To clarify the mechanism of high susceptibility to infection in cirrhotics, the changes in adenylate energy charge and MTT assay in peripheral blood lymphocytes were studied in cirrhotic and noncirrhotic patients in the early postoperative period after hepatectomy. The adenylate energy charge measured by radioactive labeling of the lymphocyte adenine nucleotide pool showed no significant difference preoperatively between cirrhotics and noncirrhotics, but a significant difference was observed in the pre- and postoperative distribution of adenine nucleotide metabolites (P less than 0.01). In the cirrhotic group, the adenylate energy charge of
lymphocytes decreased
significantly to 0.807 +/- 0.011 on the third postoperative day compared with preoperative value (0.891 +/- 0.006, P less than 0.01) and was restored to the normal range on the fifth and tenth postoperative days (0.886 +/- 0.006, 0.899 +/- 0.014), while no significant decrease was observed in the noncirrhotic group. MTT assay revealed that lymphocyte cell function decreased significantly in cirrhotics after hepatectomy. These results indicate that, in cirrhotic patients, the energy metabolism of lymphocytes is already impaired to some extent preoperatively, and that it undergoes further deterioration when surgical stress is applied. It is suggested that the
decreased energy
metabolism in the lymphocyte may be responsible for the increased susceptibility to infection in postoperative cirrhotics.

We describe an epidemic of self-limited (6 weeks) thyrotoxicosis which affected 12 index cases, 5 household contacts, and 6 retrospectively identified cases in July, August, and September, 1987 in the town of Winterswijk (28,011 inhabitants), The Netherlands. A small goiter was present in 9 of the 12 index patients, tender upon palpation in only 2. Signs and symptoms of thyrotoxicosis were accompanied by a low grade fever in combination with

No evidence was found for thyrotoxicosis factitia. After 10 months, all patients were euthyroid, without a goiter or thyroid autoantibodies. Thus, a new variant of thyroiditis, atypical subacute thyroiditis, was probably the cause of this unusual outbreak. It is unclear at present if this variant of thyroiditis is common in communities and represents a separate disease entity.

Eleven patients with rheumatoid arthritis that had been refractory to conventional drug therapy were treated with total lymphoid irradiation (TLI). Followup continued for 6 months in 9 patients, 12 months in 6 patients, and 24 months in 3 patients. in all patients. T lymphocyte subsets decreased after TLI, and showed a transient increase at 6 months post-TLI. The observed increase in morbidity and mortality after TLI is evidence that discourages the use of this therapeutic technique, at least in its present form.

Studies were initiated to assess the response of patients with disseminated melanoma to recombinant alpha interferon (rIFN-alpha A) and to monitor effects of rIFN-alpha A on several tests of immune function. Twenty patients were treated with rIFN-alpha A given by i.m. injection in escalating doses from 15 to 50 X 10(6) um-2. The responses of two patients were considered unevaluable. Of the remainder there was complete remission of tumour in two and stable disease in two. Subsequent progression of tumour in one of the latter patients coincided with development of antibodies to IFN. Side effects (usually

) were dose rate limiting in 11 patients. Laboratory tests on samples taken 6 hours after rIFN-alpha A indicated a marked
lymphopenia
and a reduction in natural killer (NK) cell activity particularly against K562 target cells. Longer term changes measured in samples taken 2 days after the previous rIFN-alpha A injections consisted of neutropenia and an increase in the T4/T8 ratio due mainly to a relative increase in OKT4 positive T cells compared to OKT8 positive T cells. NK activity against the K562 target cell increased in most patients during the first week of treatment and then returned to below or near pretreatment levels thereafter against the K562 target cell. This contrasted with NK activity against the melanoma target cell which showed a more gradual increase over the duration of the treatment in 6 patients. The latter correlated with an increase in mitogen stimulated IL 2 production from their blood lymphocytes and may indicate that the cytotoxic activity resulted from lymphokine-activated killer (LAK) cells. These results confirm the activity of rIFN-alpha A against melanoma in certain patients. They suggest that further studies are needed to select patients who may respond to rIFN-alpha A and to optimize treatment regimens. Tests of IL 2 production and LAK activity may assisted in achieving these objectives.

In 3 patients with low-grade astrocytomas clinical pharmacology of interferon-beta (10(7) U/mg protein) was investigated. Interferon-beta with escalating dosage (2.3, 6.9, 23, 69 X 10(6) U/patient) was given to each patient in 4 infusions at weekly time intervals. In these patients dose-dependent plasma-levels of interferon-beta of up to 5800 IU/ml were achieved. Plasma concentrations showed a biphasic decline (T1 1/2:0.095-0.49 hrs and T2 1/2: 5-14.5 hrs). Observations of 12 patients with AIDS at this institution from March 1981 to April 1984 are reported. Ten patients were homosexuals and two were bisexual. Another patient had a chronic mucocutaneous infection with herpes simplex and another an intestinal cryptosporidiosis and Kaposi's sarcoma. Alpha-A-interferon was used to treat patients with Kaposi's sarcoma and three patients with limited disease showed a favorable response. Six patients with advanced disease died.

Lymphopenia
, T cell deficiencies, and imbalances in T cell subpopulations established the diagnosis of AIDS. Treatment with arabinoside A and fibroblast interferon was not effective. THF was then administered by daily injections for 3 weeks. At the end of the 2nd week of treatment, increased alertness and appetite were noted. Also documented was a transient increase in circulating lymphocytes, T cells, and helper cells. However, at the end of the 3rd week, the patient developed bronchopneumonia followed by renal failure and died in an acute encephalopathic state. Only 1 other (unsuccessful) attempt to treat an advanced case of AIDS with a thymic hormone has been reported. It is suggested that THF treatment might be more effective if offered at a very early stage of AIDS. It is hypothesized that a sufficient pool of precursor T cells is required to serve as target cells for immune modifiers such as THF. This precursor pool probably shrinks beyond rescue in the advanced stages of AIDS. The authors are currently involved in trials of immunomodulation with THF at early stages of immune impairment in pre-AIDS patients.

The ATC therapy is supposed to have a positive effect on all cancers and immunodeficiencies. Side effects are denied, although oncologists have noticed

, leukocytosis and others. The dermatologist supposedly establishes tumor cell and leukocyte cultures from blood and mixes them later on. The cytokines (ATC), produced by the lymphocytes, are harvested in 40 ampules and then subcutaneously injected. One cycle with the ATC therapy costs around 3000 DM. The dermatologist believes that tumor cells can only be immunologically identified during the dividing phase. He stimulates the cultured tumor cells to divide. At the same time he adds the autologous immunocompetent cells which should be trained to identify tumor cells and to produce cytokines. The dermatologist uses many terms unconventionally and imprecisely. The in-vitro training of lymphocytes for tumor defense with the method of the dermatologist is experimentally unproven. The production of the ATC preparations is kept secret. Also unclear are the mechanisms of action and the clinical efficacy. Besides neologisms, the dermatologist does not offer any scientific or clinical facts for his therapy of cancers.
