A case of severe diarrhoea and steatorrhoea following chemotherapy and abdominal irradiation for
lymphosarcoma
, is described. Investigations demonstrated bile acid
malabsorption
, and treatment with cholesytramine and a low fat diet was successful. A questionnaire was sent to thirty-two subjects who had undergone similar treatment for similar pathology and failed to demonstrate any sustained alteration of bowel function. Additionally, nine of these subjects underwent a 14C-glycocholic acid test and no evidence of interruption of the enterohepatic circulation of bile acids was found.

A case of extensive primary lymphocytic lymphoma (
lymphosarcoma
) of the gastrointestinal tract is reported. At the time of presentation the patient had diffuse involvement of the gastrointestinal tract from the stomach to the ano-rectal junction.
Malabsorption
was present. Pre-treatment immunologic studies showed elevated serum IgA and IgG, but impaired humoral and cellular immunity. Death occurred 13 months after diagnosis.

Three horses suffering from
malabsorption
were shown to have alimentary
lymphosarcoma
predominantly affecting the small intestine and the associated lymph nodes. The diffuse cellular infiltration in two of the case produced marked changes in the villous architecture reducing the available mucosal surface area, and, with lowered or barely detectable disaccharidase activities, contributed to the impairment of digestive-absorptive processes. One of the horses maintained a voracious appetite and was not diarrhoeic, but failed to gain weight, indicating differences in the production and utilisation of energy sources absorbed from the small and the large intestines. A large proportion of plasma cells in the infiltrate, and the resulting morphological changes in two of the cases bore a striking resemblance to features of alpha chain disease, an immunodeficiency disease of man, although immunological studies were not conducted on these horses.

Specificity of the oral glucose tolerance test (OGTT) for the diagnosis of small
intestinal malabsorption
in the horse was assessed by comparing the results of OGTT with the results of a histopathological examination of the small intestine in 42 adult horses affected by chronic weight loss. The horses were assigned to three groups on the basis of the results of the test. Five horses were considered to have a normal OGTT absorption result (Group 1); all the horses had a histologically normal small intestine. Twenty-five horses had a partial
malabsorption
result (Group 2) seven of which had normal small intestinal morphology, whereas the remaining 18 had a variety of pathological lesions including
lymphosarcoma
, villous atrophy, granulomatous enteritis and eosinophilic gastroenteritis. Twelve of the 42 horses had a total
malabsorption
result (Group 3), and all had a severe infiltrative lesion in the small intestinal wall (either
lymphosarcoma
or granulomatous enteritis).

An improved folic acid absorption test is described in detail. Using the folic acid and other absorption tests, a study of intestinal function has been made in eight patients with
lymphosarcoma
, eight with chronic lymphocytic leukaemia, two with Hodgkin's disease, and one each with reticulum cell sarcoma, Whipple's disease, and macroglobulinaemia. It was found that laboratory evidence of
intestinal malabsorption
was extremely common in patients with this group of diseases. The folic acid absorption test was the most sensitive single index of
malabsorption
of the tests used in this study. Correlation between the different absorption tests was poor, and there does not appear to be a characteristic biochemical profile of intestinal function in these patients.
