Absorption of folates, vitamin B6, pantothenate, and riboflavin from a natural food source (
yeast)
and from synthetic folylmonoglutamate was studied in 24 elderly subjects (age range, 73 to 101 years) and in 12 healthy younger subjects (age range, 24 to 42 years). All subjects absorbed riboflavin, vitamin B6, and pantothenate from yeast. Ingested folypolyglutamates (the preponderant folates in
yeast)
proved to be a very poor source of folates for the elderly subjects, whereas synthetic folylmonoglutamate served as a good source. In the younger subjects, yeast was a significant folate source. It is suggested that the folate deficits so common in the elderly are caused by impaired ability to obtain folate from ingested foods. The resultant folate deficit may induce changes in the epithelial structure and enzyme secretion of the small bowel, which may further exacerbate folate

Sucrase-isomaltase deficiency is an inherited disaccharidase deficiency that leads to
malabsorption
of sucrose, with resulting diarrhea and abdominal distention and cramps. We investigated the sucrose-splitting effect of viable yeast cells in eight children with congenital sucrase-isomaltase deficiency, by means of the sucrose hydrogen breath test. This test is based on the fact that hydrogen is released from the malabsorbed sucrose by the colonic microflora. We found that 0.3 g of lyophilized Saccharomyces cerevisiae, given after loading with 2 g of sucrose per kilogram of body weight, reduced hydrogen excretion in all patients, on average by 70 percent, in parallel with a complete loss or evident reduction of clinical symptoms. In vitro, lyophilized and fresh S. cerevisiae (fresh baker's
yeast)
had appreciable sucrase activity, a low isomaltase and maltase activity, and virtually no lactase activity. The sucrase activity was more inhibited by undiluted than by diluted gastric juice. We conclude that patients with congenital sucrase-isomaltase deficiency who intentionally or unintentionally consume sucrose can ameliorate the
malabsorption
by subsequently ingesting a small amount of viable yeast cells, preferably on a full stomach.

The feasibility and efficacy of adding microbial beta-galactosidase enzymes directly to milk at the time of consumption was explored in adult lactose-malabsorbers. The hydrogen breath test, and on one occasion, the rise in blood glucose, were used as indices of the completeness of intraintestinal hydrolysis and absorption of milk lactose. When added to 360 ml of cow milk containing 18 g of lactose, empirical dosages of three beta-galactosidases--one from Kluyveromyces (
yeast)
and two from Aspergillus (fungal)--had some effectiveness in reducing postprandial H2 excretion, although no in vivo treatment at the dosages chosen was as effective as pre-incubation of the milk in vitro. The yeast enzyme also reduced symptom frequency as compared to intact milk and enhanced postprandial rises in blood glucose. The replacement therapy with exogenous, food-grade beta-galactosidases may provide a useful intervention to reduce lactose
malabsorption
and milk intolerance in individuals with primary lactase deficiency.
