A case of plasmodium falciparum malaria in a 21-year old nurse, who never left Poland, was described. Malaria was confirmed by parasitological examination of peripheral blood smears in which 20% of erythrocytes infected with Plasmodium falciparum were found. The course of the disease was very severe but the patient survived. The only possibility of infection must have been through skin lesion on the nurse's hand during obtaining blood from the patient infected with Plasmodium falciparum.

In Poland, cases of imported malaria are noted at present. In Polish citizens who work abroad, malaria is considered as an occupational disease. 95 cases of malaria in Polish citizens treated at the Clinic of the Institute of Maritime and Tropical Medicine in 1984-1993 were subjected to retrospective analysis, considering their occupational, epidemiological and clinical aspects as well as permanent sequelae of the disease. The most numerous groups among the patients examined were seafarers, fishermen, priests and missionaries. They were infected mostly in West Africa, during the first year of work. The majority of patients neglected chemoprophylaxis of malaria. In about 1/3 of patients, the resulting incapacity for work exceeded 1 month and more. In 7% of patients, 6 months after the treatment, permanent sequelae of the disease were observed: damage to the central nervous system, liver, kidneys and heart. The social and material costs of this invasive occupational disease were high. Malaria is an important health problem in Poles who work in endemic areas. Malaria prevention should be intensified.

Thirty five patients with imported malaria were hospitalised in a period of 1980-93 in Department of Infectious Diseases of Pomeranian Medical School, Szczecin, Poland. The diagnosis of malaria was established on a base of clinical feature, the presence of Plasmodium in peripharal blood smears and, in some cases, on positive serological tests. Thirty two patients were Polish citizens, and three persons were foreigners. Malaria was caused mostly by invasion of Plasmodium falciparum (62.8), then P. vivax (31, 4), in 1 case--P. ovale and 1 case--mixed invasion occurred (P. falciparum and P. vivax). The majority of cases caused by P. falciparum were imported from Central Africa. Invasions of P. vivax were brought from North Africa, India and Middle East. Malaria in Polish patients was connected with occupational exposure and lack of proper antimalarial prophylaxis was obvious. A clinical course of disease was serious, with one mortal case. Fever, headache, abdominal pain, weakness, jaundice, insomnia were main complaints. Anemia, leucopenia, thrombocytopenia, hyperbilirubinemia, hypertransaminasemia and high serum concentration of urea were observed. A level of parasitemia in peripheral blood varied from minimal to very high (22.5%) in cases of P. falciparum invasions. In treatment chloroquine, fansidar, quinine, primaquine, halfan were used.

Malaria epidemiological situation in Poland since nineteenth century to 1995 has been described. The changes observed during this period are enormous. Poland has been transformed from endemic country with huge epidemics into the country with sporadic imported malaria cases.

Background: Since 1963, malaria has been exclusively recognized in Poland as an imported disease, which has become increasingly frequent in recent years. When associated with performing duties in malaria-endemic areas, malaria is acknowledged as an occupational disease. Methods: Ninety five cases of malaria recognized as an occupational disease in Polish citizens in the years 1984-1993 were studied retrospectively in terms of working conditions, epidemiologic and clinical features, and permanent disability sequelae. Results: Among patients examined, young and professionally active persons who called at tropical ports or who worked in rural areas or in the regions of military conflict were predominant. Most infections were contracted in Africa during the first few months of the stay. The observed morbidity and recrudescence rates were mainly attributable to Plasmodium falciparum. Disregard of prevention and chemoprophylaxis of malaria were key risk factors. In 33% of the patients, severe symptoms and complications occurred and these required long-term therapy. In seven cases, disturbances of the central nervous system, hepatic, renal, or cardiac system occurred 6 months after termination of treatment, and these occurrences resulted in permanent disability and an inability to work. Conclusions: The disability and inability to work and its legal consequences, such as indemnity, demonstrate that malaria may present a serious socioeconomic problem, even in the country where it is an imported disease.

In 1997 37 malaria cases were registered in Poland. All of them were imported, mainly from Africa (26), and some from Asia. Plasmodium falciparum infection was confirmed in 23 cases. Among 37 malaria cases 31 were men. The majority of malaria cases (27) were in 20 to 49 year old group. Eighteen persons travelled abroad in the connexion with their job (especially as sailors and agropilots) and eleven--as missionaries. Three men (44, 47 and 70 years old) with P. falciparum malaria died.

Malaria infections among seafarers have been regularly reported from Denmark, Germany, France, Belgium, Italy, Poland, The Netherlands, United Kingdom and other countries. No statistics are available on malaria infections among crews of ships flying "flags of convenience". The estimated number of malaria cases in international seafarers may be between 500 and 1000 each year; and fatalities among them are reported. This has been an important health problem for seafarers and preventive interventions are essential. In the strategy of malaria prevention in this occupational group the most important objective is to prevent death caused by severe Plasmodium falciparum infections, in which the clinical manifestations occurred during the ship's voyage, and the diagnosis and treatment were delayed. Protective measures against malaria on ships, benefits, risks and limitations of chemoprophylaxis are discussed.

The study group comprised 118 patients suspected of malaria who underwent an examination following their return to Poland from the tropical countries. Evaluation of the admitted patients was based on epidemiological, clinical and parasitological criteria. The obtained data allowed to confirm acute malaria in 20 patients (16.9%) and malaria in the past, in 32 patients (27.1%). In the remaining 65 patients (55.1%) we excluded malaria. Malaria caused by P. vivax was confirmed in 5 patients, by P. falciparum in 9 patients. A mixed invasion was diagnosed in 6 patients including: P. falciparum and P. vivax in 3 cases; P. vivax, P. ovale and P. malariae in 2 cases; P. falciparum, P. vivax and P. ovale in 1 patient. We noted no relationship between the severity of the clinical course and IFA results. It was found an atypical clinical course of malaria and diagnostic difficulties in patients who used chemosuppression for malaria prophylaxis.

In 1999 32 malaria cases were registered in Poland. All of them were imported, mainly from Africa (25). Plasmodium falciparum infection was confirmed in 20 cases. Among 32 malaria cases 26 were men and 26 in 20 to 49 years old group. Ten persons travelled abroad in the connection with their job, nine--as tourist. In 1999 51-year-old man died with myocarditis complicating malaria.
