabeth Central Hospital, Blantyre in 1973. There were 4700 admissions which was more than twice the number seen in Blantyre. However, the age distribution, the pattern of disease and the overall hospital mortality were similar. Infections (

, gastroenteritis/dysentery and meningitis) were the most common cause of admission, and the major causes of death were still

Vanuatu, which formerly was known as the New Hebrides, is an archipelago in the southwest Pacific Ocean with a tropical humid climate. The national health system and the state of health of the population generally are satisfactory. The main diseases of public-health importance are

. Their epidemiology and control are discussed. An epidemic of Plasmodium falciparum malaria, which commenced in 1980, is described; this possibly can be explained by an increase in chloroquine-resistant P. falciparum over the same period. Other diseases also are reviewed. Many tropical diseases of great significance elsewhere are not of much significance in Vanuatu. Medical advice for visiting travellers is provided.

From the moment WHO was established in 1948, the control of venereal diseases was felt to deserve highest priority, together with activities to control
malaria
and
tuberculosis
. International action was needed in view of the high morbidity and mortality from venereal diseases, their serious human and social consequences, and the prevalence of congenital syphilis and other sexually transmitted diseases (gonorrhoea, chancroid, venereal lymphogranulomatosis, granuloma inguinale). WHO immediately set up a global programme for the control of STDs and, with the participation of other agencies, especially UNICEF, furnished countries with assistance in the form of personnel, equipment and funds for the operation of programmes to assess the extent and impact of STDs and to plan and implement practical measure of control. The 1950s witnessed a steady and considerable decline in syphilis and gonorrhoea and many health authorities relaxed their control activities and efforts to maintain public awareness of the problem. In contrast to the prevailing optimism, WHO repeatedly stressed the possibility of a renewed upsurge of STDs. In the 1960s and 1970s, there was a sharp rise in STDs, both in the "classic" diseases (the five venereal diseases mentioned above) and also in the "second generation" STDs (chlamydial infection, genital herpes, human papillomavirus and other infections). Through its programme for the control of STDs, WHO put forward suitably designed control strategies, essentially based on information and education for health, screening for STDs, diagnosis and treatment of cases, contact tracing, and the training of health personnel. By the end of the 1970s, the bacterial, but not the viral STDs, had been contained in the industrialized countries. In many of the developing countries, STDs remained a priority public health problem, above all on account of the seriousness of their sequelae. In 1981, a new sexually transmitted disease-the acquired immunodeficiency syndrome (AIDS)-was identified. As of 1982, the WHO Programme on STDs organized meetings to define the extent of the problem, compare experience, promote and coordinate research and propose strategies for prevention. In 1987, WHO established a Global Programme on AIDS. It is clear that the control of STDs is now more than ever a priority. We have strategies for the prevention and control of STDs and the WHO Programme will continue to collaborate closely with countries in strengthening their national control programmes.(ABSTRACT TRUNCATED AT 400 WORDS)

The registry of patients at the hospital of Kampene, Zaire, covering the period 1986-87 was examined to determine the hospital's rate of utilization and accessibility, to evaluate mortality, and to ascertain the prevalence of infectious diseases. The 1986 data of the hospital laboratory indicated a high incidence of infectious and parasitic diseases: ancylostomiasis (33.6%); ascariasis (22.9%); schistosomiasis (3.4%); multiple intestinal parasitic infections (10.9%);
malaria
(43%), often chloroquine-resistant; filariasis (70.8%); and alcohol-acid resistant
tuberculosis
bacilli (15%). Sexually-transmitted diseases such as vaginitis (80%) were caused by polygamy, prostitution, and promiscuity, HIV serodiagnosis could not be performed because of a lack of equipment. A high infant mortality rate was caused by neonatal tetanus, toxic gastroenteritis, measles (5.1% lethality: 2 died out of 39 cases), and epidemic cerebrospinal meningitis. Malnutrition caused kwashiorkor and avitaminosis. 792 births were registered at the maternity ward in 1986: 52.8% were male and 47.2% were female; 48 (6.1%) were stillborn or died in the following days; 104 (13.1%) were born prematurely; and 24 (3.1%) were twins. Cesarean section was performed in 43 cases (5.4%). There was a total of 15,099 outpatient visits during a 1-year period. The bed occupancy rate of the surgical ward ranged between .7 and .8 during 1987. Recovery and hospitalization days per doctor or health assistant were very high compared to Italian standards. was a high 1.8%, but malnutrition rated even higher: 21.4%. The utilization of the hospital was high, Maternal-child protection measures, especially in the area of nutrition, require the training of community health workers and traditional birth attendants; however, cost-benefit considerations limit resources and the implementation of primary health care is curtailed by economic and cultural factors.

