Three, single-day nutritional surveys at weekly intervals were conducted in the general medical wards of an urban municipal teaching hospital. The techniques of nutritional assessment included anthropometric measures (weight/height, triceps skin fold, arm-muscle circumference,

, and hematocrit). The prevalence of protein-calorie malnutrition was 44% or greater by these criteria (weight/height, 45%; triceps skin fold, 76%; arm-muscle circumference, 55%;

lymphopenia
of 1,200 cells/cu mm or less was found, a level likely to be associated with diminished cell-mediated immunity. On the basis of a retrospective study of 79 well-documented cases of Hodgkin's disease it has been possible to determine the prognostic significance of several factors in regard to 3-year survival. All patients evaluated were in stage III or IV and receiving polychemotherapy. According to the survival curves, the following factors adversely affected the prognosis to a significant extent: clinical stage IV, the presence of systemic symptoms, male sex, and failure to receive maintenance therapy. Also, patients treated by polychemotherapy only on relapse following other prior treatment, had a somewhat worse prognosis, though in out patient material this was not statistically significant. Other criteria significantly affecting the probability of 3-year survival in a negative sense were: decreased activity index, anemia,

The prevalence of protein calorie malnutrition (PCM) based on ten nutritional parameters was studied in 307 patients undergoing major elective surgical operations. These parameters included anthropometric measurements (weight/height, triceps skin fold thickness, arm muscle circumference) and biochemical (serum total proteins, albumin, transferrin, prealbumin, retinol binding protein) and immunological tests (total lymphocyte count and delayed hypersensitivity test). Using these criteria, the prevalence of PCM was high. Eighty-six percent of patients had at least one abnormal parameter. The prevalence of PCM as judged by weight/height and arm muscle circumference was 49% and 62% respectively. The incidence was higher in cancer than non cancer patients (63% vs 43%). Although
serum albumin
and total protein levels were normal in 93.5% of patients, acute serum protein markers such as transferrin, prealbumin and retinol binding protein were low in 20-30%.
Lymphopenia

Withholding iron dextran treatment normally given to pigs at 1-3 days of age to prevent anemia resulted also in neutropenia. Polyinosinic acid:polycytidylic acid (poly I:C) at 0.5 mg/kg IV at 25 days of age resulted in induction of putative interferon 2 to 24 hours later, with significantly (P less than 0.05) lower concentrations in iron-deficient (Fe-) female pigs than in iron-supplemented (Fe+) female pigs. Poly I:C caused several transient toxic manifestations, including elevations in blood urea nitrogen, creatinine, aspartate aminotransferase, potassium (K), total bilirubin and phosphorus (P), marked leukopenia (both neutropenia and

Thirty hospitalized patients with newly diagnosed tuberculosis were studied prospectively with a range of in vitro and in vivo tests of immune function. Responses were compared with those of healthy controls matched for age, sex, ethnic group and diet. A series of metabolic and immunologic abnormalities was found, including evidence of undernutrition, anaemia, neutrophil leucocytosis, monocytosis,
lymphopenia
, hyperglobulinaemia and raised erythrocyte sedimentation rate. Some patients had accelerated, others diminished, cutaneous tuberculin hypersensitivity, and some had diminished mononuclear cell proliferative and lymphokine responses to tuberculin (purified protein derivative, PPD). The patients were not uniform in their responsiveness, but could be arranged within a spectrum which showed a relationship to crude bacillary excretion and response to treatment. 27% of patients were characterized by hypersensitivity, with normal in vitro cellular responses and skin tests to PPD, scanty bacillary excretion and rapid bacteriologic sputum conversion to negative cultures with treatment. In contrast, 30% of patients were relatively anergic with negative skin tests, reduced or absent in vitro cellular reactivity to PPD, moderate or heavy bacillary excretion and later (greater than 4 weeks) bacteriologic sputum conversion. The remainder of the patients fell between these two groups. There were no correlations between cellular immunity on the one hand, and radiological extent of disease, levels of serum immunoglobulins, peripheral white cell counts or ESR on the other. The acute effects of cyclophosphamide, vincristine, and L-asparaginase on the humoral and cell-mediated immune responses in dogs were evaluated. Dogs were given a single dose of cyclophosphamide (10 mg/kg of body weight, IV), a single dose of vincristine (0.025 mg/kg, IV), a single dose of L-asparaginase (400 U/kg, IV), or a combination of these drugs at the aforementioned dosages administered simultaneously. The total number of leukocytes decreased significantly (P = less than 0.05) on posttreatment day 3 in the cyclophosphamide-treated dogs. Nitrogen balance in six obese adolescent boys and three obese girls was studied during two nonconsecutive three-week dietary periods. Protein intake was based on an ideal body weight calculated from the measurement of total body water. The diets consisted of 1.5 gm meat protein/kg IBW/day plus 1.0 gm glucose/kg IBW/day, or an isonitrogenous diet made isocaloric with fat. Each dietary period was preceded by a controlled five-day diet designed to achieve weight maintenance. The first dietary period was followed by a one-month period of re-equilibration. The addition of carbohydrate to protein produced a significantly better cumulative nitrogen balance than P + F. Significant nitrogen losses persisted throughout the entire dietary period of P + F in three patients, but were observed in only one patient during P + G.
Lymphopenia
occurred in one subject on P + F but did not occur during P + G.
Serum albumin
concentrations were unchanged on both diets. Transferrin values decreased during P + F, but did not differ significantly from prediet levels during P + G. However, the observation of prolonged nitrogen losses in one subject on P + G and the limited experience with highly restrictive diets in obese adolescents emphasize that such diets must be used with caution and monitored carefully.

The uptake of tritiated thymidine by isolated peripheral blood lymphocytes obtained from male guinea pigs immunized with bovine
serum albumin
was studied in animals maintained on various amounts of Vitamin C for 28 days. Animals were pair-fed on ascorbate-free diet and were supplemented intraperitoneally with 0, 25, or 250 mg Na-ascorbate per day. Scorbutic animals lost weight rapidly during the final 2 experimental weeks. Their daily food intake averaged only 4 g/day during the last week; thus, pair-fed ascorbate-supplemented groups were also subjected to acute nutritional stress. Lymphocytes from guinea pigs receiving 250 mg Na-ascorbate per day incorporated in vitro the highest amounts of tritiated thymidine both in the absence of nonspecific mitogen and in the presence of concanavalin A or phytohemagglutinin. Responses to lipopolysaccharide were not conclusive. Total circulating white cells counts and relative numbers of T and B lymphocytes were assessed in a second study made under identical constraints. In scorbutic animals the percentage of B lymphocytes increased and that of T
lymphocytes decreased
continuously over the 4-week period. The opposite effect was observed in vitamin C-supplemented animals. These studies suggest that very high doses of ascorbate support elevated mitotic activity after 4 weeks of much reduced food intake.
