In 2000, 24 malaria cases were registered in Poland. All of them were imported, mainly from Africa (15 cases). Plasmodium falciparum infection was confirmed in 15 cases, P. vivax--in seven. Among 24 malaria cases 22 were men and 20 in 20-49 years old group. Eleven persons travelled abroad in the connection with their job, six--as tourists, three--came from malaria endemic or epidemic countries. In 2000, 25-year old man with malaria falciparum died.

Typhoid fever is an acute infectious disease caused by Salmonella enterica serotype typhi. The infection is responsible for endemic or epidemic outbreaks in tropical and subtropical countries, especially in Indian subcontinent, Southeast Asia, Central and South Africa. Latin America, and it occurs sporadically in Poland. We reported two cases of imported typhoid fever after travelling to India and Nepal. In the tourists returning from the area hyperendemic for malaria, visceral leishmaniosis, amoebiasis and haemorrhagic fevers and not following tropical hygiene measures, persistent fever was a source of difficulties in differential diagnosis. In the first case, lack of anti-malarial chemoprophylaxis in the presence of anaemia and thrombocytopenia strongly suggested Plasmodium spp. infection. Two daily peaks of fever with splenomegaly, lymphadenopathy, leucopenia, high transaminases levels and co-existing positive serology for L. donovani pointed to visceral leishmaniosis. Late occurrence of specific anti-S. typhi agglutinins in the Widal test, cross-reactivity with S. paratyphi A and negative urine bacteriological culture were observed. In the second case, gastrointestinal disturbances, including pain, abdominal tenderness and diarrhoea gave a suspicion of amoebic colitis. Stool and urine cultures were negative for S. typhi and cross reactions with S. paratyphi A and C were reported. Typhoid fever was finally confirmed in both patients by an isolation of S. typhi from peripheral blood cultures. The effectiveness of treatment of choice with ciprofloxacin or ceftriaxone in a case of multidrug-resistant (MDR) strain of S. typhi was documented.

In 2001, 27 malaria cases were registered in Poland. All of them were imported, mainly from Africa (18 cases). Plasmodium vivax infection was confirmed in eleven cases, P. falciparum--in ten, P. falciparum/P. vivax--in four, P. malariae--in one and Plasmodium sp.--in one. Among 27 malaria cases 17 were men and 16 in 20-49 years old group. Twelve persons travelled abroad as tourists, five--as missionaries, three--in the connection with their job. In 2001 malaria deaths were not notified.

Most of the cases (15) were caused by Plasmodium falciparum. 17 cases were men, 9 were women. Mean age was 40. The youngest patient was 11, the oldest 71. Malaria occurs in Poland sporadically. It is imported from endemic regions, exceptionally by children. Lack of knowledge of the clinical course may result in false diagnosis and fatal outcome of the disease. In 16 cases P. falciparum etiology was confirmed and in 4--P. vivax. Among the cases 14 were men and 6 were women. The youngest patient was 1 and the oldest 56, 11 of the patients were in the age group 21 to 40 years. Most commonly the reasons for travel were tourism or family visits (9 cases) and professional or missionary travel (5 cases). In 7 cases some kind of chemoprophylaxis was documented, including at least 4 for whom it was judged inappropriate. In other prophylaxis was not used (5 cases) or the information was missing. No malaria deaths were reported in Poland in 2005.

The evolution of some attempts to improve the global health situation is reviewed and realisation of the declaration of Millenium Developmental Goals is being discussed. Improved health care and education have been finally accepted as the mile stones of the global development. However, the obstacles to achieve better global health are as follows: global health services crisis, optimal health development strategy in various countries still discussed, insufficient population education and involvement in health matters. In Poland endemic malaria and Taenia solium infections have been eradicated. The incidence of geohelminthoses is substantially reduced. The major parasitological problems now are imported parasitoses (mainly malaria) and infections of zoonotic origin (trichinellosis, alveolar and cystic echinococcoses as well as toxoplasmosis and toxocarosis). Effective control of these parasitoses depends much on more professional teaching and training of health personnel in medical parasitology, better health education of population at large and more active surveilance and control interventions of sanitary-epidemiological services.

There were 19 cases of malaria meeting European Union case definition for confirmed case registered in Poland in 2006. All of them were imported, including 1 case of relapse: 17 from Africa, 1 from Asia and 1 from Oceania. Species of Plasmodium was determined for 12 cases (68%): P. falciparum in 12 cases and P. vivax in one. There were 15 cases in males and 4 in females. Age at onset ranged from 17 to 59 years and a considerable number of cases occurred in persons 50 years old or older (5.26%). Common reasons for travel to endemic countries included tourism or family visits (10 cases) and professional or missionary travel (5 cases). Only four cases used chemoprophylaxis and the relevant information was missing in 4 cases. In two cases of malaria caused by Pl. falciparum the clinical course was severe and one of them died.