In Germany, the last period of the Second World War and the following years were characterized by deficiencies of hygiene which had not occurred previously in Middle Europe during the 20th century. There were focuses of typhus, typhoid fever,
tuberculosis
, diphtheria, scarlet fever, and meningitis. Insufficiencies in the removal of faeces caused high incidences of shigellosis, hepatitis A, and ascariasis. As a result of insufficient body care, many people were infested with fleas, lice and scabies. The migration of large proportions of the population resulted in an increasing prevalence of syphilis an gonorrhea. As the population resettled, the first steps towards reorganization of public health could be done. The spread of typhoid fever was controlled by drinking-water disinfection with chlorine, repair of sewage systems, and patient isolation. The application of DDT helped to reduce scabies and pediculosis, resulting in decreasing typhus risks. During the first two decades after the war, there was a steady decrease of the incidence of infectious diseases. The reconstruction of the towns resulted in improved housing conditions and a decreasing number of persons per housing area, reducing the intensity of physical contacts of the inhabitants with each other. The nutrition and clothing situation of the population improved, which, in addition to a general rise of the standards of hygiene, brought about an increase of the individual resistance to infection. A further reduction of sporadic and epidemic outbreaks of infectious diseases was achieved by the introduction of chemotherapy and antibiotics. Increasing prosperity was accompanied by new problems of hygiene. Infectious diseases almost eradicated in West Germany, were imported by air travellers. Ten imported cases of smallpox were reported between 1957 and 1972, eight of which originated from Southeast Asia.

cases was reported. In West Germany there is, however, no more spread of the disease under present conditions, cholera caused similar problems. The 1961 cholera epidemic started in Southeast Asia and caused minor outbreaks in Mediterranean countries like Italy and Spain. A significant spread of the disease throughout Europe was prevented by generally high standards of drinking water and sewage treatment. The authors used 1987 data from the Epidemiologic Surveillance Project (ESP) of the Centers for Disease Control to examine the completeness of race-ethnicity reporting in the National Notifiable Diseases Surveillance System. And, to the extent possible, they used ESP to assess racial and ethnic disparities in the occurrence of selected notifiable infectious diseases. For the 30 reporting areas (29 States and the District of Columbia) that provided data to ESP for all of calendar year 1987, approximately 60 percent of case reports were accompanied by specified race-ethnicity for affected persons. This percentage varied widely by disease and State. In general, non-Hispanic whites had morbidity rates (cases per 100,000 population per year) that were among the lowest compared with rates for other groups, and Native Americans commonly had rates that were among the highest. The ranking of morbidity rates among blacks, Hispanics, and Asians and Pacific Islanders varied by disease, although the last group had strikingly higher rates for
malaria
and
tuberculosis
. The age distribution of persons with cases was often lower among minority groups than among non-Hispanic whites, but the authors were unable to calculate age-specific or age-adjusted rates. Potential biases that limit interpretation of the findings are reviewed. Efforts to eliminate racial-ethnic disparities in the occurrence of infectious diseases would be aided by effective surveillance data. For the ESP to meet its potential in this regard, however, substantial improvements in the reporting of race-ethnicity for notifiable diseases are needed.

Most of primary and secondary parasitic liver diseases, at present can be property treated with drugs. Venezuelan pharmaceutic market has some peculiarities that have determined the disappearance from the market of many drugs such as emetine, thiabendazole, quinacrine and niclosamide. Diloxanide never appeared. Venezuela has no commercial international treatises that protect international patents in the pharmaceutical area. In addition, government regulation of cost of drugs is very strict. This is particularly true with old drugs (such as emetine or quinacrine) which had such a low price that is non-commercial for the maker of the drug, usually a large transnational, and is withdrawn from the market. Flexibility of prices is quite easy for new antibiotics which are very expensive. Frequently small national companies import the drug from Italy and Japan which sell the drug independently from international treats. Such companies frequently produce the drug for the government social system, but are unreliable and also frequently they withdraw the drug a variable period of time. A conventional information system has been used by management to stimulate excellent primary health care (PHC) worker performance through a combination of peer review, motivation by ranking and reward, and taking corrective action in Maharashtra, India. Maharashtra has a large public health system which serves a population of around 62 million. The existing management information system (MIS) was sharpened. Reports were reviewed monthly. The reports were of 6 programs--family planning, maternal and child health, leprosy control,
tuberculosis
control,
malaria
control, and blindness control--for all areas, comparing performance to targets, and integrating program components into 1 single measure by weights. Criteria guided the choice of 39 indicators from existing reports. Targets were fixed for all indicators. Weights were assigned to each indicator to arrive at some appreciation of a program's performance trends. Information analysis focussed on poor performance. This resulted in a battery of questions, a "public health audit." It was characterized by the search for causes of unsatisfactory functioning in different areas and in various programs. The health workers involved have been able to develop more desirable approaches to each of the program components through discussion and analyses. The monthly peer review leads to improved skills and professionalism. The format leads to proposals of corrective action. This had led to a large number of administrative and policy measures that have increased the output of the health care delivery system. District ranking has been extended to lower levels, to the medical officers in charge of PHC workers and the multipurpose workers/ auxiliary nurse midwives of subcenters within the PHC. The MIS was very productive. The improvement in performance indices is given. The system helped junior medical officers, the district health officers, district medical officers and top management to attain desired results. Its most important aspect has been peer review and discussion and its training function.
