A case of
eosinophilic gastroenteritis
in a 42-year-old man is described. The patient had diarrhoea, faecal blood loss, a protein-losing enteropathy,
malabsorption
of fat, xylose and vitamin B12. Co-existent hypopituitarism, diabetes insipidus and hypothalamic dysfunction was demonstrated. Complete clinical recovery occurred with pituitary replacement therapy alone. The association of this endocrine abnormality with the picture of
eosinophilic gastroenteritis
has not previously been described.

A patient with diffuse
eosinophilic gastroenteritis
, associated with progressive chronic interstitial pulmonary fibrosis, was followed for 29 years. His major symptoms were recurrent gastrointestinal bleeding, anemia, diarrhea,
malabsorption
, and increasing respiratory insufficiency. Multiple jejunal biopsies revealed eosinophilic infiltration and extensive villous atrophy. Results of a lung biopsy and pulmonary function tests were consistent with chronic interstitial pulmonary fibrosis. His gastrointestinal symptoms responded to steroid therapy, but he died of progressive respiratory insufficiency. At autopsy, villous atrophy was noted to extend throughout the whole extent of the small intestine in association with multiple "pseudopolypoidal" inflammatory lesions.

We report the case of a patient with recurrent subocclusive episodes and diarrhea (no
malabsorption
) associated with ascites, in the absence or liver, kidney or heart disease. The demonstration of hypereosinophilia in the peripheral blood and in the ascites fluid and the failure to identify parasitic or haematological disorders have led to a through examination of the stomach (Endoscopy, Echoendoscopy), small bowel (X-rays and Computerized Axial Tomography) and colon (colonoscopy) in a search for parietal lesions. The absence of segmental lesions and the observation of CAT images of diffuse, regular thickening of the ileum and of the mesentery, coupled with the monotonous clinical history spanning over three decades, have led to a diagnosis of
eosinophilic gastroenteritis
with involvement of the serosal layer. Serosal involvement is rare in eosinophilic disease of the gut; in analogy with other cases reported in the literature, steroids have improved clinical symptoms and normalized the hematological picture.

Eosinophilic Gastroenteritis
(EG) is a poorly understood disorder defined by eosinophilic infiltration of the bowel wall, eosinophilia and gastrointestinal symptoms. The disease's aetiology, course and treatment are not well known. We report two atypical cases of EG: one involving the mucosal layer and another involving the serosal and muscularis layer. The first shows how EG may present with a long history of episodes of intestinal obstruction and
malabsorption
and how the disease could take a severe course and may be unresponsive to treatment. The second case shows EG presenting as acute abdomen and which subsequently became asymptomatic without therapy, regardless of the fact that peripheral eosinophilia remained present. This case raises the problem of how to treat an asymptomatic patient, what parameters should be considered in order to assess the progress of the disease and the indications for treatment.

of the muscularis and serosal type which involved the terminal ileum. The diagnosis was initially acute appendicitis, but clues to the correct diagnosis included the presence of
malabsorption
, protein-losing enteropathy, abdominal pain and ascites, and especially a history of recurrent bouts. Peripheral eosinophilia is found in most cases and biopsy, or a peritoneal tap when ascites is present, may demonstrate a predominantly eosinophilic infiltration. Radiographic findings include a rigid, open ileocecal valve. There is rapid response to corticosteroid therapy, but long term follow-up is required because the disease may run a chronic, relapsing course.
