and horizontal diplopia. Clinical symptoms were consistent with inflammation of the right superior oblique tendon sheath, but a computed tomographic scan showed no pathology. Treatment with systemic corticosteroids reduced the symptoms, but the patient was left with a persistent muscle imbalance.

The long-term results of soft tissue metacarpophalangeal reconstruction without articular resection were reviewed in 16 hands of 12 patients with painful ulnar deviation-subluxation deformity. No splinting was used beyond 3 weeks. Patients had either

or rheumatoid arthritis. The mean age at operation was 66 years, with mean disease duration of 15.9 years before operation. At follow-up (mean, 81 months), complete
pain
relief occurred in 88% of patients and 56 degrees and 64 degrees mean active metacarpophalangeal and proximal interphalangeal range of motion was present, respectively. Ulnar drift was corrected to 6 degrees on the average. The first semiobjective grading scale for metacarpophalangeal reconstruction was introduced. There were 82% good or excellent results.

Advances in management of the rheumatic diseases over the past 20 years have been substantial. Over this period, survival of patients with
systemic lupus erythematosus
improved dramatically. Previously fatal renal crisis in scleroderma became treatable. Survival in Wegener's granulomatosis improved from 7% after 2 years to over 90%. Gout became an easily and effectively managed disease. Polymyalgia rheumatica became readily recognized and dramatically treatable. Less quantifiably, the shift toward more aggressive use of an increasing repertoire of "disease-modifying" agents in rheumatoid arthritis gave hope of having altered the natural history of the disease. Replacement of destroyed joints dramatically reduced

and improved function in appropriately selected individuals. An increasingly broad mission of the National Institutes of Health has provided support for systematic evaluation of clinical management, including the Multi-Purpose Arthritis Centers, the American Rheumatism Association Medical Information System, and the Cooperating Clinics, effectively complementing research in fundamental mechanisms of disease. The role of the concerned clinician and the clinical epidemiologist in identification of new syndromes and new diseases and in innovating approaches to their management has been extremely important; this role appears far from over.

A 39-year-old woman developed transient erythema and arthralgia in spring 1987. In June she had a tick bite followed by local erythema and later migrating skin changes. Furthermore she developed

in various joints with Raynaud's phenomenon at the fingers, swelling of the knee joints and shoulder pain. Demonstration of antibodies against B. burgdorferi antigen was shown in one institution (IFL, Western blot) while the same serum in two other institutions remained negative (IHA, ELISA). Antibiotic treatment was only temporarily successful. While the demonstration of antinuclear factors could be attributed to cross-reacting antibodies in borreliosis failing effects of absorption of serum with this antigen led to the assumption
SLE
as the underlying disease. Further indications were lymphopenia, increasing titers of anti ds-DNA antibodies and renal involvement as erythrocyturia and proteinuria. Sudden relief of the symptoms after treatment with steroids may be taken as further prove for this assumption. The interference of both diseases and their similarity in symptoms may impede correct diagnosis.

A cohort of 59 initially healthy subjects with chronic false-positive seroreactions for syphilis was followed for 3 to 19 years (mean 13 years) by data linkage to computerized population registry and to national hospital discharge registry using a unique personal identification number. One subject had moved abroad, but all others were known to be alive at the end of the follow-up period. Four subjects developed

and two developed rheumatoid arthritis. One subject was admitted to hospital because of protracted substernal

Three hundred and sixty two Japanese patients with rheumatoid arthritis (RA) and 455 patients with other rheumatic diseases (
SLE
233, PSS 63, MCTD 51, PM-DM 41 Behcet's disease 33 and OA 33) were examined for the evaluation of the 1987 revised ARA criteria for RA. In our cases sensitivity was slightly decreased and specificity was markedly increased 5 out of 7 criterions compared with the results reported by ARA. In the investigation how many number of criterions at least which the patients with RA should satisfy, 4 out of 7 criterions in the 1987 criteria turned out the highest figure in the accuracy (the mean of sensitivity and specificity values). So the patients who satisfied at least 4 criterions were classified to have RA. Sensitivity of the 1987 criteria decreased to 90% although that of the 1958 ARA criteria was 93%. Specificity were markedly increased from 88% to 95% (the 1958 and 1987 criteria, respectively). These results might be based on the revision of the duration of "morning stiffness" (lasting at least 1 hour) and on the deletion of "joint
pain
or tenderness" which was relatively less specific for RA.

From 1985 to 1987, we had 4 patients with infected emphysematous bulla, three males and one female ranging from 18 to 61 years of age. The male patients had been suffering from emphysematous bullae and had complications stemming from the resulting infection. In the female case, the infection occurred at the pneumatocele which had grown at pneumonia was caught during steroid therapy for
SLE
. While all cases were medicated with antibiotics intravenously, they were drained with a small caliber tube percutaneously, then were washed using a cytocidal agent (dilute povidone iodine). In two cases, an infected emphysematous bulla and an infected pneumatocele cleared, then shrank and closed. In the remaining cases, the infected emphysematous bullae also cleared and shrank, then the patients underwent operations. All patients then healed. The therapeutic combination of percutaneous drainage with a small caliber tube and washing of infected emphysematous bulla is available for therapy or preoperative treatment for the following reasons: First of all, especially in patients who have remarkably disturbed pulmonary function, it can be performed safely and pt's symptoms and the general condition of the patient improves rapidly. The second reason is that, in some particular cases, it achieves the remarkable shrinkage and closure of infected emphysematous bulla. The third reason is that, the patients experience minimum
pain
with the use of a small caliber drainage tube.

). We report the case of a female patient with
SLE
in whom a hepatocellular carcinoma was discovered after 4 years of treatment with danazol. Except for 3 days of hypochondrium

, there were neither clinical signs of liver tumor nor biological abnormalities. An ultrasonography showed 2 tumors of the liver. At histological examination after surgery, one of the tumors was found to be a benign adenoma, while the other was a well differentiated hepatocellular carcinoma. When longterm danazol therapy is required, ultrasonography may be useful for early tumor detection.
