The patient described in this clinical demonstration suffered from severe myasthenia gravis shortly before the detection and for 3 years after surgical removal of a cystic thymoma. At the end of this period, when the myasthenia subsided, she developed
systemic lupus erythematosus
simultaneously with pleural implantation
metastases
of the thymoma. After local radiation therapy and under systemic immunosuppression she has remained asymptomatic since the spring of 1978. The pathogenetic and immunogenetic basis of myasthenia gravis and autoimmune diseases associated with thymoma and thymus hyperplasia is reviewed, and a plea is entered for more comprehensive and integrative internal medicine.

erythematous (SLE) who developed, respectively, a breast carcinoma and a malignant melanoma 8 and 15 years after the diagnosis of

; both patients died with evidence of widespread, rapidly progressive
metastatic disease
at a time when the SLE was minimally active and did not require immunosuppressive therapy. The association of SLE and solid tumors in the same patient is reviewed. The frequency of this association appears to be low and the most often described tumors are of uterine and bladder origin. The clinical course of the solid malignancy in these patients is not always described in detail. Careful epidemiologic studies on the true incidence of solid tumors in patients with SLE are required to better understand this association.

Lupus
anticoagulant was present in this case of paraneoplastic scleroderma revealed by hemolytic-uremic syndrome, suggesting that the autoantibody played a significant role in the sequence of events leading to anuria. Reviewing the literature we found several observations of paraneoplastic scleroderma, and in other series cases of scleroderma-linked (and in rare instances cancer-linked) antiphospholipid autoantibodies. Search for antiphospholipid antibodies should be considered in patients with systemic scleroderma as well as in patients with
metastatic cancer
. Presence of such procoagulant autoantibodies might predict future complications and should influence treatment strategy.

and bilateral psoas abscess, an uncommon condition caused by septic
metastases
from ulcerous necrotic lesions involving both lower limbs. We describe the clinical features and the complementary work up, which included ultrasound. Similarly, treatment is described, as well as the results yielded by bacteriologic analyses of pus specimen. The literature is reviewed, highlighting the most important criteria.

In the last 8 years, 511 patients (267 men and 244 women) were investigated. It was found that 44 cases (8.6%) were false adenopathies (various types of tumoral masses) but placed in the nodes areas (localized, generalized or deep). There were 467 cases of true lymphadenopathies, 58 new cases yearly (2.32% of all admitted patients and 6.9% of those with blood diseases). Out of these 467 cases, 330 (70.6%) were malignant neoplastic diseases: malignant lymphomas--206 cases (62.4% of all malignancies), leukemias--99 cases (30%), carcinomatous
metastases
--25 cases (7.6%). Nonmalignant lymphadenopathies were found in 137 cases (29.4%): specific infections (tuberculosis) and nonspecific ones in 87 cases (63.5%), nonimmune diseases (
SLE
, PAN, sarcoidosis) in 50 cases. Generalized adenopathies were recorded in 47% of the cases, the involvement of a single node group in 21.8% of the cases, other types of distribution being rare. The general symptoms were absent in 20.5% of the cases, being present in the remainder of 79.5%, especially in the malignant lymphomas, leukemias, nonimmune diseases. The main complications occurring against the background of the etiological affections of lymphadenopathies were: infections (respiratory, urinary, tegumental) in 19.7% of the cases and cardiovascular disturbances (myocardiopathies, rythm and conduction disturbances) in 9.6% of the cases.

Plasma gamma-glutamyltranspeptidase (GGTP) levels were measured in 435 cancer patients, 120 healthy controls, 15 patients with systemic
lupus
erythematosis, and 10 patients with rheumatoid arthritis. The mean GGTP activity of all cancer patients studied, with the exception of malignant lymphoma, was significantly elevated compared to control values. Several patient groups were retrospectively analyzed to determine whether GGTP levels correlated with clinical status. Patients who were disease-free had GGTP levels in the normal range, whereas patients with
metastases
had elevated levels. Serially increasing GGTP levels were associated with disease progression and death. Persons who remained free of disease had serial GGTP levels within the normal range. Furthermore, decreasing levels were associated with response to therapy. These results indicate that GGTP levels may have prognostic value in various human malignancies.
