In order to be applicable to sero-epidemiology, serological reactions must lend themselves to micro-techniques and automation. Their results, notably concerning malaria, sleeping sickness, amoebiasis, schistosomiasis and filariasis, complement those of direct parasitological examination. Serology is often more accurate, but is also more costly and should be reserved for those cases in which other analytical methods are either impossible or too undependable (low transmission level). Serological methods generally involve little risk or false negativity, but they can be falsely positive and improvement of the specificity of available testing procedures is at present one of the major technological problems of parasitic sero-epidemiology.

A comparison is made of enzyme-immunoassay and radio-immunoassay for the detection of antibody in Chagas's disease, sleeping sickness, malaria, schistosomiasis ans invasive amoebiasis. Both assays were sensitive and reproductible and gave comparable results.

The microbiological and parasitic risks associated with the tremendous increase in the tourist traffic from West Germany and Austria into warmer countries are considerable even in the case of shorter stays, as has been confirmed by observations made on those returning from such areas. The most important groups of diseases -- worm diseases, amoebiasis, and malaria -- have maintained this ranking in Central and South America; in Africa, especially in the east, the incidence of malaria tropica is increasing. The proportions for Austria show that even numerically there are no great differences compared with West Germany. It seems that, as far as malaria is concerned, efficient prophylactic measures and reliable information are absent; at any rate, the cases of malaria tropica were severe and in the main no prophylactic measures had been taken. With the exception of Japan, the risk of hepatitis in all warmer countries and in the East in general is many times higher than in the Federal Republic of Germany and the length of stay is partly a determining factor. For instance, the risk of hepatitis is roughly forty times higher in India. All figures shown in this survey are supported by tables, literature, and personal experience.

The hepatic manifestations were studied in 65 patients having uncomplicated primary attacks of vivax and falciparum malaria. Hepatomegaly due to a "non-specific reactive hepatitis" occurred in 57% of cases. Jaundice occurred in 15% of patients and was invariably associated with hepatomegaly. The clinical syndromes of jaundice and hepatomegaly in uncomplicated primary attacks of malaria have to be distinguished from those related to disorders like viral hepatitis, hepatic amoebiasis, typhoid hepatitis, infectious mononucleosis and Q fever. The causes for the jaundice and the pathogenesis for the hepatic lesions have been discussed.

The steady growth of intercontinental travel is causing an increasing incidence of tropical diseases in Switzerland. Malaria and amebiasis deserve special attention, due to their frequency and potential fatality. The epidemiology, clinical findings, diagnostic approach, and current treatment of malaria and amebiasis are briefly reviewed. If these infections are recognized in time and treated according to modern standards, fatal outcome is extremely rare.

The specificity of a circulating antibody observed in American trypanosomiasis and reacting with endocardium, blood vessels, and the interstitium of striated muscle (EVI factor) was evaluated in the indirect fluorescent antibody test with 60 sera from patients with malaria, leishmaniasis, echinococcosis, amebiasis, African trypanosomiasis, toxoplasmosis, and trichinosis, collected from areas where Chagas' disease is not endemic. Two sera, 1 from a patient with Plasmodium falciparum malaria and 1 from a patient with a relapse pretreatment post kala-azar dermal leishmaniasis, were positive for the EVI factor. In the leishmaniasis group, 3 of 8 sera reacted with 0ovine, murine, and human skeletal muscle. In this reaction, which differs from the EVI test, the sarcolemma and the intracellular structures were stained.

Imported parasitic diseases have been increasingly important in basic and clinical medicine in developed countries, as the number of travellers in developing countries, where numerous kinds of parasitic diseases are highly endemic, have been increasing. Recent epidemiologic investigation demonstrated that malaria, amebiasis, trypanosomiasis, leishmaniasis and cysticercosis seemed important as judged from the number of infected individuals, primarily Japanese travellers. Although these parasitic diseases pose some serious medical problems, establishment of the effective chemotherapeutic strategy is of urgent significance. In this respect, activities of the Research Group of Development of Chemotherapeutic Agents against Tropical Parasitic Diseases supported by the Japanese Ministry of Health and Welfare appear to be important to keep appropriate drugs in Japan.

The IFA test developed using Leishmania donovani promastigote and amastigote antigens showed very high antibody titre in clinically and parasitologically established cases (30) of kala-azar, the geometrical mean reciprocal titre (GMRT) being 870 +/- 5.4 and 5370 +/- 1.80 respectively with the two antigens. In contrast, the GMRT of normal subjects of endemic area was only 12.44 +/- 6.19 and 80.35 +/- 1.66 respectively with these antigens. None of the subjects from non-endemic area suffering with other parasitic diseases, such as malaria, filaria, amoebiasis, leprosy or tuberculosis (20 cases each) gave a positive response to any of the antigen. The test holds promise in the diagnosis of Kala-azar.

A recombinant iron-containing superoxide dismutase (recFeSOD) of Entamoeba histolytica was produced in a prokaryotic expression system. Purified recFeSOD was found to be enzymatically active as determined by (i) inhibition of ferri-cytochrome c reduction, (ii) dismutation of superoxide anions generated by human neutrophils and (iii) inhibition of nitroblue tetrazolium reduction. The enzymatic properties of recFeSOD were similar to those of the native protein in trophozoite extracts. In an ELISA using recFeSOD as antigen, 96% of sera from patients having invasive amebiasis were reactive whereas none of the healthy controls or of patients suffering from malaria, bacterial or viral infections were reactive. Only sera of Toxoplasma-, Leishmania- or Trypanosoma-infected individuals exhibited partial cross-reactivity to recFeSOD.

The experience in treating infectious diseases in wounded and traumatized persons from the contingent of the Soviet Troops in ++Afghanistan is described. The syndrome of the joint effect of the injury and infection is characterized. Features of the chemotherapy of the injured patients with viral hepatitis, malaria, typhoid fever and enteric and ++extra-enteric amebiasis under the ecological and professional stress and with an account of the etiology of the wound infection are presented. The favourable effect of the developed methods for chemotherapy of the joint affections was shown.
