Persistent unexplained lymphadenopathy (LAS) with intermittent fever, weight loss, night sweats and malaise was observed from March to October 1983 in 16 of 133 intravenous drug addicts who had been followed for at least two years in a Center for Drug Addicts Assistance in Milan, Italy. All the subjects lived in a restricted suburban area and indulged in frequent toxicomanic practices and mutual sexual intercourse. The subjects showed immunological alterations such as
lymphopenia
(50%), decreased T helper/T suppressor ratio (93%), both these abnormalities (43%), decreased T helper cells (75%), increased T suppressor cytotoxic cells (81%), decreased natural killer (NK) activity (77%), anergy (50%) or hypoergy (43%) to recall skin testing and elevated levels of IgG (87%). Anti-HTLV III antibodies were found in 14 of 16 (87%) patients with LAS and in 3 of 11 (27%) symptom-free drug addicts belonging to the same group. It will be important to assess in the future whether this clinical and immunological picture results in
acquired immunodeficiency syndrome
in an area so far untouched by this disease.

Clinical, biopsy and necropsy studies of a homosexual boar revealed angio-proliferative lesions in the skin of the thigh, scrotum and the inguinal lymph node. Angiofibromas were identified in the dermis and subcutis of the thorax and mandible. The avidin-biotin-peroxidase complex technique demonstrated Factor VIII-related antigen as a marker for the neoplastic endothelial cells of the tumors. This boar also showed weight loss and
lymphopenia
. Explants of tumors were maintained in cultures for 22 passages and cultured cells produced tumors when injected subcutaneously into nude mice. No viral antigen was detected in the cultured tumor cells. Similarities and differences between the lesions in the boar and human Kaposi's sarcoma are discussed.

The immunological status of two multiply transfused patients with severe haemophilia A and diffuse lymphadenopathy was evaluated. But patients appeared clinically well and denied homosexuality or intravenous drug abuse. Immunological studies revealed depressed cellular immune function with anergy, relative
lymphopenia
, decreased T helper cells, and increased T suppressor cells in both patients and an altered T-helper-cell/T-suppressor-cell ratio in one. Hyperresponsiveness of the humoral immune system was demonstrated by elevated IgG and IgM. Lymph-node biopsy revealed benign nonspecific hyperplasia in both patients. The lymphadenopathy and immunological features in these two haemophiliacs bear a striking resemblance to the
acquired immunodeficiency syndrome
(
AIDS
) of homosexuals, intravenous drug abusers, and Haitian immigrants. These findings may represent a prodromal phase or a forme fruste of
AIDS
. Transmission of an infectious agent in blood products seems likely.

Acquired immunodeficiency syndrome
(
AIDS
) is a disease of unknown aetiology characterised by opportunistic infections, unusual malignancies, and aberrations in cellular immunity;
lymphopenia
and reversal of helper/suppressor T-cell ratios are the most common. In this study the immune function in 25 children and adolescents with haemophilia was assessed. Normal humoral immunity was observed in all patients evaluated. T and B cell numbers and stimulation were within normal limits. However, 5 of the patients had reversed helper/suppressor T-cell ratios similar to, but less pronounced than, the reversed ratio observed in patients with clinical signs of
AIDS
. These results suggest that patients with haemophilia may be at increased risk of
AIDS
and that regular investigation of cellular immunity is warranted in these patients.

97 symptom-free homosexuals were studied clinically, serologically, and with in-vivo and in-vitro tests of cellular immune function in the context of the
acquired immunodeficiency syndrome
(
AIDS
). A high proportion of these men showed abnormalities:
lymphopenia
(33%), decreased T-helper/T-suppressor (Th/Ts) cell ratios (43%), both these abnormalities (12%), decreased total T-helper cells (15%), monocyte chemotactic (10%) and phagocytic (27%) defects, anergy to three recall antigens (32%), and anergy to purified protein derivative despite BCG inoculation (55%). The lymphocyte abnormalities and anergy characteristic of
AIDS
were seen in 5%. No clear clinical or serological associations were seen for the
AIDS
-like defects. Trends of association were seen between higher lymphocyte counts, lower Th/Ts ratios, more T-suppressor cells and serological evidence of previous virus infection. The combination of lymphocyte abnormalities and anergy observed in these symptom-free homosexuals may represent a latent phase of
AIDS
.

16 Zambian patients with Kaposi's sarcoma (KS) were studied to determine whether they had evidence of
lymphopenia
with decreased T helper/T suppressor (Th/Ts) ratios or previous infection with opportunistic pathogens. Serological tests for viruses commonly associated with the
acquired immunodeficiency syndrome
(
AIDS
) were also carried out. 12 patients had a decreased Th/Ts and 2 of these were also lymphopenic. Serological evidence for infection with Toxoplasma and with Pneumocystis was present but this was not significantly more common in KS patients than in controls. All 16 patients had antibodies to cytomegalovirus (CMV), 15 had antibodies to Epstein-Barr virus and 13 to human T leukaemia virus (HTLV) infected cells. 5 patients had evidence of previous infection with hepatitis B virus. African patients with KS seem to have an immunological and virological profile similar to that seen in American patients with
AIDS

Because the current outbreak of
acquired immunodeficiency syndrome

) among previously healthy adults may be caused by a transmissible biologic agent, and because it may be preceded by immunologic abnormalities with or without a prodromal illness, we studied seven female sexual partners of male patients with the syndrome. The male patients were all drug abusers. One of the seven women was found to have the full-blown syndrome, a second had an illness consistent with the prodrome of
AIDS
(generalized lymphadenopathy,
lymphopenia
, and a decreased ratio of helper to suppressor T cells), and four others had generalized lymphadenopathy or
lymphopenia
, with or without a decreased ratio of helper to suppressor T cells. Only one woman had no abnormalities. These findings suggest that
AIDS
may be transmitted between heterosexual men and women.

Two patients with hemophilia A had generalized lymphadenopathy,
lymphopenia
, elevated IgG values, depressed T4 (helper) lymphocytes, elevated T8 (suppressor) lymphocytes, and abnormally low T4/T8 ratios. One of the patients, who also had hepatosplenomegaly, underwent cervical lymph node biopsy; the node contained 43% T8-lymphocytes, a marked elevation over the small fraction of T8 cells usually found in lymph nodes. These patients may have a form of the
acquired immune deficiency syndrome
described in male homosexuals, Haitians, intravenous drug abusers, and recently, in patients with hemophilia. We studied T cell phenotypes in 43 patients with hemophilia. Fourteen of 28 patients given commercial factor VIII concentrates had abnormal T4/T8 ratios; none of nine patients who used cryoprecipitate had abnormal values. T4 helper cells were significantly lower, T8 suppressor cells significantly elevated, and T4/T8 ratios significantly lower in the lyophilized concentrate users and in patients with hemophilia as a total group. A 36-year-old man had chronic, debilitating diarrhea due to cryptosporidiosis. This patient had longstanding common variable hypogammaglobulinemia and recurrent bacterial infections. Immunologic evaluation after discovery of Cryptosporidium showed
lymphopenia
with persistently reduced numbers of helper/inducer cells (OKT-4), variable numbers of suppressor/cytotoxic cells (OKT-8), OKT-4/OKT-8 ratio of 0.09, and increased levels of serum alpha-interferon, all of which describe the
acquired immunodeficiency syndrome
. Oocysts of Cryptosporidium were found in feces from the patient's cat, thus identifying a possible source of his infection. The patient had disseminated candidiasis, cytomegalovirus pneumonia, and cryptosporidiosis when he died.

Eighty-three symptom-free hemophiliacs were studied clinically, serologically and by in vitro tests for cellular immunity in a geographical area in which
AIDS
has not yet been encountered despite the exclusive use of concentrates manufactured from American sources of plasma. Some patients showed the following abnormalities:
lymphopenia
(4%), decreased T-helper/T-suppressor (Th/Ts) cell ratios (49%), or both abnormalities (2%). Low Th/Ts were mostly due to absolute or, less frequently, relative increases in Ts cells. The prevalence rates for these abnormalities were the same in patients treated with factor VIII or factor IX concentrates. There was an association between the higher Ts and lower Th cells counts and the low Th/Ts ratios and greater annual consumption of factor VIII and factor IX concentrates. These results support the view that protein load might be an important pathogenetic factor in lymphocyte abnormalities in symptom-free hemophiliacs.
